Containment Cooling System -

No. 12 Fan Coil Unit CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 -

Rx Power 100% - Unit Load 1130 MW DESCRIPTION OF OCCURRENCE:

During normal operation on March 24, 1981, the No. 12 Containment Fan Coil Unit was declared inoperable in order to replace a leaking nipple (- 1 gpm) in the service water outlet drain line of the unit.

Action Statement 3.6.2.3 was entered at 1615 hours0.0187 days <br />0.449 hours <br />0.00267 weeks <br />6.145075e-4 months <br />.

IAW NRC IE Bulletin 80-24, the NRC was notified of this event at 1634 hours0.0189 days <br />0.454 hours <br />0.0027 weeks <br />6.21737e-4 months <br />.

This occurrence constituted abnormal degradation of the containment boundary IAW Technical Specification 6.9.1.8.c.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure -

A pinhole leak was discovered in the weld connecting the carbon steel nipple to a stainless steel coupling in the unit's outlet drain line.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3 requires that with one group of Containment Cooling Fans inoperable, restore the inoperable fan group to operable status within seven days or be in hot standby within the next six hours and in cold shutdown within the following thirty hours.

CORRECTIVE ACTION

The weld was reworked requiring the nipple to be replaced in kind.
