PILGRIM STATUS REPORT FOR THE PERIOD AUGUST 26 - SEPTEMBER 8, 1989 Enclosed-is the Pilgrim bi-weekly status report 'from the NRC Resident Office at. Pilgrim. The NRC Restart Staff is in place and was on-site monitoring licensee activities on an extended day shift basis during this report period. The Pilgrim bi-weekly status report is intended to provide NRC management and ' the public with an overview of plant activities and NRC restart-inspection activities. PILGRIM STATUS REPORT FOR THE PERIOD AUGUST SEPTEMBER 8, 1989 , 1.0 Plant Status

                      'As of 8iOO a.m. on September 8, 1989, the reactor was operating at approxi-mately 58% power with the turbine generator synchronized to the grid.

l 2.0. Power Ascension Test Program Status. At the close of the last report period, the plant was raising power from

                     -40% following a power reduction to complete backwashing of a condenser waterbox. 'The plant ~ reached 75% power on August 25, 1989.
                     =0n August 30, 1989, the reactor automatically scrammed.from about 75%

power due to a fault on the main generator's voltage regulation circuit'ry.

                     - A component failure in the vcitage regulation circuitry caused a turbine runback and the reactor scrammed on reactor vessel high pressure. All systems responded as designed.and the event was of minor safety signific-ance. Detail NRC review of this event will be documented in NRC Inspec-tion Report No. 50-293/89-10.

At 10:16 p.m. on September 6, 1989, the licensee brought the reactor cri-tical following replacement of the failed component and inspection of other plant systems / components. The turbine generator was synchronized to the electrical power distribution grid at 10:14 a.m. on September. 7, 1989. At the close of this report period, the plant was at 58% power. The licensee ~ continued to conduct routine surveillance along with preven-tive:and corrective maintenance throughout the period.

3. I ems of Special Interest NRC Restart Assessment Panel An NRC Restart Assessment Panel teleconference was held on August 28, 1989 at the NRC Region I Office in King of Prussia, Pennsylvania. NRC manage-ment from Region I, the Office of Nuclear Reactor Regulation (NRR), .and the NRC resident staff participated. The Panel was briefed by the licen-see on their plans for the remainder of the 75% plateau and the remainder -
                    .of the Power Ascension Test program. The Panel meets periodically to co-ordinate planning and execution of NRC inspection and licensing activities related to Pilgrim. The Panel is chaired by Mr. Samuel J. Collins, Deputy Director, Division of Reactor Projects, NRC Region I.

Enclosure 2 4.0 Emergency Notification System (ENS) Reports Reactor Scram Due to Failed Potential Transformer On August 30, 1989, the licensee notified the NRC via ENS at 8:20 p.m. of the reactor scram detailed in Section 2.0 above. Inadvertent Actuation of Low Pressure Coolant Injection Loop Select Logic On September 5,1989, the licensee experienced an inadvertent actuation of the Low Pressure Coolant Injection (LPCI) loop select logic and an inadvertent start of the "A" Emergency Diesel Generator during the performance of a surveillance test. However, no reactor vessel injection occurred. Troubleshooting by the licensee revealed that the actuations were apparently due to the placing of a jumper on the wrong relay. The plant was in cold shutdown at the time. The licensee notified the NRC via ENS. Inoperable High Pressure Coolant Injection System On September 7, 1989, the licensee declared the High Pressure Coolant Injection (HPCI) system inoperable due to minor flow oscillations which caused the HPCI turbine to momentarily trip on overspeed during startup. The HPCI system is a core standby cooling system that maintains an adequate coolant inventory inside the reactor vessel to prevent fuel clad melt. The licensee notified the NRC via ENS of the HPCI system inoperability. Inadvertent Opened Secondary Containment Doors On September 7,1989, both the inner and outer secondary containment reactor building access doors were inadvertently opened simultaneously for about three seconds. This occurred due to personnel error. This was reported to the NRC via ENS. Detailed NRC review of the events described above will be documented in Inspection Report 50-293/89-10. 5.0 Pilgrim Restart Staff Activities The Pilgrim Restart Staff was in extended shift coverage throughout this report period, consistent with reduced testing activities. On August 30, 1989, immediately following the reactor scram, the Senior Resident Inspector was in the control room and observed operator actions in response to the scram. An NRC-developed inspection plan addressing the 50%-75% power plateau, as well as the primary functional areas of operations, maintenance, surveillance, and startup testing was implemented. The inspectors assessed operator performance, organizational interfaces, adherence to procedures, and Technical Specification compliance. permitted to proceed to the next NRC: approval point, 100% of rated power. On' September 7, .1989, NRC Region I issued NRC Inspection. Report 50-293/89-07, which details NRC Restart Staff inspections during the period of May 25 - July 10,1989. This report will be available in.the Local.Public Document Room. The inspection report details several violations'of NRC requirements. However,'the Restart Staff. determined that the' licensee's conduct and control of the first phase of the Shutdown from Outside the Control Room test was excellent. Contact with the press and public continued throughout the period. The NRC Restart Manager'and the Region I Public Affairs Officer have been handling all inquiries.
