The 21 SPT infeed breaker (21ASD).did riot close resulting in an automatic start and blackout loading of 2A Diesel Generator (D/G). The automatic start and loadl.ng of the D/G is an Engineered Safety Feature .. (ESF). The cause of the 2A Vital :aus trip is equipment failure. A fuse in ihe 21 SPT potential secondary, that supplies power to the meteritig circuit, had A review was conducted of other fuse concerns (in similar Additional similar failures of this type.fuse.were not identified.

This event is considered to be *an isolated occurrence. -The 2A 4KV Vital Bus was reconnected to No. 22 SPT at 2225 hours0.0258 days <br />0.618 hours <br />0.00368 weeks <br />8.466125e-4 months <br /> on January 4; 1992. The failed fuse was replaced.

On January 4i at 2143 hours0.0248 days <br />0.595 hours <br />0.00354 weeks <br />8.154115e-4 months <br />, 2A Vital Bus {EBI tripped during*a transfer of the bus from 22 Station Power Transforiner (SP'i'} to 21*

The 21 SPT infeed breaker (21ASD) did not close resulting in an automatic start and blackout loading of 2A Diesel Generator (D/G} [EKI. The automatic start and loading of the.DIG.is an Engineered Safety Feature (ESF). -. On January 4, 1992 at 2320 hours0.0269 days <br />0.644 hours <br />0.00384 weeks <br />8.8276e-4 months <br />, in accordance with Code of Federal Regulations lOCFR 50.72(b} (2) (ii), the Nuclear-Regulatory Commission (NRc)*was notified of the inadvertent start of 2A D/G. APPARENT CAUSE OF OCCURRENCE:

The cause of the 2A Vital Bus trip is equipment failure. A cartridge fuse in the 21 SPT potential transformer secondary, that supplies po_wer to the* metering circuit, had failed. A review was conducted of other fuse concerns (in simi.lar systems) . Additional similar failures of this type fuse were not identified.

This: event .is considered to be an isolated occurrence. . ANALYSIS OF OCCURRENCE:

CORRECTIVE ACTION: The 2A 4KV Vital Bus .was reconnected to No. 22 SPT at. 2225 hours0.0258 days <br />0.618 hours <br />0.00368 weeks <br />8.466125e-4 months <br /> on January .4, 1992. The failed fuse was replaced.
