Auxiliary Feedwater Storage Tank (AFWST)

< 200,000 Gallons CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 -

Rx Power 3% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

While increasing power level during startup, it was discovered that the level in the AFWST was below the minimum level (200,000 gallons) required by Technical Specification 3.7.1.3.

Action Statement 3.7.1.3 was entered at 1018 hours0.0118 days <br />0.283 hours <br />0.00168 weeks <br />3.87349e-4 months <br /> which requires the level to be restored within four hours or be in Hot Shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel Error -

The Control Operator had given instructions to open the supply valve, but not in sufficient time to prevent the temporary loss of required level in the tank.

ANALYSIS OF OCCURRENCE:

In the process of using the AFWST as a makeup supply of water to the steam generators, the supply valve for the AFWST was throttled too far to compensate for the water removed.

CORRECTIVE ACTION

The tank was refilled to > 200,000 gallons of stored water and the Action Statement was terminated at 1040 hours0.012 days <br />0.289 hours <br />0.00172 weeks <br />3.9572e-4 months <br />.

The Control Operator has been counseled for his error in judgment.
