Enclosed please find a copy of the Final After Action Report/Improvement Plan for the Full Participation Exercise for the 10-Mile Emergency Planning Zone (EPZ) around the Indian Point Energy Center held on October 7,2014. Also included are the evaluations of 48 out of sequence drills and demonstrations held in the 2-year cycle associated with this exercise.

New York State and local governments successfully demonstrated the capability to implement their off-site radiological emergency response plans and procedures, based on

,the evaluation of this exercise by the FEMA Region II Regional Assistance Committee and a team of Federal evaluators.

There were no Deficiencies as a result of this exercise. There were no new Areas Requiring Corrective Actions (ARCAS) identified; four ARCAs were corrected on the spot by re-demonstration. In addition, two prior ARCAS were resolved by demonstration.

Therefore, we find that State and local preparedness is adequate to protect the health and safety of the public living in the vicinity of the Indian Point Energy Center and there is reasonable assurance that appropriate measures can be taken offsite in the event of a radiological emergency.

SUMMARY

On October 7, 2014, a hostile action-based (HAB) exercise was conducted in the 10-mile plume exposure pathway emergency planning zone (EPZ) around the Indian Point Energy Center (IPEC). The exercise was evaluated by the Federal Emergency Management Agency (FEMA),

Region II. The purpose of the exercise was to demonstrate and assess the level of State and local preparedness in responding to a radiological emergency. The exercise was held in accordance with FEMA's policies and guidance concerning the exercise of State and local radiological emergency response plans (RERP) and procedures.

The most recent previous exercise at this site was conducted on October 2, 2012. The qualifying emergency preparedness exercise was conducted on March 3, 1982.

FEMA wishes to acknowledge the efforts of the many individuals in New York State; Westchester, Rockland, Orange, and Putnam Counties in New York; and Bergen County, New Jersey who participated in this exercise.

Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities. The cooperation and teamwork of all participants were evident during this exercise.

This report contains the draft evaluation of the biennial exercise and of the following out-of-sequence activities in Orange, Putnam, Rockland and Westchester counties: Day Cares and School Interviews; Reception Centers; General and Special Population Bus Companies; School Bus Companies; Traffic Control Points; and Medical Services Drills.

The State and local organizations, except where noted in this report, satisfactorily demonstrated knowledge of their emergency response plans and procedures and adequately implemented them.

SUMMARY

2.1 Exercise Purpose and Design The DHS/FEMA Region II Office evaluated the full participation, hostile action-based exercise on October 7, 2014 to assess the capabilities of the local emergency preparedness organizations in implementing their Radiological Emergency Response Plans and Procedures to protect the public health and safety during a radiological emergency involving the Indian Point Energy Center (IPEC). The purpose of this report is to present the results of the findings on the performance of the offsite response organizations during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities Capabilities-based planning allows for exercise planning teams to develop exercise objectives and observe exercise outcomes through a framework of specific action items that were derived from the Target Capabilities List (TCL). The capabilities listed below form the foundation for the organization of all FEMA Region II REP Program objectives and observations in this exercise.

-Emergency Operations Center (EOC) Management: Is the capability to provide multi-agency coordination (MAC) for incident management by activating and operating an EOC for a pre-planned or no-notice event. EOC management includes EOC activation, notification, staffing, and deactivation; management, direction, control, and coordination of response and recovery activities; coordination of efforts among neighboring governments at each level and among local, regional, state, and federal EOCs; coordination of public information and warning; and maintenance of the information and communication necessary for coordinating response and recovery activities.

- Emergency Public Information and Warning: Is the capability that includes public information, alert/warning and notification. It involves developing, coordinating, and disseminating information to the public, coordinating officials, and incident management and responders across all jurisdictions and disciplines effectively under all hazard conditions.

- Citizen Evacuation and Shelter in Place: Is the capability to prepare for, ensure communication of, and immediately execute the safe and effective sheltering-in-place of an at-risk population (and companion animals), and/or the organized and managed evacuation of the at-risk population 16

After Action Report/Improvement Plan Indian Point Energy Center (and companion animals) to areas of safe refuge in response to a potentially or actually dangerous environment. In addition, this capability involves the safe reentry of the population where feasible.

- Emergency Public Safety and Security Response: Is the capability to reduce the impact and consequences of an incident or major event by securing the affected area, including crime/incident scene preservation issues as appropriate, safely diverting the public from hazards, providing security support to other response operations and properties, and sustaining operations from response through recovery. Public Safety and Security Response requires coordination among officials from law enforcement (LE), fire, and Emergency Medical Services (EMS).

- Hazardous Materials Response and Decontamination: Is the capability to assess and manage the consequences of a hazardous materials release, either accidental or as part of a terrorist attack. It includes testing and identifying all likely hazardous substances onsite; ensuring that responders have protective clothing and equipment; conducting rescue operations to remove affected victims from the hazardous environment; conducting geographical survey searches of suspected sources of contamination and establishing isolation perimeters; mitigating the effects of hazardous materials, decontaminating on-site victims, responders, and equipment; coordinating off-site decontamination with relevant agencies, and notifying environmental, health, and law enforcement agencies having jurisdiction for the incident to begin implementation of their standard evidence collection and investigation procedures.

  • Mass Care: Is the capability to provide immediate shelter, feeding centers, basic first aid, bulk distribution of needed items, and related services to persons affected by a large-scale incident, including special needs populations. Special needs populations include individuals with physical or mental disabilities who require medical attention or personal care beyond basic first aid. Other special-needs populations include non-English speaking populations that may need to have information presented in other languages. The mass care capability also provides for pet care/handling through local government and appropriate animal-related organizations. Mass care is usually performed by nongovernmental organizations (NGO), such as the American Red Cross (ARC), or by local government-sponsored volunteer efforts, such as Citizen Corps. Special-needs populations are generally the responsibility of local government, with medical needs addressed by the medical community and/or its alternate care facilities. State and Federal entities also play a role in public and environmental health by ensuring safe conditions, safe food, potable water, sanitation, clean air, etc.

  • Triage and Pre-Hospital Treatment: Is the capability to appropriately dispatch EMS resources; to provide feasible, suitable, and medically acceptable pre-hospital triage and treatment of patients; to provide transport as well as medical care en-route to an appropriate receiving facility; and to track patients to a treatment facility.

2.3 Scenario Summary

-0800 hours: Start of Exercise Unit 2 is at 100 % power.

Unit 3 is at 100 % power.

Indian Point Energy Center (IPEC) badged contractors (ABC Diving Company's divers) are currently cleaning the Unit 1 trash racks. The diver is currently in the water and the diver tender is on the Unit I Wharf monitoring the air-lines and air manifold for the diver. The air compressor supplying the manifold is sitting adjacent to the fence line by the Service Water (SW) pumps with air hoses running through the Protected Area (PA) fence to the Unit 1 Wharf diving location. The diving coordinator is currently in the Unit 3 waterfront warehouse getting some needed supplies.

The van turns right heading in the direction that tractor trailers normally proceed. As they drive past the Security Officer at the front gate, the sliding back door on the driver's side slides open and a gunman with a machine gun engages in gun fire with the Security Officer at the front gate.

-0820 hours: The Security Officer at the front gate manages to get down on the floor of the Guard Station as bullets fly overhead. He was able to see two (2) occupants in the back seat when the door opened and radios into Central Alarm Station/ Secondary Alarm Station (CAS/SAS) about the mini-van and its occupants. The Dodge Caravan hits the gas and speeds into the tractor trailer inspection lanes, crashing through the barricade proceeding west on Kenny Lane onto the site.

The IPEC Security Shift Supervisor contacts the Unit 2/3 CCRs and reports that armed intruders have gained entry into the Owner Controlled Area (OCA) and a Hostile Action is occurring.

-0821 hours: As the Dodge Caravan drives down Kenny Lane, IPEC Security officers in a vehicle traveling South on Overlook Drive (who heard the radio communications) park their vehicle across Overlook Drive adjacent to the pop-up barriers and take cover behind the concrete walls next to the Unit 2 Steam Generator (S/G) mausoleum. The IPEC officers engage in gunfire with the vehicle, flattening both passenger side tires on the mini-van. The sliding back door on the passenger side slides open and a second gunman returns fire with the Security Officers. The IPEC Security officers neutralize the mini-van passenger side gunman.

-0822 hours: The driver of the Dodge Caravan loses control of the vehicle as the rubber of the passenger side tires comes off and the mini-van attempts to continue driving on the rims. The mini-van veers sharply left and goes off the side of the road and into the telephone pole at the tree line adjacent to the Salt Barn. The IPEC Security Officers see 2 personnel exit the crashed mini-van on the driver's side and utilizing tree cover run towards the Salt Barn. The Security Officers are unable to get a clear shot of the 2 running intruders, but maintain "Eyes On" the mini-van passenger side armed intruder who lays motionless hanging out of the mini-van passenger side rear sliding door. One armed intruder has been neutralized.

-0823 hours: The IPEC Security Officers continue to engage in gunfire with the 2 armed intruders while they are running into the Salt Barn. One of the two armed intruders is neutralized 19

IPEC Security Officers have "Eyes On" the second neutralized armed intruder now lying on the driveway of the Salt Barn. Two armed intruders have now been neutralized.

Both Unit 2 and Unit 3 reactors are successfully manually tripped in response to the hostile action occurring on site.

-0827 hours: A loud explosion is heard and felt and the Salt Barn erupts in flames sending shrapnel from the butler building type walls into the surrounding areas. No IPEC Security Officers are injured during the explosion, but they are able to see the body of the 3rd armed intruder lying motionless on the ground outside of the Salt Barn. All three armed intruders who entered the site thru the front gate have now been neutralized. Light grey smoke slowly rises from the building and blows directly eastward towards Buchanan and Reuter-Stokes (R/S)

-0835 hours: The Unit 2 Shift Manager declares an Alert based on EAL HA4.1 "A hostile action is occurring or has occurred within the Owner Controlled Area as reported by the Security Shift Supervisor." The IPEC Emergency Response Organization (ERO) are notified and instructed to report to alternate facilities.

-0836 hours: Two (2) small explosions are heard and grey smoke is seen slowly rising behind the Unit 3 Waterfront warehouse building. Security (CAS/SAS) is able to see on their cameras holes blown in the OCA and PA fence lines behind the Unit 3 Waterfront Warehouse and 5 -

armed intruders are scrambling up the hill and penetrating the OCA fence line, PA fence line and entering a hole that has been cut in the back of the Unit 3 Waterfront warehouse butler style building. The intruders were coming from the direction of the Gypsum Plant adjacent to the southern side of the IPEC site.

The IPEC Security Shift Supervisor (or CAS/SAS) contacts the Unit 2/3 CCR's and reports that armed intruders have gained entry into the Protected Area and a Hostile Action is occurring.

-0843 hours: Decoy smoke bombs are deployed outside of the door to the Unit 3 waterfront warehouse. Three (3) armed intruders are seen in the large white smoke cloud running towards the field enclosure that contains the power for the Security fence, just out front and East of the Warehouse building. An IPEC Security Officer from the Unit 3 Admin Building Roof engages 20

-0844 hours: An IPEC Security vehicle inside of the PA fence line arrives at the Unit 3 waterfront warehouse. An IPEC Security Officer exits the vehicle, and taking cover behind a large excavation dirt pile in the Unit 3 waterfront warehouse parking lot and takes aim at the front entrances of the building, essentially pinning any additional suspects inside of the building.

The IPEC Security Officer reports that he also has "Eyes On" all 3 motionless bodies in front of the Unit 3 waterfront warehouse.

-0848 hours: Security personnel outside of the Unit 2 Salt Barn report to CAS/SAS that the fire in the Unit 2 Salt Barn appears to be out and no other assistance is required.

-0850 hours: An Incident Command Post (ICP) is established. Unit 2 CCR completed Alert Part 1 notifications to off site, including selecting "No Release to atmosphere" on the Part 1 Form.

-0851 hours: The Unit 2 Shift Manager declares a Site Area Emergency based on EAL HS4.1 "A hostile action is occurring or has occurred within the Protected Area as reported by the Security Shift Supervisor".

-0855 hours: While patrolling areas inside of the road blocks set up around the IPEC site, State Police radio back to ICP reports that there is a suspicious vehicle across the street from IPEC, inside the Buchanan Switchyard. The Gate to the switchyard is open and no personnel seem to be around the vehicle. The vehicle is partially obscured from the road and went previously unnoticed. The vehicle is a grey 2014 Dodge Caravan with heavily tinted windows similar to the vehicle that was reported earlier at the IPEC site.

-0900 hours: A bomb located inside the grey 2014 Dodge Caravan inside the Buchanan Switchyard detonates and explodes. The mini-van blows up and disintegrates, taking out all off-site power to the IPEC site.

Unit 2 experiences total loss of off-site power. 21/22/23 EDGs all start and run normally with no issues. Unit 3 experiences a total loss of off-site power. 31/32/33 EDGs all start and run normally with no issues.

-0906 hours: Unit 2 CCR Completed Site Area Emergency (SAE) Part 1 notifications to off site, including selecting "No Release to atmosphere" on the Part 1 Form.

-0935 hours: ERO facilities are staffed and operational (excluding Joint Information Center) to include: Alternate Emergency Operations Facility (AEOF), Alternate Operations Support Center (OSC)/Technical Support Center (TSC) and IPEC Liaisons at the Alternate Incident Command Post (ICP).

-0940 hours: Tactical teams enter the Unit 3 waterfront warehouse. All three (3) armed intruders take cover behind warehouse shelving units and two (2) armed intruders provide cover fire by engaging with the State Police (SP) while the third armed intruder sets the stored warehouse items in the building on fire. As the fire blazes in the building, all three (3) armed intruders are neutralized.

-0945 hours: Two sequential loud explosions are heard. A section of the metal roof of the Unit I Screenwell house is blown off. The second explosion appears to have been adjacent to the SW pumps fence line and SW pumps 21/22 (essential SW Header) appear to be lost. The Unit 2 CCR immediately realizes that they will have to secure EDGs due to loss of cooling water to the EDGs. The Unit 2 CCR will attempt to quickly devise a plan to try and save the EDGs if at all possible.

Unit 2 continues to maintain core cooling (secondary heat removal) via 21/23 Auxiliary Boiler Feed Pumps (ABFPs) which are currently being powered from the EDGs and 22 ABFP which is steam driven, all supplied water from the Condensate Storage Tanks (CSTs).

-0945 hours: IPEC Security in CAS/SAS are able to see on the cameras one body (Unit 1 diver buddy) laying under a section of the Unit 1 Screenwell House roof on the Unit 1 Wharf and a second body in a diver's suit floating in the Hudson River just adjacent to the Unit 2 Intake Structure floating dead-man style. The Unit 1 Screenwell House is engulfed in flames. Grey smoke slowly rises from the building and blows directly eastward towards Buchanan and R/S Monitor #6.

-0948 hours: 21/22/23 EDGs sequentially trip off-line due to overheating with no SW cooling from the essential SW header (or the Unit 2 CCR may choose to unload and trip EDG Output 22

Due to the electrical transient on the busses from tripping the EDGs, the 120v Solenoid valve PCV- 131 OA (ABFP 22 Steam Supply Pressure Control Valve) goes shut and 22 ABFP trips off-line. Unit 2 has now lost all core cooling (secondary heat removal) with the loss of 21/23 ABFPs which were being powered from the EDGs prior to the loss of EDGs and the loss of 22 ABFP.

-1003 hours: The Emergency Director declares a General Emergency (GE) based on EAL HG4.1 "A hostile action has occurred such that plant personnel are unable to operate equipment required to maintain safety functions OR A hostile action has caused failure of Spent Fuel Cooling Systems and imminent fuel damage is likely." (Unit 2 has lost secondary heat sink when the power [EDG's] to the 2 operating ABF Pumps was lost and the third ABFP tripped off with no indication of when Unit 3 can start and bring power over from the Unit 3 Appendix R EDG to provide additional secondary heat sink via the ABF Pumps again).

-1010 hours: CAS reports to Unit 2 CCR that from their cameras they can see that the fire in the Unit 1 Screenwell House appears to be out. The fire in the Unit 3 waterfront warehouse continues to burn out of control.

-1018 hours: AEOF has completed GE Part 1 notifications to off site including selecting "No Release to atmosphere" on the Part 1 Form.

-1035 hours: The Joint Information Center (JIC) is manned and is preparing Press Releases and Media Briefings.

Note: This time for the JIC can change based on the time of declaration of the Alert.

-1040 hours: Security/LLEA/State Police escorts Operations personnel as required for the events that unfolded during the event. Unit 2 CCRs priorities to the ICP should be:

1. Primary Auxiliary Building (PAB) Pipe Pen Mezzanine to secure seal injection/return to Reactor Coolant Pump (RCP) seals to prevent further damage to the seals and a possible 23 gpm (per RCP) leak to containment. Priority 1
2. Unit 3 Appendix R EDG to bring power back to safeguards busses. Priority 2
3. SW locations on the Unit 2 Waterfront/PAB to try to recover SW. Priority 3 23

-1050 hours: Unit 3 Operators start up the Appendix R EDG and report to Unit 3 CCR that the Appendix R EDG is operating normally and ready to be tied into the Unit 2 Safeguards busses.

-1054 hours: Unit 2 CCR brings Unit 3 Appendix R EDG to the Unit 2 Safeguard bus by manipulating breaker controls located in the Unit 2 CCR.

-4100 hours: Unit 2 CCR starts Component Cooling Water (CCW) Pumps, Residual Heat Removal (RHR) Pumps and Safety Injection (SI) Pumps as necessary to establish cooling to the primary plant.

-1130 hours: Unit 2 CCR begins seeing effects of cooling the primary plant.

-1145 hours: After establishing and securing on-site "crime scene" locations, the ICP, Alt.

TSC/OSC, Unit 2 CCR, and IPEC Security coordinate and plan movement of personnel from the Alt. TSC/OSC to the site, in a controlled fashion, after designating specific movement paths for ERO personnel.

Each jurisdiction and functional entity was evaluated on the basis of its demonstration of criteria delineated in exercise evaluation areas contained in the FEMA REP Program Manual, April 2012. Detailed information on the exercise criteria and the extent-of-play agreement used in this exercise are found in Appendix D of this report.

CONDITION: During the exercise there was confusion concerning the declaration Emergency Classification Levels (ECLs) and how the declaration of an ECL at IPEC related to each county. At approximately 1045, two call-takers at the PICC provided information to the public that was inaccurate. A General Emergency (GE) ECL was declared at 0958 and the GE ECL was posted in the hotline Fact Chat and on the status board at 1024. At 1045 hours0.0121 days <br />0.29 hours <br />0.00173 weeks <br />3.976225e-4 months <br />, a call-taker informed a member of the public that the plant was at a Site Area Emergency ECL. Also around that time, one call-taker provided information to a caller that "Orange County was not in a General Emergency."

For current conditions the staff had to reference the PICC group chat, the Fact Chat, email, status boards and verbal updates provided by supervision. The status board in the room happened to be behind the call-taker that provided the inaccurate information. Possible misunderstanding of the difference between GE, local State of Emergency and protective actions as Orange County was in the Emergency Planning Zone and affected by the GE, but had no protective actions at the time information was provided that "Orange County was not in a General Emergency."

REFERENCE:

NUREG-0654/FEMA-REP-1, E.7 EFFECT: Public information provided to callers could be inaccurate and cause confusion and the public to take actions other than those prescribed by offsite response organizations.

CORRECTIVE ACTION DEMONSTRATED: After training was held, the Evaluator continued to observe actions and response to various phone calls and all information provided to the public was accurate. At 1219, a call was directed to the specific call-taker that had provided misinformation to the public. The call-taker responded appropriately to the questions provided by the caller, noting the appropriate ECL, that no release was occurring and to stay tuned to EAS radio stations. The caller inquired about protective actions and the call-taker got the specific location of the caller and noting that they were outside the Emergency Planning Zone, that no protective actions were required at the time and to stay tuned to the radio.

As such, who would be responsible for notifying the four registered day care centers reflected in the OCCEMP was left in doubt during the exercise. During interviews, two separate answers were given as to who would be responsible for notifications of day cares by the BOCES representative and the Operations Section Chief.

CORRECTIVE ACTION DEMONSTRATED: Consistent throughout the Orange County (OC) Comprehensive Management Plan Annex 19 special facilities are not designated by name or position responsible for the notification of day care center within the OC Emergency Planning Zone. The four identified day care centers would be notified by the agency and not necessarily by a specific individual or position.

Current plans do not adequately address or describe the plans, policies and procedures that would be required for implementation.

CORRECTIVE ACTION DEMONSTRATED: Detailed Traffic and Acess contol points are not traditionally included in specific detail in the Orange County Comprehensive Emergency Plan however the detail setup and location and trafic control patterns are maintained in great detail in a seperate prodecure by the Local Orange County Sheriffs office and New York State Police. These images are provided to the law enforcement individual through Google Earth or emailed via their communication systems.

ISSUE NO.: 32-12-3d2-A-04 ISSUE: The Orange County Public Information Officer was not informed of the alternate evacuation route as a result of a traffic impediment (simulated rollover of a tractor trailer carrying milk) at 12:22. There is potential for this information not to be appropriately and adequately communicated out to the public who will be evacuating, thereby increasing the potential for increase in traffic congestion that would slow the evacuation process.

The FEMA REP Program Manual dated April 2012 states under Criterion 3.d.2 "Impediments to evacuation are identified and resolved: The impediment must occur during the evacuation and be on an evacuation route such that re-routing of traffic is required, triggering decision making and coordination with the JIC to communicate the alternative route to evacuees leaving the area".

CORRECTIVE ACTION DEMONSTRATED: Based on the detour the Department of Public Works (DPW) intended to deploy, law enforcement personnel, variable message boards and physical traffic control equipment (cones and barriers) to establish and route traffic around the impediment. This decision was also communicated back to the Operation Section Chief (OSC), command staff, the Public Information Officer (PIO) who provided a press release to the media in a 48

CONDITION: During the Rockland County MS-I drill on June 16, 2013, an EMT from the Haverstraw Ambulance Corp. monitored a patient for radiological contamination. The EMT found radiological contamination on the patient at several areas, but failed to monitor the entire patient resulting in some areas of contamination to be missed.

POSSIBLE CAUSE: A possible cause for the issue identified above may be a failure to follow procedures and/or lack of adequate training.

REFERENCE:

NUREG-0654/FEMA-REP-1, F.2; H.10; K.5.a, b; L.1, 4 EFFECT: Not monitoring the entire body of the patient can result in areas of contamination to be missed. A complete and accurate assessment cannot be provided to the hospital and could increase the risk of the spread of contamination if not handled properly.

CORRECTIVE ACTION DEMONSTRATED: An exercise time-out was announced by the exercise controller and just in time training was conducted to educate the EMTs how to properly perform an operational check on the survey meter. Following the exercise time-out, the EMTs successfully demonstrated an operational check of the survey meter. The EMT, checked the meter to a known source and the meter read within acceptable limits (as indicated on the source).

ISSUE NO.: 32-14-6dl-A-03 CRITERION: The facility/ORO has the appropriate space, adequate resources, and trained personnel to provide transport, monitoring, decontamination, and medical services to contaminated injured individuals.

CONDITION: At 1017, an EMT made a phone call to notify the hospital that they were in route, transporting a patient contaminated with radiation. No details about the levels of contamination were given and none were requested. The second EMT realized that the contamination levels were not communicated with the hospital and it was determined to call the hospital back. At 1020, a second call to the hospital was made to give the doctors the levels of contamination found on the patient. After the EMT got off the phone, he wasn't sure that they knew what he was talking about. He called again and spoke with someone that identified themselves as a nurse. The EMT advised the evaluator that the nurse wasn't interested in the contamination numbers 63

POSSIBLE CAUSE: EMT called the wrong hospital using methods not identified in the Rockland County Radiological Emergency Response Plan.

ISSUE NO.: 32-12-6dl-P-01 ISSUE: During a MS-I exercise at Good Samaritan Hospital, the radiation physicist covered the probe that he used with his Ludlum Model 177 frisker to conduct surveys for contamination. He used a plastic glove for this purpose. However, the fingers of the glove touched everything that he surveyed and could have potentially caused a cross-contamination problem.

CORRECTIVE ACTION DEMONSTRATED: On May 16, 2013, the Radiation Physicist at Good Samaritan Hospital successfully demonstrated the ability to use a plastic bag to cover the instrument probe and to effectively monitor a patient. The hospital procedure, revised in April, 2012, not only requires a plastic bag to be used to cover the instrument, but also provides an illustration of how it should look.

ISSUE NO.: 32-12-6dI-A-01 ISSUE: During a MS-1 exercise at Good Samaritan Hospital, REA team members did not always follow the hospital procedure. The hospital procedure, which is summarized in a Patient Management - Wall Chart, dictates that samples should be taken prior to decontamination of the patient. Despite this, REA team members immediately began patient decontamination and had to be prompted to stop and obtain biological samples. Procedures for patient decontamination were not strictly followed. Contrary to the hospital procedure, the REA staff did not use "chux" and medical tape to drape the wound. Instead, they placed a cloth under the patient's elbow. In addition, they decontaminated the wound from the inside out, instead of following the procedural requirement for decontamination of the wound from the outside in. Finally, anti-contamination clothing was not donned or removed in the order that appears on the Anti-contamination clothing - Wall chart. In one instance, a gown was removed in a manner that could unnecessarily lead to a spread of 66
