LICENSE NO. DPR-70 DOCKET NO. 50-272 REPORTABLE OCCURRENCE 81-34/03L During further examination of leakage in the Spent Fuel Pit during the installation of high density fuel racks, and in keeping with the philosophy of a previous report, it was determined that this event was reportable.

Therefore, pursuant to the requirements of Salem Generating Station Unit No. 1 Technical Specifications, Section 6.9.1, we are submitting Licensee Event Report for Reportable Occurrence 81-34/03L.

Leak in Spent Fuel Pit CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 -

Rx Power 100% - Unit Load 1130 MW DESCRIPTION OF OCCURRENCE:

During an inspection of the Sp8nt Fuel Pit, a leak of approximately 2.7 liters/hour was discovered at telltale drain number 6.

The Spent Fuel Pit level has been maintained at 23 feet of water over the top of the irradiated fuel assemblies seated in the storage racks in accordance with Technical Specification 3.9.11 since the occurrence.

This occurrence constituted an abnormal degradation of a system designed to contain radioactive material resulting from the fission process IAW Technical Specification 6.9.1.9.d.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The apparent cause of the leak was leaking welded seams.

ANALYSIS OF OCCURRENCE:

Various non-destructive examinations were utilized to determine the cause of the leakage.

Four areas with weld flow indications were identified:

one 3" defect in a side wall: three 1/2" defects in the floor seams.

CORRECTIVE ACTION

A stainless steel plate was welded over the defect in the side wall.

Epoxy was used to seal the floor seams.

Average leakage is now approximately 30 ml/hr.

Inspection of the remaining seams is planned when the fuel racks are relocated.

A supplemental report will address the results of the inspection and any corrective action taken.
