No. 11 Pressurizer Level Channel 3 Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

At 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> on April 26, 1981, Action Statement 3.3.1.1 was entered because the Pressurizer Level Channel 3 was inoperable.

A Channel Check indicated that Channel 3 was 6% high relative to Channels 1 and 2.

The bistable. relay was tripped at this time IAW Action 7.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure -

Valve 1PS8 packing leak caused decreased pressure to the Pressurizer Level Channel 3 Transmitter.

ANALYSIS OF OCCURRENCE:

Action Statement 3.3.1.1, Action 7, requires that with the number of operable channels one less than th.e total number of channels, startup and/or power operation may proceed until performance of the next required channel functional test provided the inoperable channel is placed in the tripped condition within one hour.

CORRECTIVE ACTION~

Valve 1PS8 was repacked and tested satisfactorily.

The Pressurizer Level Channel 3 instrument loop was then reset and tested satisfactorily.

Action Statement 3.3.1.1 was terminated at 2332 hours0.027 days <br />0.648 hours <br />0.00386 weeks <br />8.87326e-4 months <br /> on April 26, 1981.
