NRC Report #158 10/29/91 Page-2 This.is the current standard water pump driven gear used in

DSRV-4 engines. We have experienced five (5; water pump

- ' failures as a result of premature gear wear at non-nuclear installations. Additionally, one (1) gear has been not ed at a non nuclear site with an excessive wear condition.

These failures are characterized as a breaking down of the gear tooth surface. As the driven gear weers, the gear backlash increses which in turn increases the torsional

- excitation fot;es and--vibration in the pump. This increase L -in vibration has caused complete pump failure with a resulting high jacket water temperature condition and engine shutdown.

While!a~ complete pump failure has not occurred in a nuclear application, we have observed severe gear wear at Georgia Power's Plant Vogtle and Carolina Power's Shearon Harris Plant.

Our investigation into these reports reveals that gear wear

-induced pump f ailurcs nave occurred at approximately 1,.500

. operating-hours. Whili gears have a finite life, operating

-hours should exceed 25,000 hours0 days <br />0 hours <br />0 weeks <br />0 months <br />.

Preventive action has been taken by a drawing - revision to--

increase _the gear tooth hardness. This revision (Revision C) also requires the stamping of the part number and revision level on the gear huh, Corrective action must be taken _ by the ' individual site on

-inetalled' gears or any gear or gear. carrier assemblies that are in stores. Recommended corrective action'is as follows:

-1) Perform a visual inspection of the installed gear for evidence of premature wear. Evidence of excessive pitting or spalling or a-network of ourface cracks are signs of impending gear failure. We.are unable to predict, with any accuracy, the remaining service life of such a gear.

2) If the gear is not stamped with a revision level .

I or if the revision level is stamped, but is prior to Revision'"C", the gear should be replaced at the next refueling outage.
