Pressurizer Over Protection System - Channel 1 Inoperable.

This report was initiated by Incident Report 81-194.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 - Rx Power 0% - 0 MWe DESCRIPTION OF OCCURRENCE:

At 0815 hours0.00943 days <br />0.226 hours <br />0.00135 weeks <br />3.101075e-4 months <br />, June 16, 1981, leakage through 1PRl to the pressurizer relief tank (PRT) was indicated by a pressurizer relief valve high tailpipe temperature of approximately 220*F.

The Pressurizer Over Protection System (POPS) channel 1 isol'ation valve, lPR6 was closed to isolate the leakage to the PRT.

Closure of 1PR6 blocked the pressure relief path for POPS channel 1 (lPRl) and the channel was declared inoperable and action statement 3.4.9.3.a was entered.

Work order OD-941881 was issued for inspection and repair of 1PRl.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

lPR1 had developed an internal Jeakage path during normal operations.

ANALYSIS OF OCCURRENCE:

With one POPS inoperable, either restore the inoperable POPS to operable status within 7 days or depressurize and vent the RCS through a 3.14 square inch vent (s) within the next 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />, maintain the RCS in a vented condition until both POPS have been restored to operable status.

CORRECTIVE ACTION

Disassembly and insoection of lPR1 revealed steam cutting of the valve cage.

The valvo cage was replaced in kind by maintenance and lPR1 was re-assembled, satisfactory functionally tested, and returned to normal service.
