AUDIT 82V-5, DEFICIENCIES 3, 4, AND 5 YCRD-50-566/82-11, YCRD-50-567/82-11 10 CFR 50.55(e)

FINAL REPORT Description of Deficiency During the performance of QA audit 82V-5 of Morgan Engineering, Inc.,

Alliance, Ohio, who were contracted to design and manufacture two 250-ton polar cranes for the Yellow Creek Nuclear Plant, deficiencies were noted because of noncompliances.with the accepted QA program or violation of TVA's contract requirements as follows:

Procurement Control - Vendors included on the Approved Vendors List (AVL) had not been identified as to their facility location or the basis on which the vendors had been qualified and included in the AVL. Also noted was the violation of TVA's requirement concerning inspection of items at subvendor's facilities before shipment.

Document Control - The latest revisions of shop drawings were not being consistently distributed. In addition, superseded drawings had not been retrieved.

Welding Control - The temperatures of welding electrode holding ovens were not being maintained at 250 F as specified by internal procedures.

Safety Implications Had this condition remained uncorrected, the deficiencies identified above in Morgan Engineering's QA program could have resulted in the improper fabrication of the two 250-ton polar cranes. This condition would have jeopardized the integrity of the polar cranes. Therefore, TVA assumes this condition could have adversely affected the safe operation of the plant.

Corrective Action TVA has notified Morgan Engineering of the subject deficiencies. Because of the deferred status of construction at Yellow Creek, the contract with Morgan Engineering has been cancelled. No corrective action will be required at this time for the polar cranes since these items have been scrapped. Before any future procurement actions are taken with Morgan Engineering a detailed quality assurance audit will be performed to ensure that their QA program is being satisfactorily implemented.

The above deficiencies do not affect any other TVA nuclear plant.
