ABSTRACT (Limit to 1400 spaces, (e., spproximetsly fifteen single-space typewritten Unes} (16) b NO SUBMISSION DATE 1151 I I I On June 9, 1993, at 1443 hours0.0167 days <br />0.401 hours <br />0.00239 weeks <br />5.490615e-4 months <br />, lC Vital Bus sensed an undervoltage condition resulting in an automatic start and blackout loading of lC Diesel Generator (D/G) . The automatic D/G start.and loading is an Engineered sa.fety Feature (ESF} . At the time of the event, Maintenance personnel were conducting the monthly functional surveillance test of the lC 4KV Vital Bus in accordance with procedure Sl.MD-FT.4KV-0003(Q) I "ES FAS Instrumentation Monthly Functional Test lC 4KV Vital Bus Undervoltag_e". The root cause of the lC Vital Bus trip is personnel error attributed to inattention to detail. The Maintenance technician did not self-check as required by station work practices. The technician conducting the monthly functional surveillance test was supposed to place the 1B3 test signal switch (one (1) of three (-3) SEC 91% undervoltage test signal switches located on the lC SEC lower section) to the "off" position. He inadvertently placed one (1) of the other two (2) test signal switches to the "on" position satisfying the logic for single vital bus undervoltage ESF Mode Op sequence. This event has been reviewed by Maintenance Department management. Positive disciplinary action with the individual(s) involved has been completed. The circumstances. surrounding this event will be reviewed with applicable Maintenance Department personnel.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 (Hot standby)

DESCRIPTION OF OCCURRENCE:

On June 9, 1993, at 1443 hours0.0167 days <br />0.401 hours <br />0.00239 weeks <br />5.490615e-4 months <br />, lC Vital Bus {EB} sensed an undervoltage condition resulting in an automatic start and blackout loading of lC Diesel Generator (D/G) {EK}. The automatic D/G start and loading is an Engineered Safety Feature (ESF) . _At the time of the event, Maintenance personnel were*conducting the monthly functional surveillance test of the lC 4KV Vital Bus in accordance with procedure Sl.MD-FT.4KV-0003(Q), "ESFAS Instrumentation Monthly Functional Test lC 4KV Vital Bus Undervoltage".

On June 9, 1993, at 1536 hours0.0178 days <br />0.427 hours <br />0.00254 weeks <br />5.84448e-4 months <br />, in accordance with Code of Federal Regulations lOCFR 50.72(b) (2) (ii), the Nuclear Regulatory Commission (NRC) was notified of :the lC 4KV Vital Bus sensed undervoltage and subsequent lC D/G start and load.

ANALYSIS OF OCCURRENCE:

The 4KV Vital Busses-provide load to various vital (lE) equipment.

The lC 4KV Vital Bus specifically provides load to the lC 460 VAC Vital Bus; the lC 230 VAC Vital Bus and the following safety related pumps:

No._ 12 Centrifugal Charging Pump {BQ}

No. 12 Safety Injection Pump {BQ}

No. 12 Containment Spray Pump {BE}

No. 11 Service Water Pump {BI}

This event was the result of a personnel error. Per design, the lC D/G started, connected to the lC Vital Bus and loaded the bus (as applicable). All equipment functioned as designed. Due to the automatic actuation ESF signal, this event is* reportable in accordance with Code of Federal Regulations lOCFR 50.73(a) (2) (iv).

APPARENT CAUSE OF OCCURRENCE:

The root cause of the lC Vital Bus trip is personnel error attributed

  • to inattention to detail. The Maintenance technician did not
  • self-check as required by station work practices.

The technician conducting the monthly functional surveillance test was supposed to place*the 1B3 test signal switch (one (1) of three*

PRIOR SIMILAR OCCURRENCES:

A similar event occurred on* July 27, 1992, reference Salem Unit 2 LER 311/92~013-00. 2A Vital Bus sensed an undervoltage condition resulting in an automatic start and blackout loading of 2A D/G. The cause of the event was personnel error. Although both the 1992 event and the current event involve personnel er~or, the specific circumstances differ. In the 1992 event, the technician placed a second switch in the 2A cabinet to the "on" position instead of placing the 2B cabinet switch to the "on" position.*

SAFETY SIGNIFICANCE:

As stated above, all equipment functioned as designed. The lA and lB 4KV Vital Busses and their associated D/Gs were available during this event. This event did not affect the health or safety of the public.

CORRECTIVE ACTION:

This event has been reviewed by Maintenance Department management.

Positive disciplinary action with the individual(s) involved.has been completed~

The circumstances surrounding this event will be reviewed with .
