Service Water Leak in Containment CONDITIONS PRIOR TO OCCURRENCE:

While performing routine shift rounds at 1908 hours0.0221 days <br />0.53 hours <br />0.00315 weeks <br />7.25994e-4 months <br /> on April 22,

-, an operator discovered a Service Water Leak emanating from

~o. 13 Containment Fan Cooling Unit.

In accordance with NRC

  • - Bulletin 80-24, the NRC was notified by telephone at 2313 hours0.0268 days <br />0.643 hours <br />0.00382 weeks <br />8.800965e-4 months <br /> with written confirmation transmitted within the next 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

Investigation by maintenance personnel revealed that.a welded joint connecting a 3/4" nipple and drain valve 13SW64 was leaking.

This occurrence constituted a degradation of a containment boundary in accordance with Technical Specification 6.9.1.8.c.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure -

A leak was discovered in the weld connecting a carbon steel nipple to a stainless steel flange to which valve 13SW64 is attached.

ANALYSIS OF OCCURRENCE:

No Action Statement was entered because of this occurrence since the reactor was in the cold shutdown mode of operation.

CORRECTIVE ACTION

The weld was reworked requiring the nipple to be replaced in kind.
