SMALL STEAM LEAKS - - - (UPDATE)

On January 2,1987, the licensee discovered three small steam leaks on two of the four main steam lines. -The leaks were from broken spare instrument lines located between the turbine.

control valves and-the main turbine itself. (See PN0-III-87-01.)-

Thn. instrument line failures were at a bimetallic weld connecting the instrument tubing to a valve. The licensee plugged, capped and welded the broken lines.

The licensee identified five other instrument lines having this same bimetallic weld connection.

'Of thsse.five lines, three are spare and two are-in-service providing steam pressure indication to the control room. ' The-licensee plugged, capped and welded the three spare lines. The two in-ssrvice lines were visually and liquid penetration inspected satisfactorily. The licensee completed repairs.and returned the steam lines to service on January 5~,1987.

.At n:on on January 5, the licensee synchronized to the grid. Subsequently, the licensee

-id;ntified a cracked weld on another instrument line/ valve connection and six small packing 1caks. The turbine has been taken off-line and the licensee is performing repairs.

Additionally, the licensee observed five missing or loose connectors on. snubber brackets associated with the steam lines downstream of the stop' valves. The licensee has initiated shift observations in the area of-the failed instrument lines and the loose connectors. The

. instrument line failures may be attributed to vibration induced fatigue. This same vibration ,

' may have caused the loose connectors. '
