Inoperable Fire Hose CONDITIONS PRIOR TO OCCURRENCE:

At 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> on April 5, 1981, Fire Hose No. lFP-75 located on Elevation 100' (100-Al-NN14) was discovered to be inoperable because of a 6" slash along its longitudinal axis.

Action State-ment 3.7.10.4 was entered at this time.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Apparently, the slashed hose was the result of malicious mischief or vandalism by person or persons unknown.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.10.4 requires that with this hose inoperable, route an additional equivalent capacity fire hose to the unprotected area(s) from an operable hose station within one hour.

Operations personnel duplicated the cut in the hose using a 4" bladed heavy duty pocket knife which suggests a deliberate intent to damage the hose.

CORRECTIVE ACTION

The fire hose was replaced in kind.
