The security officer took possession of the drawing and notified the Security Shift Supervisor. The uncontrolled

" Safeguards Information" drawing was associated with an ongoing station modification to install a new plant security computer system. Use of the drawing was required to transition the tamper alarm for Security Termination Box "TB-4" to the plant security computer (PSC). This work was completed on 12/06/98 at 1104 hours0.0128 days <br />0.307 hours <br />0.00183 weeks <br />4.20072e-4 months <br />. Investigation established that the drawing was used at Termination Box "TB-4" and documented as returned to the designated

" Safeguards Information" storage container on 12/05/98 at 1712 hours0.0198 days <br />0.476 hours <br />0.00283 weeks <br />6.51416e-4 months <br />.

However, the investigation could not conclusively establish the circumstances l

which resulted in the drawing being found uncontrolled in the Unit #3 Cable l Room. The root cause of this event is " inappropriate action" (failure to L follow established procedures and policies for control of Safeguards

Information). Corrective actions for this event include: prohibiting two employees involved in the event from being allowed independent and unrestricted access to " Safeguards Information"; and implementing a more restrictive access policy for accessing " Safeguards Information" storage l containers.

Backcround l

Installation of the new plant security computer has required that drawings l classified as " Safeguards Information" be used in the field by craft personnel to perform various installation activities.

Craft implementation personnel who are designated to have independent and unrestricted access to " Safeguards Information" are trained as a Safeguards

" Routine User" in accordance with section 206.6.2.1 of Duke Power Nuclear  ;

System Directive (NSD) 206, " Safeguards and Information Control". Craft  ;

implementation personnel who are not trained as a Safeguards " Routine User" may, in accordance with section 206.6.2.2 of NSD 206, view or work with Safeguards Information under the direction of a Safeguards " Routine User".

To facilitate field work associated with the installation of the new plant security computer, craft personnel were provided with a designated safeguards storage container within the Engineering Safeguards Work Area; a location within the site protected area. Unrestricted access to this container is limited to craft personnel designated as a Safeguards " Routine User", the lead Oconee Security Computer Replacement Project Modification Engineer and the Oconee Site Safeguards Administrator. Controls associated with the safeguards storage container include the use of a container open/ closed log, document sign in/out log, and double verification that the container has been properly locked when the need for access no longer exists. When not locked in the designated storage container, as required by section 206.7.1 of NSD 206, " Safeguards Information" must be under the control of an authorized person; a Safeguards " Routine User".

The uncontrolled " Safeguards Information" drawing was discovered in the Unit #3 Cable Room, immediately adjacent to Security Termination Box "TB-4". The Unit #3 Cable Room is a vital area that is provided with alarmed i security controlled access doors.

The uncontrolled " Safeguards Information" drawing depicted the "as built" wiring schematics and power device information for Security Termination +

Boxes #1, #2, #3 and #4. The drawing did not depict four (4) engineering l c%nges associated with the tamper alarm for each termination box that were  ;

made as part of the plant security computer modification. The wiring and power supplies depicted on the uncontrolled drawing are located within  ;

signal supervised and alarmed cabinets with access controlled by Oconee l Site Security. l As required by section 206.7.11 of NSD 206, " Safeguards and Information l Control", the loss or compromise of " Safeguards Information" shall be j reported to the site safeguards Administrator or, if unavailable, Site  ;

Security. Training given to Safeguards " Routine Users" states that, "If l documents are lost, misplaced or otherwise compromised, immediately contact i the Security shift Supervisor."

Another event involving Safeguards Information (SGI) occurred on March 3, 1998 when a safeguards container in the engineering safeguards work area was not properly locked and secured (Ref. PIP 4-0-98-1025).

D2scription of Event The information provided below is a synopsis of events associated with the uncontrolled " Safeguards Information" drawing 798-08-E '"As Built" as determined by the licensee investigation and reflects statements obtained from the individuals involved.

Employee #3 then indicated the date/ time and signed the "Out" section of the log indicating removal of the " Safeguards Information" i drawings from the Craft Safeguards Container.

Employee #3 transferred the " Safeguards Information" drawings in his possession to Employee #1 and requested that he (Employee #1) return the drawings to the Craft Safeguards Container at the end of the work day.

Employee #3 stated this entry was to obtain " Safeguards Information" drawings l required to perform work activities for the remainder of the shift. However, as indicated by the Container Document Log, no drawings were removed from the container. .

During this time period Employee #3 stated that he found the l missing " Safeguards Information" drawing, 798-08-E "As Built" l improperly filed in another drawing folder within the Craft i Safeguards Container. Employee #3 stated he then entered the date/ time and signed the "In" column of the Container Document Log; the "out" column for this line on the Container Document Log was previously completed and lined through when

" Safeguards Information" drawing 798-08-E "As Built". At the time of discovery, the drawing was folded and laying flat on the floor immediately adjacent to Security Termination Box  !

"TB-4". The Security Officer took possession of the drawing, reviewed the immediate area for additional uncontrolled  !

drawings and notified the Security Shift Supervisor.

l Conclusion  !

The licensee investigation into this event could not conclusively determine j how drawing 798-08-E "As Built" came to be found by Security in the Unit #3 J Cable Room on 12/06/98 at 1515 hours0.0175 days <br />0.421 hours <br />0.0025 weeks <br />5.764575e-4 months <br />. However, contrary to the employee's statements, the most plausible explanation is that the drawing was inadvertently.left in the Unit #3 Cable Room by Employee #3 after work was  ;

completed on Security Termination Box "TB-4" on 12/06/98 at 1110 hours0.0128 days <br />0.308 hours <br />0.00184 weeks <br />4.22355e-4 months <br />. l This assumption is supported by: 1

1. The licensee determination that " Safeguards Information" drawing 798-08-E "As Built" was properly returned to the Craft Safeguards Container i on 12/05/98 at 1712 hours0.0198 days <br />0.476 hours <br />0.00283 weeks <br />6.51416e-4 months <br />.

Numerous inconsistencies that were identified during separate interviews with Employee #3 concerning the disposition of " Safeguards Information" drawing 798-08-E "As Built" on 12/06/98 between 0845 and l 1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br />.

In the absence of-an absolute determination of how " Safeguards Information" drawing 798-08-E "As Built" came to be located in the Unit #3 Cable Room, the most probable root cause of this event is the inappropriate action of Employee #3 by failing to follow established policies and procedures to properly maintain control of the " Safeguards Information" drawing.

1. The " Safeguards Information" drawing was controlled by Oconee Site Security upon discovery.
2. A " Heightened Awareness" was issued to the Oconee Site Security Force for unannounced alarms on field devices associated with the potentially compromised " Safeguards Information" drawing 798-08-E "As Built". This

" Heightened Awareness" was continued throughout the investigation of this incident and was discontinued on 12/21/98.

3. An inventory of the Craf t Storage Container was conducted in which it was verified by craft personnel that no other " Safeguards Information" drawings were missing.
4. The lock to the Craf t safeguards container was changed by the Site Safeguards Administrator and the new lock combination provided to Employee #7, the lead Oconee Security Computer Replacement Project Modification Engineer, and Employee #8, the craft Security Computer Replacement Project Implementation Supervisor.

Subseauent:

1. On 12/08/98, an individual was trained and designated as the point of contact for issuing and retrieving " Safeguards Information" documents from the Craft Safeguards Container. Designated points of contact will continue to be utilized for issuance of " Safeguards Information" documents from the Craft safeguards Container until the plant security computer modification is completed and/or use of the Craft Safeguards Container is no longer required for the storage of " Safeguards Information" documents.
2. On ' 12 /16 / 9 8, based upon information obtained during the course of the
licensee investigation, the Safeguards " Routine User" status for l

Employees #3 and Employee #5 was revoked by the Oconee Site Safeguards Administrator for failure to comply with Duke Power Nuclear System Directive 206, " Safeguards Information and Control", requirements.

Specifically, when " Safeguards Information" drawing 798-08-E "As Built"

was identified as being missing, Employee #3 and #5 (trained as

3. On 12/21/98, the Safeguards " Routine User" status for all craft personnel involved in installation of the plant security computer (PSC) was j rescinded. Prior to being reauthorized independent and unrestricted access to " Safeguards Information", these individuals were retrr: ed as a Safeguards " Routine User". l
4. On 12/28/98, the Oconee Nuclear Site " Security and Engineering Safeguards Workplace Procedure" was revised to require that: l "When a safeguards container is provided for use by craft personnel j during security system projects / modifications, overall control of  !

the containers shall be maintained by Security. A limited number of individuals shall be designated as being responsible for the j issuance and retrieval of ' Safeguards Information' documents within i I

the containers. Once each month, at a minimum, Security shall inventory Craft Safeguards Containers to verify that assigned

' Safeguards Information' documents are within the container or properly controlled by a Safeguards ' Routine User'".

1. For the remaining duration that the Craft Safeguards Container is

. utilized for storage of " Safeguards Information" documents, Security will conduct a monthly inventory of the container as identified in

" Subsequent" corrective action item #4 above.

2. Corrective (disciplinary) action will be taken with the involved l employees.

Also the wiring and power supplies depicted on the uncontrolled and potentially compromised " Safeguards Information" drawing 798-08-E "As Built" are located within signal supervised and alarmed cabinets with access controlled by site Security. Therefore, the potential compromise of

" Safeguards Information" drawing 798-08-E "As Built" would not provide an individual with undetected access into protected or vital areas of the Oconee Nuclear Site.
