Individual Rod Position Indicators - 2D1 and 2D5 - Inoperable.

This report was initiated by Incident Report 81-202.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

During unit startup operations at 2318 hours0.0268 days <br />0.644 hours <br />0.00383 weeks <br />8.81999e-4 months <br />, June 21, 1981, action statement 3.1.3.2 was entered v. hen the control operator noticed the control rod bank demand indicator and the individual rod position indicators for control rods 2D1 and 2D5 were in disagreement by greater then + 12 steps.

Since with 2 individual rod position indicators inoperable per bank the limiting condition for operation was exceeded, the control rods were inserted and the unit was placed in mode 3 in accordance with the provisions of applicability state-ment 3.0.3 at 0016 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br />, June 22, 1981.

This occurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

The individual rod position indicators had drifted out of calibration.

ANALYSIS OF OCCURRENCE:

Technical specification 3.0.3 requires that when a limiting condition for operation is not met, within one hour action shall be initiated to place the unit in a mode in which the specification does not apply.

CORRECTIVE ACTION

The Performance Department recalibrated the individual rod position indicators satisfactorily and they were returned to service.
