N G YELLOW CREEK NUCLEAR PLANT UNITS 1 AND 2 - QA PROGRAM COMPLIANCE WITH ANSI N45.2.11 - YCRD-50-566/81-19, YCRD-50-567/81 FINAL REPORT The subject deficiency was initially reported to NRC-0IE Inspector D. Quick on July 31, 1981 in accordance with 10 CFR 50.55(e) as Construction Quality Assurance Audit YC-G-81-07. This was followed by our interim reports dated August 31, October 7, and December 16, 1981.

ENCLOSURE YELLOW CREEK NUCLEAR PLANT UNITS 1 AND 2 QA PROCRAM COMPLIANCE WITH ANSI N45.2.11 YCRD-50-566/81-19, YCRD-50-567/81-16 10 CFR 50.55(e)

FINAL REPORT Description of Deficiency The Yellow Creek Construction QA Program fails to comply with ANSI N45.2.11 in several areas because there are no procedures to control the following:

(a) The review, approval, and revision of configuration control and as-constructed drawings at the project level.

(b) The storage and control of site-originated drawings that have been classified as QA drawings. -

(c) The establishment of distribution lists for drawings.

(d) The establishment of distribution lists for design information requests.

(e) Record classification (lifetime or nonpermanent) of field change requests or site-originated drawings that have been classified as QA drawings.

Procedures did exist for all areas except (b) above. However, the procedures did not adequately address the other aspects listed.

The cause of this deficiency was the failure to fully specify all requirements of ANSI N45.2.11, " Quality Assurance Requirements for the design of Nuclear Power Plants" in the construction engineering procedures (CEP).

Safety Implications

Had this condition remained uncorrected, the lack of procedures regarding the review, approval, and revision of configuration drawings could result in a deficient configuration control drawing being used in the final construction of a safety-related systhm which could lead to a condition that could adversely affect the safe operation of the plant.

Corrective Action The following actions were taken to ensure Yellow Creek Construction QA Program complies with ANSI N45.2.11, in above-mentioned areas.

Page 2 (a) CEP-3.02, " Configuration Control for Drawings," and CEP-3 03, "As-Constructed Drawings," have been revised to adequately provide for review, approval, and revision control of configuration control and as-constructed drawings at the project level.

(b) CEP-6.07, " Site-Originated Drawings," has been prepared to provide for review, approval, and revision control of site-originated drawings that have been classified as QA drawings.

(c) CEP-6.01, " Drawing Control," has been revised to provide for specific assignment for distribution of drawings to appropriate site personnel.

(d) CEP-3 05, " Design Information Request," has been revised to specify distributien of Design Information Requests.

(e) CEP-3 01, " Field Change Requests," has been revised and CEP-6.07,

" Site-Originated Drawings," has been prepared to provide record classification of field change requests and site-originated drawings.

The above procedures are approved and have been issued.

Because of the stage of construction at Yellow Creek Nuclear Plant, no configuration drawings have been issued; therefore, no additional deficiencies were identified. The deficiencies identified were programmatic deficiencies.

TVA considers this deficiency a one-time occurrence, and the issuance of the new and revised construction engineering procedures will prevent recurrence of this deficiency.
