Descriotion of the Noncomoliance:
On February 28, 1997, the flow integrator used to measure                                      )
Spray Pond effluent flow was identified to be malfunctioning and was declared inoperable. A subsequent evaluation determined that the flow integrator had been inoperable since January 7, 1997. This instrument, required by Part A of the permit, is used to measure the contribution of Spray Pond flow to the total integrated flow at Discharge 001. To ensure compliance with Part A, Discharge 001 flow is required to be measured once per week. Until repairs to the instrurent were completed, an alternate method of determining the Spray Pond effluent flow was performed weekly using measurement of the water height over the Spray Pond discharge weir. On March 6, 1997, repairs to the flow integrator were completed, and the instrument was declared operable.
Cause of the Noncomoliance:                                                            d??
The Spray Pond flow integrator was malfunctioning as a result of debris lodged within the instrument's flow port.                                      l Previous corrective actions for minimizing flow integrator malfunctions due to debris and for increased monitoring of                          i 9        i the flow integrator readings to detect malfunctions were not effective in identifying incorrect flow integrator readings.

The procedure for recording.the Spray Pond effluent flow has been revised to perform further investigation of the Spray Pond overflow when the flow integrator reading does not change:over a week's time. If actual Spray Pond overflow effluent is discovered without a change in the flow integrator, the flow rate will be determined by measuring the height of water over the Spray Pond discharge weir, the flow integrator instrument will be inspected, and the System Manager will be notified.
An Engineering Change Request (ECR) has been initiated for the design and installation of a screen at the Spray Pond overflow weir to minimize debris entering the flow integrator. This ECR is expected to be implemented by August 31',  1997.
