FACTORYMUTUALRESEARCHCORPORATIONPURPOSEAnevaluationofselectedfiredoorsprotectingsafety-relatedareaswasconductedforPennsylvaniaPowerandLightCompany(PP6L)atSusquehannaSteamElectricStationUnits1and2.Thepurposeoftheevaluationwastoexamine,selectedfiredoorstodeterminewhetherfieldmodificationsmadetolabeledfiredoors(doorswhichhavebeentestedandacceptedbyanationallyrecognizedlaboratory)sincetheirinstallationvoidedthefireresistanceratingofthedoors;andtoexamineothernonlabeleddoorassembliesandrenderanopinionontheirfireresistancerating.

~~FACTORYMUTUALRESEARCHCORPORATIONFIREDOOREVALUATIONLabeledfiredoorsanddoorframes,alabeledaccesspanel,andanunlabeledspecialpurposedoorandframewereexaminedduringthisvisittoSusquehannaSteamElectricStationattherequestofPennsylvaniaPowerandLightCompany(PP6L).2.1FIREDOORSANDDOORFRAMEASSEMBLIESANDACCESSPANELSSixty-sixdoorsanddoorframesandoneaccesspanelwereexaminedtodeterminewhether1)labelswereinplaceandproperlyattached,2)modificationshadbeenmadetothedoororpanel,and3)modificationsmadewouldvoidthefireresistanceratingofthedoororpanel.Ofthe66doorsexamined,62doorswerelabeled,4doorswerenotlabeledorhadlabelslooselyattached.Forty-twoofthelabeledfiredoorshadbeenmodified.2.1.1LabeledDoorswithNoModificationsLabeleddoorswerevisuallyexaminedtodeterminethattheretwasnophysicaldamagetothedoorandthathardwaresuppliedaspartoftheassemblyfunctionedproperlytocloseandlatchthedoorwhenthedoorwasreleasedfromtheopenposition.Firedoorassemblieswhichhavenotbeenmodifiedshouldhavethefireresistanceratingindicatedonthelabel.2.1.2LabeledFireDoorsandPanelwithModificationsFiredooranddoorframeassemblieswhichhavebeentestedbyanationallyrecognizedlaboratory,suchasFactoryMutualResearchCorporation(FMRC)orUnderwriter'sLaboratories(UL),havealabelattachedtoboththedoorandtheframe.Thefireresistanceratingisnotedonthelabel.AgenerallyacceptedpreceptofFMRCandUListhatanyalterationoftheassemblyfromthesuccessfullyfiretestedconstructionvoidsthefireresistanceratingindicatedonthelabel.However,therearereasonsinthenuclearindustryformodifyingafiredoorassembly;amajorreasonissecurity.Caremustbeusedwhenaddingsecurityhardwaretothedoorand/orframetoassurethatthishardwarewillhaveminimumimpactonfireresistancerating,'oftheassembly.

1PACTORYMUTUALRESEARCHCORPORATIONIntheexamination,firedoorassemblieswithmodificationshadoneormoreofthefollowingadditionsoralterations:1.Switchesorelectromagnetsattachedtotheheaderofthedoorframe.Wiringisinrigidmetalconduitfromthecardreadertothefaceofthedoorframe.2.Steeloraluminumanglestrips2in.to4in.wide,1/8in.thick,and4in.to6in.longattachedtothetopofthedoorbymachinescrews.Theanglestripsactinconcertwiththeswitchorelectromagnet.3.Weatherstrippingusedtominimizeairleakagearounddoorswhereapressuredifferentialexistsbetweenadjoiningareas.Weatherstrippingwasattachedtothedoorordoorframebymeansofmachinescrews.ThelabeledfiredoorsexaminedareidentifiedinAppendixA.AlsoincludedinthisAppendixarecomaentsonadditions/modificationstodoorsandframes.A1-1/2hrlabeledaccesspanelNo.7006islocatedintheControlBuildingatelevation754.The'panelis3ftby3ftandisequippedwithamicroswitchontheframe.Modificationsmadetolabeledfiredoorsandframesandtheaccesspanelarenotexpectedtoaffectthefireresistanceratingoftheseassemblies.2.1.3FireDoorsNotLabeledDoorNo.111(elevation670,ReactorBldg.Unit1)andDoorNo.201(elevation683,ReactorBldg.Unit1)hadnolabels.ThesedoorswerecomparedwithDoorNo.110(elevation670,ReactorBldg,Unit2)whichhadal-l/2hrlabelwiththefollowingresults:2.Allthreehadthesamedimensions(height,widthandthickness)andthesameidentificationtags(exceptfortheopeningnumber)rivetedtothelatchedgeofthedoor.Thespacingofinternalhorizontalstiffenerswasdeterminedtobeapproximately8to12in.oncenteronallthreedoors.Thiswasdonebymeansofastethoscope.

FACTORYMUTUALRESEARCHCORPORATION3.Thesametypeofinsulationwasusedinthecoreofallthreedoors(glassfiberbatts).Thiswasestablishedbyremovingscrewsfromdoorhingesonalldoorsandextractingdoorfillermaterial.4.Thethicknessofthemetalfacesheetonalldoorswasthesame(0.045in.or18ga).Thiswasdeterminedbymeasuringmetalthicknessbymeansofaspeciallyadaptedmicrometer.DoorNo.115(elevation670ofReactorBldg.Unit1)alsohadnolabel.Thisdoorwasapparentlydesignedasabulletproofdoorwithhingesweldedinplace.Itcouldnotbeinternallyexaminedinthemannerdescribedabove.ThedoorwasmanufacturedbytheChicagoBulletProofDoorCompany.OtherdoorsmadebythesamecompanywerelocatedintheControlBuilding(Door462atelevation729)andintheReactorBuilding(Door571atelevation818).Boththeabovedoorshavel-l/2hrlabels.Allthreedoorshave1)thesamedimensions,2)thesamehardware,3)asolidcore.Basedonthewriter'experienceauditingfiredoormanufacturingplantsitisbelievedthatDoorNo.115wouldalsohaveal-l/2hrfirerating.However,sincethedoorcouldnotbeinternallyexamined,themanufacturerwouldhavetobecontactedandaskedtocomparetheconstructionofthisdoorwiththe1-1/2hrfirerateddoorstoconfirmtheaboVe.Door587hadalooselyattachedlabel.Thisdoorwouldnormallybetreatedasifithadnolabel.Thedoorwasnotphysicallyexaminedasabovebecauseitwaslocatedinashortcorridorwithalabeledfiredooroneitherside(Door586andastairwaydoor).2.2SPECIALPURPOSEDOORAnevaluationwasmadeofanunlabeledspecial-purpose(flood)doortodeterminewhetheritwouldprovidea1-1/2hrfireresistancerating.Thedoorexaminedwasatelevation645intheCoreSprayPumpRoomofUnit1~ThisareaisindicatedasArea1-1Binthe"FireProtectionReviewReport".Thedoorwas3ftby7ftandconsistedof1/4in.platesteelwith1in.x2in.x3/16in.steelchannelforperipheralframingandhorirontalstiffening10in.oncenter.Thedoorwaslatchedbytwelve1in.square,steelbarlatchesengagingthe5/8in.thicksteelframeonthetop,bottomandsides.Gasketingisemployedtomakeawaterproofsealwiththe5/8in.thicksteelframe.Steeldoorsofthistypehavenotbeentestedforfireresistanceinthiscountry.TheconstructionofthisdoorwascomparedtotheRulesoftheFireOfficesCommittee(FOC),Section1,Specification1,fromGreatBritain,

1]1FACTORYMUTUALRESEARCHCORPORATIONwhichstatesthatdoorsfabricatedinaccordancewiththoserulesmaybeexpectedtoprovide6hrs'ireresistancewhenthereisonedooroneachsideoftheopening.Thisisbasedonnumeroussuccessfulfiretestsofthesedoorsinthatconfiguration.Firetestsonsingledoorsofthisdesign,conductedtorequirementsofBritishStandard476,Part8,havesatisfiedrequirementsofthatstandardfor'hours.ThistestmethodisequivalenttothetestprocedureusedintheU.S.,ASTME-152,indeterminingadoor'sfireresistancerating,withoneexception:ahosestreamtestisnotrequiredinBS476.Thisexceptionisnotconsideredcriticalforsteeldoorssincethehosestreamisappliedtothehingesideofthedoorwhichdrivesthedooragainsttheframestop.RulesoftheFOCrequiretwopointlatchesofnolessthan3/8in.diametersteelinsuchdoors.Thewaterproofdoorinquestionhastwelve1in.squaresteellatchbarswhichengagetheframe;hence,thedesiredl-l/2hrfireresistantperformancecanreasonablybeexpectedinafiresituation.2.3ELECTRICALEQUIPMENTCHASESAnevaluationwasmade.ofdoorstochasescontainingelectricalequipmentatelevation771intheControlBuilding.Thesedoorswerenumbered594,595,598,andoneunnumbered.Thedoorsarefittedwithafusiblelink-1Iactuatedlouver(llin.xllin.)locatedinthetopthirdofthedoor.Thedoorsandlouversborel-l/2hrULlabels~FMdoesnotapprovelabeledfiredoorswithlouvers.However,ULtestsandlistslouversforinstallationinlabeledfiredoors.TheNationalFireProtectionAssociation(NFPA)allowstheuseoflouversinlabeledfiredoorswheretheopeningisnotinanexitorlocatedwhereproductsofcombustionflowingthroughthelouveropeningpriortoitsoperationcouldjeopardizetheuseofexits.Thewallconstructionofthecablechasesconsistsoftwo1in.thicklayersofTypeXgypsumwallboardonsteelstuds.Thewallboardisonthecorridorsideonlywithnosheathingprovidedonthechasesideofthestuds.Thisisnotatestedfireratedconstruction.TomeetFMrecommendationslouversshouldberelocatedtothefaceofthecablechaseneartheceilingtoachievefastresponse.Louveropeningsinthedoorshouldbeclosedwith16gagesheetmetalwithaminimum2in.overlaparoundtheopening.Thesheetmetalcoveringshouldbeattachedwithmachinescrews5in.oncenter.

FACTORYMUTUALRESEARCHCORPORATIONThecons'tructionoftheelectricalequipmentchaseissuchthatal-l/2hrfireresistancecanbeexpectediffireexposureisonthecorridorside.However,ifthefireexposurecamefromwithinthechase,fireresistanceratingwouldbeminimal.Expansi'onoftheexposedsteelstudsduetoafireinsidewouldresultinmovementoffastenersholdingthegypsumwallboardin.placewithpossiblelossofstructuralintegrityofthewall.

FACTORYMUTUALRESEARCHCORPORATIONRECOMMENDATIONSEightdoorassembliesinControlandReactorBuildings,requirethefollowing:DoorNo.115(Elevation670,ReactorBuilding,Unit1):Themanufacturershouldbecontactedtodeterminethefireratingofthisdoor.i'~DoorNo.559,Elevation771,ControlBuilding:Thelatchmechanismonthisfireexitdeviceneedsrepair.Latchingisnotreliableand-boltretractioniserratic.i3.DoorNo.586,Elevation783,ControlBuilding:Thisdoorwaspreparedforadeadboltwhichhasnotbeenprovided.The1in.diameterholeinthefacesheetshouldbecoveredwith16gagesheetmetalfastenedwithmachinescrews.TomeetFMrecoamendationsventilationlouversinthedoorstothefourchasescontainingelectricalpanelsandcablesatElevation771,ControlBuilding(Doors.No.594,595,598,andoneunnumbered)should,berelocatedtothefaceofthecablechaseneartheceilingtoachievefastresponse.Louveropeningsinthedoorshouldbeclosed-~~~~~with16gagesheetmetalwithaminimum2in.overlaparoundtheopening.Thesheetmetalshouldbeattachedwithmachinescrews5in.oncenter.DoorNo.601,Elevation779ReactorBuilding:Oneboltattachingtheframetothewallismissingandshouldbereplaced.Also,thecloserattachmenttotheframeheaderislooseandshouldbetightened.
