Dear Mr. Shealy:

We have previously furnished you with the Independent Facility Radiological Survey and a revised Attachment 4, " Radioactive Materials Inventory" on January 27, 1984 We are now submitting the remainder of the documentation outlined in our letter of November 10,1983 which transmitted the reference Application and some additional revisions to the Application. The enclosures are as follows:

1) "A Special Radiological Survey of Selected Sites in the Vicinity of BNFP October 1983 and January 1984," EMP-AGNS-84-1 by John M. Palms, Ph.D.,

e t al . This report describes the environmental studies made both on-aice and of f-site and confirms that the BNFP has not contributed any measurable radioactive contamination to the environment.

2) A revised RHA-200 form showing Mr. Christian T. Nielsen as the Company representative and the Radiation Safety Of ficer as Dr. Michael T. Ryan.

The substitution of Mr. Nielsen has been caused by my retirement ef fective March 30,1984. Also attached is a brief desqription of activities a*

BNFP~ showing that no use of radioactive ma t e rials. is involved (Attachment 3 of the revised form).

The subs titution of Dr. Ryan has been caused by the erans fee of the previous RPO, Mr. Jim Purvis by CNSI who is under contract to provide radiologica1' surveillance and assistance to AGNS. A revised Attschment No. 2 of the form, 'which is a brief description of the duties of CNSI and a copy of Dr. Ryan's resume, is also included.

3) A copy of CMSI Procedure No. S20-AD-Oll, Rev. O which outlines the pro-cedures to be used in implementing the contract is also enclosed for your information and review.

As to security, we have hired Sizemore Security to perform 24-hour per-day, 7-day per-week security services . The Sizemore Security guard will, among other duties, perform the quarterly security inspections to the site described in Enclosure No. 2. This guard also controls the gate across Osborn Road blocking access to the site.
