RHR Pump Seal Failure CONDITIONS PRIOR TO OCCURRENCE:

Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

On April 4, 1981, the No. 12 RHR Pump was declared inoperable be-cause of a leak of approximately 2.5 gpm emanating from the pump's mechanical seal.

Action Statement 3.4.1.4.a was entered at 0017 hours1.967593e-4 days <br />0.00472 hours <br />2.810847e-5 weeks <br />6.4685e-6 months <br />.

The pump was repaired, tested and returned to service at 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br /> on April 13, 1981.

At 0300 hours0.00347 days <br />0.0833 hours <br />4.960317e-4 weeks <br />1.1415e-4 months <br /> on April 14, 1981, the pump was again declared inoperable because of a leaking seal and Action Statement 3.4.1.4.a was re-entered at this time.

A new set of "O" rings was installed and the pump was declared operational at 1445 hours0.0167 days <br />0.401 hours <br />0.00239 weeks <br />5.498225e-4 months <br /> on April 16, 1981.

This occurrence constituted operation in a degraded mode I.AW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The pump's mechanical seal had failed.

ANALYSIS OF OCCURRENCE:

Action Statement 3.4.1.4.a requires that with less than two residual heat removal loops operable, immediately initiate corrective action to return the required loops to operable status as soon as possible.

When the pump was declared inoperable on April 14, 1981, it was disassembled and inspected.

No obvious equipment failure was evident.

However, it is believed that one or both "O" rings had become twisted during the assembly of the pump on April 13, 1981.

New "O" rings were installed, the pump was reassembled, tested satisfactorily and declared operational at 1445 hours0.0167 days <br />0.401 hours <br />0.00239 weeks <br />5.498225e-4 months <br /> on April 16 1981.

CORRECTIVE ACTION

A new mechanical seal consisting of the shaft sleeve, shaft sleeve "O" rings, throttle bushing assembly and circulating ring assembly was installed on the pump.

A satisfactory surveillance test was performed and the Action Statement terminated at 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br /> on April 13, 1981.

When the pump was declared inoperable on April 14, 1981, new "O" rings were installed.
