NINE MILE POINT NUCLEAR STATION, UNIT 2 - SENIOR REACTOR OPERATOR RETAKE EXAMINATION REPORT NO. 05000410/2008303

Dear Mr. Polson:

On October 10, 2008, the U.S. Nuclear Regulatory Commission (NRC) completed a Retake examination at Nine Mile Point Unit 2. The enclosed report documents the examination findings, which were discussed on October 18, 2008, with Mr. Robert Brown, General Supervisor of Operations Training.

The examination included the evaluation of one applicant for an instant senior reactor operator license. The operating examination was developed using NUREG-1021, "Operator Licensing Examination Standards for Power Reactors," Revision 9, Supplement 1. The license examiner determined that this applicant satisfied the requirements of 10 CFR Part 55, and the appropriate license has been issued.

No findings of significance were identified during this examination.

In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter and its enclosure will be available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRCs document system (ADAMS). ER 05000410/2008303; October 6-10, 2008; Nine Mile Point Nuclear Generating Station Unit 2,

Operator Licensing Retake Examination Report.

NRC examiners evaluated the competency of one retake applicant for an instant senior operator license at Nine Mile Point Unit 2. The facility licensee developed the examination using NUREG-1021, "Operator Licensing Examination Standards for Power Reactors," Revision 9,

Supplement 1. An NRC examiner administered the operating test from October 6 to October 10, 2008. The license examiner determined that the applicant satisfied the requirements of 10 CFR Part 55, and the appropriate license has been issued.

REACTOR SAFETY

1RO2 Evaluation of Changes, Tests, or Experiments (Closed) Green Finding 05000410/2008301-01 During the initial licensed operator examination administered during the week of March 10, 2008, a Green finding was identified by the NRC because senior reactor operators were trained to interpret and execute the Primary Containment Control Emergency Operating Procedure (EOP-PC) step PCP-10 in accordance with guidance of the Transient Mitigation Guidelines (TMG) section of the Operations Department Manual. As a result, the crews did not perform an emergency reactor vessel depressurization when suppression chamber pressure exceeded the limits of the pressure suppression pressure curve. This inappropriate guidance resulted in the failure to perform the EOP step as written. The facility wrote CR-NM-2008-2013 to address this issue.

The examiner reviewed the facility corrective actions. The corrective actions included review of industry practices and revision of the incorrect guidance. The examiner determined that the corrective actions satisfactorily addressed the issue. This finding is closed.

4OA5 Other Activities (Retake Operator License Examination)

.1 License Applications

a. Scope

The examiner reviewed one license reapplication submitted by the licensee to ensure the application reflected that the applicant satisfied relevant eligibility requirements. The application was submitted on NRC Form 398, Personal Qualification Statement, and NRC Form 396, Certification of Medical Examination by Facility Licensee.

b. Findings

No findings of significance were identified.

.2 Operator Knowledge and Performance

a. Examination Scope

The NRC examiner administered the retake operating examination to one applicant during the week of October 6 - 10, 2008. This examination consisted of fifteen job performance measures (JPMs).

b. Findings

The applicant passed the examination.

.3 Initial Licensing Examination Development

a. Examination Scope

The facility licensee developed the examination in accordance with NUREG-1021, Revision 9, Supplement 1. All licensee facility training and operations staff involved in examination preparation and validation were listed on a security agreement. The facility licensee submitted examination materials on the schedule agreed to with the chief examiner. The chief examiner reviewed the draft examination package against the requirements of NUREG-1021, Revision 9, Supplement 1, and conducted an onsite validation of the operating examination and provided comments during the week of August 18, 2008. The licensee satisfactorily completed comment resolution prior to administration of the examination.

b. Findings

The NRC approved the initial examination outline and advised the licensee to proceed with the operating examination development.

The examiner determined that the operating examination initially submitted by the licensee was within the range of acceptability expected for a proposed examination.

.4 Simulation Facility Performance

a. Examination Scope

The examiner observed simulator performance with regard to plant fidelity during the examination validation and administration.

b. Findings

No findings of significance were identified.

.5 Examination Security

a. Examination Scope

The examiner reviewed examination security for examination development and during both the onsite preparation week and examination administration week for compliance with NUREG-1021 requirements. Plans for simulator security and applicant control were reviewed and discussed with licensee personnel.

b. Findings

No findings of significance were identified.

4OA6 Meetings, Including Exit

The examiner discussed examination development and administration and applicant performance with Mr. Robert Brown, General Supervisor of Operations Training, on October 10, 2008. The licensee acknowledged the comments presented. On October 18, 2008, Mr. Brown was notified by telephone that the applicant had passed his examination and a license number was provided.

The licensee did not identify any information or materials used during the examination as proprietary.
