Dear Mr. Tailleart:

Enclosed please find a copy of the Final After Action Report/Improvement Plan for the FullParticipation Exercise for the 10-Mile Emergency Planning Zone (EPZ) around the IndianPoint Energy Center held on October 7,2014. Also included are the evaluations of 48 outof sequence drills and demonstrations held in the 2-year cycle associated with this exercise.

New York State and local governments successfully demonstrated the capability toimplement their off-site radiological emergency response plans and procedures, based on,the evaluation of this exercise by the FEMA Region II Regional Assistance Committee and a team of Federal evaluators.

There were no Deficiencies as a result of this exercise.

There were no new Areas Requiring Corrective Actions (ARCAS) identified; four ARCAs were corrected on the spot by re-demonstration.

In addition, two prior ARCAS were resolved by demonstration.

Therefore, we find that State and local preparedness is adequate to protect the health andsafety of the public living in the vicinity of the Indian Point Energy Center and there isreasonable assurance that appropriate measures can be taken offsite in the event of aradiological emergency.

The exercise was evaluated by the Federal Emergency Management Agency (FEMA),Region II. The purpose of the exercise was to demonstrate and assess the level of State and localpreparedness in responding to a radiological emergency.

The exercise was held in accordance with FEMA's policies and guidance concerning the exercise of State and local radiological emergency response plans (RERP) and procedures.

The most recent previous exercise at this site was conducted on October 2, 2012. The qualifying emergency preparedness exercise was conducted on March 3, 1982.FEMA wishes to acknowledge the efforts of the many individuals in New York State;Westchester,

Rockland, Orange, and Putnam Counties in New York; and Bergen County, NewJersey who participated in this exercise.

Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought thisresponsibility by volunteering to provide vital emergency services to their communities.

Thecooperation and teamwork of all participants were evident during this exercise.

This report contains the draft evaluation of the biennial exercise and of the following out-of-sequence activities in Orange, Putnam, Rockland and Westchester counties:

Day Cares andSchool Interviews; Reception Centers; General and Special Population Bus Companies; SchoolBus Companies; Traffic Control Points; and Medical Services Drills.The State and local organizations, except where noted in this report, satisfactorily demonstrated knowledge of their emergency response plans and procedures and adequately implemented them.There were no Deficiencies and no new Areas Requiring Corrective Actions (ARCAS)identified; four ARCAs was corrected on the spot by re-demonstration.

The purpose of this report is to present the results of the findings on theperformance of the offsite response organizations during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities Capabilities-based planning allows for exercise planning teams to develop exercise objectives and observe exercise outcomes through a framework of specific action items that were derivedfrom the Target Capabilities List (TCL). The capabilities listed below form the foundation forthe organization of all FEMA Region II REP Program objectives and observations in thisexercise.

-Emergency Operations Center (EOC) Management:

Is the capability to provide multi-agency coordination (MAC) for incident management by activating and operating an EOC for a pre-planned or no-notice event. EOC management includes EOC activation, notification,

staffing, and deactivation; management, direction,
control, and coordination of response and recoveryactivities; coordination of efforts among neighboring governments at each level and among local,regional, state, and federal EOCs; coordination of public information and warning; andmaintenance of the information and communication necessary for coordinating response andrecovery activities.

-Emergency Public Information and Warning:

Is the capability that includes public information, alert/warning and notification.

It involves developing, coordinating, and disseminating information to the public, coordinating officials, and incident management and responders acrossall jurisdictions and disciplines effectively under all hazard conditions.

-Citizen Evacuation and Shelter in Place: Is the capability to prepare for, ensure communication of, and immediately execute the safe and effective sheltering-in-place of an at-risk population (and companion animals),

and/or the organized and managed evacuation of the at-risk population 16 Unclassified Radiological Emergency Preparedness Program (REP)After Action Report/Improvement Plan Indian Point Energy Center(and companion animals) to areas of safe refuge in response to a potentially or actuallydangerous environment.

In addition, this capability involves the safe reentry of the population where feasible.

-Emergency Public Safety and Security Response:

Is the capability to reduce the impact andconsequences of an incident or major event by securing the affected area, including crime/incident scene preservation issues as appropriate, safely diverting the public from hazards,providing security support to other response operations and properties, and sustaining operations from response through recovery.

Public Safety and Security Response requires coordination among officials from law enforcement (LE), fire, and Emergency Medical Services (EMS).-Hazardous Materials Response and Decontamination:

Is the capability to assess and manage theconsequences of a hazardous materials

release, either accidental or as part of a terrorist attack. Itincludes testing and identifying all likely hazardous substances onsite; ensuring that responders have protective clothing and equipment; conducting rescue operations to remove affected victimsfrom the hazardous environment; conducting geographical survey searches of suspected sourcesof contamination and establishing isolation perimeters; mitigating the effects of hazardous materials, decontaminating on-site victims, responders, and equipment; coordinating off-sitedecontamination with relevant
agencies, and notifying environmental, health, and lawenforcement agencies having jurisdiction for the incident to begin implementation of theirstandard evidence collection and investigation procedures.
  • Mass Care: Is the capability to provide immediate
shelter, feeding centers, basic first aid, bulkdistribution of needed items, and related services to persons affected by a large-scale
incident, including special needs populations.

Special needs populations include individuals with physicalor mental disabilities who require medical attention or personal care beyond basic first aid. Otherspecial-needs populations include non-English speaking populations that may need to haveinformation presented in other languages.

The mass care capability also provides for petcare/handling through local government and appropriate animal-related organizations.

Mass careis usually performed by nongovernmental organizations (NGO), such as the American Red Cross(ARC), or by local government-sponsored volunteer

efforts, such as Citizen Corps. Special-needs populations are generally the responsibility of local government, with medical needsaddressed by the medical community and/or its alternate care facilities.

State and Federal entitiesalso play a role in public and environmental health by ensuring safe conditions, safe food,potable water, sanitation, clean air, etc.17 Unclassified Radiological Emergency Preparedness Program (REP)After Action Report/Improvement Plan Indian Point Energy Center* Triage and Pre-Hospital Treatment:

Is the capability to appropriately dispatch EMS resources; to provide feasible,

suitable, and medically acceptable pre-hospital triage and treatment ofpatients; to provide transport as well as medical care en-route to an appropriate receiving facility; and to track patients to a treatment facility.

2.3 Scenario Summary-0800 hours: Start of ExerciseUnit 2 is at 100 % power.Unit 3 is at 100 % power.Indian Point Energy Center (IPEC) badged contractors (ABC Diving Company's divers) arecurrently cleaning the Unit 1 trash racks. The diver is currently in the water and the diver tenderis on the Unit I Wharf monitoring the air-lines and air manifold for the diver. The air compressor supplying the manifold is sitting adjacent to the fence line by the Service Water (SW) pumpswith air hoses running through the Protected Area (PA) fence to the Unit 1 Wharf divinglocation.

The Security Officer at the front gate is only able to see adriver. The van turns right heading in the direction that tractor trailers normally proceed.

As theydrive past the Security Officer at the front gate, the sliding back door on the driver's side slidesopen and a gunman with a machine gun engages in gun fire with the Security Officer at the frontgate.-0820 hours: The Security Officer at the front gate manages to get down on the floor of theGuard Station as bullets fly overhead.

He was able to see two (2) occupants in the back seatwhen the door opened and radios into Central Alarm Station/

Secondary Alarm Station(CAS/SAS) about the mini-van and its occupants.

The Dodge Caravan hits the gas and speedsinto the tractor trailer inspection lanes, crashing through the barricade proceeding west on KennyLane onto the site.The IPEC Security Shift Supervisor contacts the Unit 2/3 CCRs and reports that armed intruders have gained entry into the Owner Controlled Area (OCA) and a Hostile Action is occurring.

-0821 hours: As the Dodge Caravan drives down Kenny Lane, IPEC Security officers in avehicle traveling South on Overlook Drive (who heard the radio communications) park theirvehicle across Overlook Drive adjacent to the pop-up barriers and take cover behind the concretewalls next to the Unit 2 Steam Generator (S/G) mausoleum.

The IPEC officers engage in gunfirewith the vehicle, flattening both passenger side tires on the mini-van.

The sliding back door onthe passenger side slides open and a second gunman returns fire with the Security Officers.

TheIPEC Security officers neutralize the mini-van passenger side gunman.-0822 hours: The driver of the Dodge Caravan loses control of the vehicle as the rubber of thepassenger side tires comes off and the mini-van attempts to continue driving on the rims. Themini-van veers sharply left and goes off the side of the road and into the telephone pole at thetree line adjacent to the Salt Barn. The IPEC Security Officers see 2 personnel exit the crashedmini-van on the driver's side and utilizing tree cover run towards the Salt Barn. The SecurityOfficers are unable to get a clear shot of the 2 running intruders, but maintain "Eyes On" themini-van passenger side armed intruder who lays motionless hanging out of the mini-vanpassenger side rear sliding door. One armed intruder has been neutralized.

-0823 hours: The IPEC Security Officers continue to engage in gunfire with the 2 armedintruders while they are running into the Salt Barn. One of the two armed intruders is neutralized 19 Unclassified Radiological Emergency Preparedness Program (REP)After Action Report/Improvement Plan Indian Point Energy Centerwhile running towards the building.

The driver of the mini-van is able to take cover inside theSalt Barn. IPEC Security Officers have "Eyes On" the second neutralized armed intruder nowlying on the driveway of the Salt Barn. Two armed intruders have now been neutralized.

Both Unit 2 and Unit 3 reactors are successfully manually tripped in response to the hostileaction occurring on site.-0827 hours: A loud explosion is heard and felt and the Salt Barn erupts in flames sendingshrapnel from the butler building type walls into the surrounding areas. No IPEC SecurityOfficers are injured during the explosion, but they are able to see the body of the 3rd armedintruder lying motionless on the ground outside of the Salt Barn. All three armed intruders whoentered the site thru the front gate have now been neutralized.

Light grey smoke slowly risesfrom the building and blows directly eastward towards Buchanan and Reuter-Stokes (R/S)Monitor #6.-0835 hours: The Unit 2 Shift Manager declares an Alert based on EAL HA4.1 "A hostile actionis occurring or has occurred within the Owner Controlled Area as reported by the Security ShiftSupervisor."

The IPEC Emergency Response Organization (ERO) are notified and instructed toreport to alternate facilities.

-0836 hours: Two (2) small explosions are heard and grey smoke is seen slowly rising behindthe Unit 3 Waterfront warehouse building.

Security (CAS/SAS) is able to see on their camerasholes blown in the OCA and PA fence lines behind the Unit 3 Waterfront Warehouse and 5 -armed intruders are scrambling up the hill and penetrating the OCA fence line, PA fence line andentering a hole that has been cut in the back of the Unit 3 Waterfront warehouse butler stylebuilding.

The intruders were coming from the direction of the Gypsum Plant adjacent to thesouthern side of the IPEC site.The IPEC Security Shift Supervisor (or CAS/SAS) contacts the Unit 2/3 CCR's and reports thatarmed intruders have gained entry into the Protected Area and a Hostile Action is occurring.

-0843 hours: Decoy smoke bombs are deployed outside of the door to the Unit 3 waterfront warehouse.

Three (3) armed intruders are seen in the large white smoke cloud running towardsthe field enclosure that contains the power for the Security fence, just out front and East of theWarehouse building.

An IPEC Security Officer from the Unit 3 Admin Building Roof engages20 Unclassified Radiological Emergency Preparedness Program (REP)After Action Report/Improvement Plan Indian Point Energy Centerwith and neutralizes all 3 intruders.

The IPEC Security Officer from the Unit 3 Admin BuildingRoof reports that he has "Eyes On" all three (3) motionless armed intruders on the blacktopoutside the Unit 3 waterfront warehouse.

-0844 hours: An IPEC Security vehicle inside of the PA fence line arrives at the Unit 3waterfront warehouse.

An IPEC Security Officer exits the vehicle, and taking cover behind alarge excavation dirt pile in the Unit 3 waterfront warehouse parking lot and takes aim at thefront entrances of the building, essentially pinning any additional suspects inside of the building.

The IPEC Security Officer reports that he also has "Eyes On" all 3 motionless bodies in front ofthe Unit 3 waterfront warehouse.

-0848 hours: Security personnel outside of the Unit 2 Salt Barn report to CAS/SAS that the firein the Unit 2 Salt Barn appears to be out and no other assistance is required.

-0850 hours: An Incident Command Post (ICP) is established.

Unit 2 CCR completed Alert Part1 notifications to off site, including selecting "No Release to atmosphere" on the Part 1 Form.-0851 hours: The Unit 2 Shift Manager declares a Site Area Emergency based on EAL HS4.1"A hostile action is occurring or has occurred within the Protected Area as reported by theSecurity Shift Supervisor".

-0855 hours: While patrolling areas inside of the road blocks set up around the IPEC site, StatePolice radio back to ICP reports that there is a suspicious vehicle across the street from IPEC,inside the Buchanan Switchyard.

The Gate to the switchyard is open and no personnel seem to bearound the vehicle.

The vehicle is partially obscured from the road and went previously unnoticed.

The vehicle is a grey 2014 Dodge Caravan with heavily tinted windows similar to thevehicle that was reported earlier at the IPEC site.-0900 hours: A bomb located inside the grey 2014 Dodge Caravan inside the BuchananSwitchyard detonates and explodes.

The mini-van blows up and disintegrates, taking out all off-site power to the IPEC site.Unit 2 experiences total loss of off-site power. 21/22/23 EDGs all start and run normally with noissues. Unit 3 experiences a total loss of off-site power. 31/32/33 EDGs all start and run normallywith no issues.21 Unclassified Radiological Emergency Preparedness Program (REP)After Action Report/Improvement Plan Indian Point Energy Center-0906 hours: Unit 2 CCR Completed Site Area Emergency (SAE) Part 1 notifications to off site,including selecting "No Release to atmosphere" on the Part 1 Form.-0935 hours: ERO facilities are staffed and operational (excluding Joint Information Center) toinclude:

Alternate Emergency Operations Facility (AEOF), Alternate Operations Support Center(OSC)/Technical Support Center (TSC) and IPEC Liaisons at the Alternate Incident CommandPost (ICP).-0940 hours: Tactical teams enter the Unit 3 waterfront warehouse.

All three (3) armed intruders take cover behind warehouse shelving units and two (2) armed intruders provide cover fire byengaging with the State Police (SP) while the third armed intruder sets the stored warehouse items in the building on fire. As the fire blazes in the building, all three (3) armed intruders areneutralized.

-0945 hours: Two sequential loud explosions are heard. A section of the metal roof of the Unit IScreenwell house is blown off. The second explosion appears to have been adjacent to theSW pumps fence line and SW pumps 21/22 (essential SW Header) appear to be lost. The Unit 2CCR immediately realizes that they will have to secure EDGs due to loss of cooling water to theEDGs. The Unit 2 CCR will attempt to quickly devise a plan to try and save the EDGs if at allpossible.

Unit 2 continues to maintain core cooling (secondary heat removal) via 21/23 Auxiliary BoilerFeed Pumps (ABFPs) which are currently being powered from the EDGs and 22 ABFP which issteam driven, all supplied water from the Condensate Storage Tanks (CSTs).-0945 hours: IPEC Security in CAS/SAS are able to see on the cameras one body (Unit 1 diverbuddy) laying under a section of the Unit 1 Screenwell House roof on the Unit 1 Wharf and asecond body in a diver's suit floating in the Hudson River just adjacent to the Unit 2 IntakeStructure floating dead-man style. The Unit 1 Screenwell House is engulfed in flames. Greysmoke slowly rises from the building and blows directly eastward towards Buchanan and R/SMonitor #6.-0948 hours: 21/22/23 EDGs sequentially trip off-line due to overheating with no SW coolingfrom the essential SW header (or the Unit 2 CCR may choose to unload and trip EDG Output22 Unclassified Radiological Emergency Preparedness Program (REP)After Action Report/Improvement Plan Indian Point Energy Centerbreakers

instead, either is acceptable for purposes of the drill).Due to the electrical transient on the busses from tripping the EDGs, the 120v Solenoid valvePCV- 131 OA (ABFP 22 Steam Supply Pressure Control Valve) goes shut and 22 ABFP trips off-line. Unit 2 has now lost all core cooling (secondary heat removal) with the loss of 21/23 ABFPswhich were being powered from the EDGs prior to the loss of EDGs and the loss of 22 ABFP.(This is the Time = 0 for the classification of the GE)-1003 hours: The Emergency Director declares a General Emergency (GE) based on EALHG4.1 "A hostile action has occurred such that plant personnel are unable to operate equipment required to maintain safety functions OR A hostile action has caused failure of Spent FuelCooling Systems and imminent fuel damage is likely."

(Unit 2 has lost secondary heat sink whenthe power [EDG's] to the 2 operating ABF Pumps was lost and the third ABFP tripped off withno indication of when Unit 3 can start and bring power over from the Unit 3 Appendix R EDG toprovide additional secondary heat sink via the ABF Pumps again).-1010 hours: CAS reports to Unit 2 CCR that from their cameras they can see that the fire in theUnit 1 Screenwell House appears to be out. The fire in the Unit 3 waterfront warehouse continues to burn out of control.-1018 hours: AEOF has completed GE Part 1 notifications to off site including selecting "NoRelease to atmosphere" on the Part 1 Form.-1035 hours: The Joint Information Center (JIC) is manned and is preparing Press Releases andMedia Briefings.

Note: This time for the JIC can change based on the time of declaration of the Alert.-1040 hours: Security/LLEA/State Police escorts Operations personnel as required for the eventsthat unfolded during the event. Unit 2 CCRs priorities to the ICP should be:1. Possible misunderstanding of the difference between GE,local State of Emergency and protective actions as Orange County was in theEmergency Planning Zone and affected by the GE, but had no protective actions atthe time information was provided that "Orange County was not in a GeneralEmergency."

These images areprovided to the law enforcement individual through Google Earth or emailed via theircommunication systems.ISSUE NO.: 32-12-3d2-A-04 ISSUE: The Orange County Public Information Officer was not informed of thealternate evacuation route as a result of a traffic impediment (simulated rollover of atractor trailer carrying milk) at 12:22.
