On March 26,2012, the U.S. Nuclear Regulatory Commission (NRC) completed an examinationat Nine Mile Point Unit 2. The enclosed report documents the examination findings, which werediscussed on in a telephone exit meeting on April 19,2012, with Mr. Aaron Armstrong, ActingGeneral Supervisor Operations Training, and other members of your staff.The examination included the evaluation of three applicants for reactor operator licenses andeight applicants for instant senior operator licenses. The written and operating examinationswere developed using NUREG-1021, "Operator Licensing Examination Standards for PowerReactors," Revision 9, Supplement 1. The license examiners determined all of the elevenapplicants satisfied the requirements of 10 CFR Part 55, and the appropriate licenses wereissued on April 19,2012. Three licenses were held pending completion of deferred eligibilityrequirements.ln accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter and itsenclosure will be available electronically for public inspection in the NRC Public DocumentRoom or from the Publicly Available Records (PARS) component of NRC's document systemAgencywide Documents Access and Management System (ADAMS). The facilitylicensee developed the examinations using NUREG-1021, "Operator Licensing ExaminationStandards for Power Reactors," Revision 9, Supplement 1. The written examination wasadministered by the facility on March 26,2012. NRC examiners administered the operatingtests on March 19-23,2012. The license examiners determined that all of the eleven applicantssatisfied the requirements of 10 CFR Part 55, and the appropriate licenses have been issued.Enclosure

4. OTHER ACTTVTTTES (OA)4OA5 Other Activities (lnitial Operator License Examination).1 License Applicationsa. Examination ScopeThe examiners reviewed all eleven license applications submitted by the licensee toensure the applications reflected that each applicant satisfied relevant license eligibilityrequirements. The applications were submitted on NRC Form 398, "PersonalQualification Statement," and NRC Form 396, "Certification of Medical Examination byFacility Licensee." The examiners also audited all of the license applications in detail toconfirm that they accurately reflected the subject applicant's qualifications. This auditfocused on the applicant's experience and on{he-job training, including controlmanipulations that provided significant reactivity changes.b. FindinqsNo findings were identified..2 Operator Knowledge and Performancea. Examination ScopeOn March 26,2012, the licensee proctored the administration of the writtenexaminations to all eleven applicants. The licensee staff graded the writtenexaminations, analyzed the results, and presented their analysis to the NRC onMarch 30,2012.The NRC examination team administered the various portions of the operatingexamination to all eleven applicants on Month 19-23,2012. The three applicants forreactor operator licenses participated in three dynamic simulator scenarios, in a controlroom and facilities walkthrough test consisting of eleven system tasks, and anadministrative test consisting of four administrative tasks. The eight applicants seekingan instant senior operator license participated in two or three dynamic simulatorscenarios, a control room and facilities walkthrough test consisting of ten system tasks,and an administrative test consisting of five administrative tasks.b. FindinqsAll eleven of the applicants passed all parts of the examination. For the writtenexaminations, the reactor operator applicants' average score was 85.77 percent andranged from 81.33 to 88 percent, the senior operator applicants' average score was89.25 percent and ranged from 82 to 97 percent. The overall written examinationaverage was 88,30 percent. The text of the examination questions, the licensee'sEnclosure 2examination analysis, and the licensee's post-examination comments may be accessedin the ADAMS system under the accession numbers noted in Attachment 1. Inaccordance with current NRC policy, the release of this written examination in ADAMSto the public will be delayed for two years.The NRC examiners completed the final grading of the written examination on April 3,2012, and conducted a review of each missed question to determine the accuracy andvalidity of the examination questions. In accordance with current NRC policy, therelease of this written examination will be delayed for two years, The text of theexamination questions, the licensee's examination analysis, and the licensee's post-examination comments may be accessed in the ADAMS system under the accessionnumbers noted in the attachment.Chapter ES-403 and Form ES-403-1 of NUREG 1021 require the licensee to analyzethe validity of any written examination questions that were missed by half or more of theapplicants. The licensee conducted this performance analysis and submitted theanalysis to the chief examiner..3 Initial Licensinq Examination Developmenta. Examination ScopeThe facility licensee developed the examinations in accordance with NUREG-1021 ,Revision 9, Supplement 1, All licensee facility training and operations staff involved inexamination preparation and validation were listed on a security agreement. The facilitylicensee submitted both the written and operating examination outlines on August 19,2011. The chief examiner reviewed the outlines against the requirements ofNUREG-1021 , and provided comments to the licensee. The facility licensee submittedthe draft examination package on November 3, 2011. The chief examiner reviewed thedraft examination package against the requirements of NUREG-1021, and provided finalcomments to the licensee on the examination on January 25,2012. The NRCconducted an onsite validation of the operating examinations and provided furthercomments during the week of February 20,2012. The licensee satisfactorily completedcomment resolution on March 19, 2011.b. FindinqsThe NRC approved the initial examination outline and advised the licensee to proceedwith the operating examination development.The examiners determined that the written and operating examinations initiallysubmitted by the licensee were within the range of acceptability expected for a proposedexamination.No findings were identified.Enclosure 3.4 Simulation Facilitv Performancea.
