NRC INSPECTION REPORT 70-925/95-02 This refers to the inspection conducted by Mr. R. J. Evans of this office on October 17-19, 1995. The purpose of the inspection was to determine whether activities authorized by the license were conducted safely and in accordance with NRC requirements.

At the conclusion of the inspection, the findings were discussed with those members of your staff identified in the enclosed report.Areas examined during the inspection are identified in the report. The inspection consisted of a review of the "Phase I" final survey program and related activities.

Within these areas, the inspeCtion consisted of selected examination of procedures and representative records, interviews with personnel, and observation of activities in progress.During the inspection, representatives from the Oak Ridge Institute for Science and Education (ORISE) performed on-site, direct measurements of radioactivity, in part, to independently assess the reliability and validity of the results of your final survey program. The results of ORISE's investigation were not available at the end of the inspection period and will be submitted to you under separate correspondence.

In accordance with 10 CFR 2.790 of the NRC's "Rules-of Practice,".a
