Fuel Handling Building Fire Alarm Zone - Inoperable.

This report was initiated by Incident Report 81-216.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 98% - Unit Load 1020 MWe DESCRIPTION OF OCCURRENCE:

On June 26, 1981, the control operator noticed a fuel handling building fire alarm on the fire protection annunciator was illuminated.

Since the presence of an alarm blocks any other alarms in that zone from anninciating, the minimum required channels for that zone were inoperable.

Action statement 3.3.3.6.a was entered at 0500 hours0.00579 days <br />0.139 hours <br />8.267196e-4 weeks <br />1.9025e-4 months <br />, June 26, 1981.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel error.

A fire alarm pull box in the fuel handling building was not reset properly.

ANALYSIS OF OCCURRENCE:

Technical specification 3.3.3.6.a requires:

Within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> establish a fire watch patrol to inspect the zone (s) with the inoperable instrument (s) at least once per hour, and restcre the inoperable instrument (s) to operable status within 14 days or, in lieu of any other report required by specification 6.9.1, prepare and submit a special report to the commission pursuant to specification 6.9.2 within the next 30 days out..ining the action taken, the cause of the inoperability and the plans and schedule for restoring the instrument (s) to an operable status.

CORRECTIVE ACTION

A fire watch patrol was established in the fuel handling building area in accordance with technical specification 3.3.3.6.a.

The pull box was properly reset and the fire protection annunciator was cleared.

Action statement 3.3.3.6.a was terminated at 1020 hours0.0118 days <br />0.283 hours <br />0.00169 weeks <br />3.8811e-4 months <br />, June 26, 1981.
