LICENSE NO. DPR-70 DOCKET NO. 50-272 REPORTABLE OCCURRENCE 81-45/03L Pursuant to the requirements of Salem Generating Station Unit No. 1 Technical Specifications, Section 6.9.1, we are submitting Licensee Event Report for Reportable Occurrence 81-45/03L.

This report is required within thirty (30) days of the occurrence.

Due to a document tracking problem at our station, this report is late.

We have revised the system to minimize the chance of recurrence.

Source Range Channel - N Inoperable CONDITIONS PRIOR TO OCCURRENCE:

At 0730 hours0.00845 days <br />0.203 hours <br />0.00121 weeks <br />2.77765e-4 months <br /> on April 29, 1981, tne Action Statement for Functional Unit 6A on Table 3.3-1 for Technical Specification 3.3.1.1 was entered due to a problem involving an intermittent high voltage to the Source Range Channel N31.

This occurrence constituted operation in a degraded mcde in accordance with Technical Specification.6.9.1.9b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The designation for the apparent cause of the occurrence is equipment failure.

The subject source range channel circuitry was functionally checked; the results of which were satisfactory and provided no indication as to what the specific cause was.

In addition, the K629 relay was resistance checked with satisfactory results.

This check also provided no indication as to what the specific cause was.

To provide for continuous monitoring, a recorder was installed in the circuit.

A review of the recorder strip charts has shown no additional voltage spikes or provided any other indication as to the source of the problem.

ANALYSIS OF OCCURRENCE:

Functional checks and troubleshooting techniques provided no real indication as to the cause or source of the intermittent high voltage to Source Range Channel N31.

Technical Specification 3.3.1.1 (Functional Unit 6A), Action Statement 4 of table 3.3.1 states that with the number of channels operable one less than required by the minimum channels operable requirement and with the thermal power level above P6, operation may continue.
