Examinations were conducted by PSE&G as as companies under contract to PSE&G. The following is a brief summary with further details found in the attached report and on file at the Salem Generating Station. Since nearly all inservice

Examination Summary 11.1 Technical Specification 4.0.5 -ASME XI (1986 Edition) During the Spring 1993 refueling outage, PSE&G, MQS and SwRI personnel performed nondestructive tions (NOE) of selected Class 1, Class 2 and Class 3 components of the Salem Generating Station, Unit 2. These examinations constituted the first inservice examination (ISI) of the second inspection interval of commercial operation.

11.1.1 ISI Examination 11.1.2 Southwest Research Institute (SwRI) under the direction of the Salem ISI Group conducted ninety-five (95) manual ultrasonic, one dred and three {103) liquid penetrant and twenty-six

This laminar reflector was able in accordance with Table IWC-3510-2 of the 1986 edition of Section XI, ASME Code. There were no other reportable indications, evaluated as an unacceptable flaw. Examination Summary (Cont'd) 11.2 Visual Examination on Valves and Bolting 11.2.1 The Salem Isr Group conducted eighty-four (84) Class 1 bolting exams (100%) in conjunction with Relief Request RR-A7 and six (6) Class 1 valve internal exams. ;*-;;_,!,;

-:.: (""?:' NOTE: Relief request RR-A7, allowed a visual inspection of all Class 1 bolted nections during refueling outages for evidence of boric acid residue with the insulation removed, in lieu of a VT-2 during plant heat-up, per ASME Section XI, 1986 edition IWA-5242(a).

There were several Class 1 Bolting exam *lies noted, including; rust, boric acid mulation, galling, tool marks, nuts seized on the stud or bolt, etc .* Work Orders and/or Deficiency Reports were generated to document and correct all anomalies.

No additional exams were performed due to these anomalies, because 100% of all components in that ry were examined.

Examination Summary (Cont'd) 11.3 Visual Examinations of Supports 11.3.1 The Salem ISI Group conducted visual tions on thirty-four (34) Class I, seven (27) Class II and two hundred and teen (219) Class III supports using Code case N-491. There were no other reportable indications, evaluated as an unacceptable flaw. 11.4 Preservice Examination of the #24 Steam Generator 14" diameter feed water piping (DCP 2EC-3216) replacement, due to erosion/corrosion wall thinning.

11.4.1 The elbow and a piping section of spool piece 2-SGF-19 were replaced under DCP 2EC-3216, due to erosion/corrosion wall thinning.

Three (3) new welds were generated as follows: ISIC Summary # 330111 330121 330124 ISIC Component ID 14-BF-2241-GR 14-BF-2241-7R 14-BF-2241-7A PSE&G WELD # FW-2-SGF-19-2A 82-SGF-19-3 FW-2-SGF-19-6 The baseline UT was performed by SwRI and the baseline MT was performed by MQS with no discrepancies noted. 11.5 Hydrostatic Tests 11.5.1 Five (5) Hydrostatic tests were completed on the Service Water Coolinq Coils and Motor Coolers of #21, 22, 23, 24 and 25 containment Fan Coil Units. No pressure boundary leakage was identified.

11. Examination Summary (Cont'd) 11.6 System Pressure 11.6.1 System leakage exams were performed in ance with ASME Section XI on all Class 1 systems. Relief Request RR-A7 was approved and used during this refueling outage. A system leakage test was performed with the Reactor Coolant System in mode 3 (normal operating temperature and pressure) with the insulation installed.

No pressure boundary leakage was detected.

Minor mechanical age was identified and work requests were initiated for repairs. Fifty-three (53) system Pressure Tests {SPT) were completed during a System Functional or Inservice Test on Nuclear Class 1, 2 and 3 components in accordance with ASME Section XI. No pressure boundary leakage was detected.

Work requests were initiated to correct minor mechanical leakage that was identified.

A non-tech spec. sample of one thou-sand five hundred and eight (1,508) tubes in #22 Steam Generator were examined to try and comply with EPRI recommended examination guidelines of performing 100% examination.of all Steam Generator tubes over a six year period. This sample combined with the Tech. Spec. sample brings tlie original percentage of tubes inspected in this Generator for_ this outage to a total of 50%. The bobbin program was expanded by 12% (per Tech Spec), due to indications.

The total number of tubes inspected in this tor this outage was 2126 or 62%. D. A non-tech.

spec. sample of one thou-sand five hundred and thirty (1,535) tubes in #24 Steam Generator were examined to try and comply with EPRI recommended examination guidelines of performing 100% examination of all Steam Generator tubes over a six year period. This sample combined with the Tech. Spec. sample brings the original percentage of tubes inspected in this Generator for this outage to a total of 51% . E. 50% (1,694) of #22 Steam Generator tubes were inspected with rotating pancake probe, to address the concern of Expansion Zone Pure Water Stress Corrosion cracking at the hot leg tube sheet (WEXTEX) for Steam Generators manufactured using sive tube expansion techniques.

The RPC program was expanded to 100% due to cations. The total number of tubes spected on this Generator this outage is a total of 3283. Page 9 of 21

A total of 16 tubes in #22 Steam Generator and #22 tubes in #24 Steam Generator were plugged this outage. A summary of Eddy Current results and tube plugging follows: A. #22 Steam Generator

1. The Tech. Spec. sample in #22 Steam Generator revealed two (2) indications exceeding the plugging limit requiring a scope expansion of 12%. These two tubes were plugged. 2. Eleven (11) tubes were plugged due to single axial indications found in the hot leg tube sheet region during RPC inspection.

2. Twelve (12) tubes were plugged due to single axial indications found in the hot leg tube sheet region during RPC inspection.

11. Examination Summary (Cont'd) 11.8 Current Status of the Steam Generators At this time the total number of tubes plugged in the Salem Unit 2 Steam Generators are as follows: Generator

21 22 23 24 Tubes Plugged 112 121 112 133 NOTE: The first.five and the last five tubes in Row 1 in each of the Steam Generators were mechanically plugged due to a vibration in the tube lane blocking device. The ing Row 1 tubes were preventively plugged due to a wear problem on the tubes caused by a vibration in the tube lane blocking vice. The remaining Row 1 tubes were ventively plugged due to the onset of stress corrosion cracking in the U-bend region as evidenced in two of the four Generators.

11.9 Technical Specification 4.6.l.2d, Containment Type "B" {Penetrations) and Type "C" {Valves) Leak Rate Testing. 11.9.1 Summary The PSE&G Research Corp. under the field vision of the Salem ISI Group, conducted local leak rate tests on sixty-three

11. Examination Summary (Cont'd) 11. 9. 2 Results A. Several Type "C" valves exceeded the mended leak rate acceptance criteria.

These valves were reworked to an acceptable condition and retested satisfactorily.

B. A total As-Found leakage (min path) of 13,560.4 SCCM was measured through all Type "B" and."C" containment penetrations.

After corrective maintenance, a total Left leakage (max path) of 28,075.1 SCCM was measured through all Type "B" and*"C" penetrations.

11.10 Technical Specification 4.6.1.3b Elevation 100 1 and 130 1 Airlock Leak Rate Tests 11.10.1 Summary PSE&G Research Corporation under the field supervision of the Salem ISI Group conducted leak rate tests on the 100 1 and 130' ment personnel airlocks.

11.10.2 Results The combined As-Found leakage for these airlocks was 4,040.0 SCCM. After nance, the combined As-Left leakage for the airlocks was 5,232.0 SCCM. Both of these measured combined leakage rates satisfies the Technical Specification acceptance criteria of s 0.05 La (10,812 SCCM)

Examination Summary (Cont'd) 11.11 Technical Specification 4.7.9.(a) "Visual Inspection of Hydraulic and Mechanical Snubbers" 11.11.1 Summary PSE&G personnel under the direction of the Salem ISI Group conducted visual examinations on twenty four (24) hydraulic and one hundred twelve (112) mechanical snubbers.

11.11. 2 Results A. There were no discrepancies noted which would affect snubber operability.

There were some minor items noted including reservoirs not full, missing washers, loose spherical bearings and binding around the bearings.

These conditions were all noted and corrected.

B. All snubbers which were removed and reinstalled received an as-left visual *exam * . 11.12 Technical Specification 4.7.9 (c) "Functional Testing of Selected Hydraulic and Mechanical Snubbers" 11.12.1 Summary -Hydraulic Snubbers Per Technical Specifications 10% of the installed hydraulic snubbers as well as all previous outage functional test failures are required to be tested. Summary -Mechanical Snubbers Per Technical Specifications, 10% of the total installed mechanical snubbers as well as previous outage functional test failures were tested utilizing the WYLE Model 150 Snubber Testing Machine. The 10% sample is broken down by snubber category size (small and mediums) with 10% from each size selected each outage. All selected snubbers are detailed below: A. The 10% sample of the medium size bers included one (1) PSA-l's, seven (7) PSA-3's and one (1) PSA-lO's.

B. Due to previous outage failures, 100% of the small size snubbers and equiv. hydraulic snubbers) are tested each outage. This resulted in tests being performed on eighteen (18) Model three (3) Model and One (1) Model Lisega 3018. c. Two (2A-SISN-105N

& 2A-SISN-105S) failed the running drag functional test and were replaced in kind. Snubber MSDSN-1003 was found with environmental damage and replaced in kind. The running drags were high but acceptable.

Summary Southwest Research Institute (SwRI) under the direction of the ISI Group performed an in-place ultrasonic examination (UT) on the #22 (S/N 1S-88P318) and #23 (S/N 1S-76P917)

Reactor Coolant Pump (RCP) Flywheels.

The Flywheels are tracked by the RCP Motor serial number and original pump location.

There were no discrepancies noted, during these examinations.

11.14 NRC Bulletin 82-02 "Degradation of Threaded Fasteners in the Reactor Coolant Pressure Boundary of Power Plants" 11.14.1 Summary Visual (VT) and Magnetic Particle (MT) exams were performed on the primary manway bolts for two (2) Steam Generators.

One (1) bolt on #22 Steam Generator cold leg was rejected during fluorescent magnetic particle for an unacceptable indication.

Deficiency Report and Code Job Package were prepared to replace the bolt in kind. 11.15 NRC Bulletin No. 88-08, "Thermal Stresses in Piping Connected to Reactor Coolant Systems".

In accordance with PSE&G's response to Bulletin 88-08 and subsequent commitments sixteen (16) 1.5-inch Safety Injection welds, and two (2) 2-inch .and five (5) 3-inch Chemical and Volume Control welds were examined with UT and PT. 11. Examination Summary (Cont'd) 11.16 NRC Bulletin 88-11 "Pressurizer Surge Line Thermal Stratification", visual examinations were performed on all existing surge line pipe supports / HEBA straints and any insulation damage. A total of nine (9) supports were examined.

There were no discrepancies noted during these nation. 11.17 NRC Bulletin 87-01, *"Thinning of Pipe Walls in Nuclear Power Plants", measurements were taken at one hundred and thirty nine (139) locations to identify possible pipe corrosion/erosion.

Significant thinning in several components resulted in thirty (30) expansion components being examined.

These components were primarily in the (MSR) system and required ment. Other than insignificant thinning noted in a few areas, no other conditions were observed.

11.18 Summary NuReg 0578 -TMI Lessons Learned -Twenty-seven (27) System Pressure Tests were completed per NUREG 0578 commitments.

Completed System Pressure Tests on the following systems: Chemical Volume and Control Containment Spray , , Liquid Radwaste Residual Heat Removal Safety Injection Sampling Waste Gas Work requests were initiated for repairs of identified leakage. A thru wall leak (linear indication) on the bonnet of 2CV55 was identified under work order number 931013014.

Initiated DR SMD-93-241 and repairs were completed.
