Defects in Hatch, Inc. Instrument Racks Six instrument racks, supplied by Hatch, inc., for use in the Cooper Nuclear

[' Station emergency diesel generator starting air and lube oil systems have been identified as~containing defects. The identified defects could have created a ,

L substantial safety hazard, if the components had been installed and used in

4. 10CFR21.21(b)(3)(iv) Nature of the defect or failure to comply and the safety hazard which is created or could be created by such defect or failure to comply.

The six instrument racks supplied by Hatch, Inc. contained numerous cases of discrepancies in the tube fittings. The discrepancies noted were 1) loose and inadequately seated ferrules, 2) missing ferrules, 3) tubing not sufficiently inserted into ferrules, and 4) cross-threaded connections.

been sufficient to start the diesels. Theer. components, if they have been installed and used ao received, could have caused both L emergency diesel generators to be inoperable. This potential for the loss of the safety function of both emergency diesel generators  ;

l 1s considered to be a substantial safety hazard.

6. 10CFR21.21(b) (3) (vi) In the case of a basic component which contains a  ;

defec. or fails to comply, the number and location of all such components in~use at, supplied for, or being supplied for one or more facilities or activities subject to the regulations in this part.

Six (6) instrument racks installed in the Diesel Generator Rooms at-Cooper Nuclear. Station (CNS). No other Hatch, Inc. racks are in use  !

or ordered for use at CNS.

7. I0' CFR21.21(b)(3)(v11) The corrective action which has been, is being, or will be taken; the name of the individual or organization responsible for the actions; and the length of time that has been or will.be taken'to 4 complete the action.

i corrective Action - A one hundred percent (100%) verification of all tube fittings in the Hatch, Inc. instrument racks has been completed. ,

All' discrepant fittings have been corrected. A review was completed to confirm that no other similar instrument racks are in use or on order at Cooper Nuclear Station. ]

Length of Time to Complete - The 100% verification of all tube I fittings and repair of discrepant fittings consumed approximately 30  ;

8. 10CFR21.21(6)(3)(viii) Any advice related to the defect or failure to comply about the facility, activity or basic component that has been, is being, or will be given to purchasers or licensees.

Not applicable. The supplier has been informed through previous correspondence and will be further advised of the District's evaluation by copy of this notification. .
