: 1. INTRODUCTION The attached Form NIS-1, ' Owner's Data Report for Inservice inspections", describes the inservice inspections performed during 1995 at Vermont Yankee Nuclear Power Station, Vernon, Vermont.
The examinations performed were those of the second outage of the first period of the third ten-1                year interval, There is one remaining outage in the first period of the third ten-year interval. The ,
nondestructive examination procedures used for inservice inspection were in accordance with the ASME Boiler and Pressure Vessel Code, Section XI,1986 Edition, as referenced in the plant Technical Specifications. The areas subject to examination and the methods used were in -
l                accordance with the Vermont Yankee inservice inspection Program and the plant Technical Specifications. This report summarizes the components examined, the types of examinations, the        l l                status of the current interva!, the non-conforming conditions noted, and the corrective actions taken. Ultrasonic, liquid penetrant, magnetic particle and visual examination techniques were employed to perform the required examinations.

EXAMINATION METHQQS L                Nondestructive examinations were performed in accordance with the procedures contained in the l                Yankee Atomic Electric Company Engineering Guidelines, Book 111, *lnservice inspection NDE Procedures", or vendor procedures reviewed and approved by Yankee Atomic Electric Company.
The examination procedures were reviewed and approved by personnel qualified to ASNT-TC-1 A, Level ill. These procedures conform to the requirements of ASME Section XI,1986 Edition. The inse.tvice examinations were performed and evaluated by personnel qualified as provided for in        ,
l                Section XI.
: 3. EVALUATION OF DATA The examinations were performed and results reviewed at the site by personnel qualified to at least Level ll in the examination discipline. Indications were evaluated to the acceptance standards defined in the Vermont Yankee Nuclear Power Station Technical Specifications, ASME Section XI, 1986 Edition, and the Yankee Atomic Electric Company Engineering Guidelines, Book 111,

ALSO, THERE WAS A LOOSE NUT AT THE TOP OF THE FW-12 SIDE OF THE HANGER. WORK ORDER 95-03147-00 WAS GENERATED TO RESET THE SPRING CAN SETTING AND TIGHTEN THE LOOSE NUT. FOLLOW UP VISUAL EXAMINATION WAS ACCEPTABLE. THE OUT OF TOLERANCE SPRING CAN SETTING WAS DETERMINED TO BE SERVICE RELATED AND ADDITIONAL HANGERS WERE EXAMINED. IN THE UPSTREAM DIRECTION, FW-14 WAS EXAMINED AND FOUND TO BE ACCEPTABLE. NO SUPPORTS EXIST DOWNSTREAM BETWEEN FW-12 & FW-13 AND THE REACTOR PRESSURE VESSEL SO NO EXPANSION WAS DONE IN THAT DIRECTION. THE EXPANSION SELECTION INCLUDED FW-18 & FW-19 (SAME TYPE AND DESIGN AND FUNCTION) . FW-18 & FW-19 WAS .
EXAMINED AND FOUND TO BE ACCEPTABLE. THE LOOSE NUT WAS EVALUATED TO BE INSIGNIFICANT TO THE PROPER FUNCTION OF THE COMPONENT. NO FURTHER EXPANSION WAS REQUIRED F-A              RCW-H161      VISUAL EXAMINATION OF RCW-H161 REVEALED LOOSE BOLTS ON THE STRUT TO PIPE CLAMP AND STRUT TO BUILDING CONNECTIONS. WORK ORDER 95-03148-00 WAS GENERATED TO TIGHTEN THE LOOSE BOLTING. FOLLOW UP VISUAL EXAMINATION WAS ACCEPTABLE. 1. INTRODUCTION The attached Form NIS-1, ' Owner's Data Report for Inservice inspections", describes the inservice inspections performed during 1995 at Vermont Yankee Nuclear Power Station, Vernon, Vermont.

The examinations performed were those of the second outage of the first period of the third ten-1 year interval, There is one remaining outage in the first period of the third ten-year interval. The ,

nondestructive examination procedures used for inservice inspection were in accordance with the ASME Boiler and Pressure Vessel Code,Section XI,1986 Edition, as referenced in the plant Technical Specifications. The areas subject to examination and the methods used were in -

l accordance with the Vermont Yankee inservice inspection Program and the plant Technical Specifications. This report summarizes the components examined, the types of examinations, the l l status of the current interva!, the non-conforming conditions noted, and the corrective actions taken. Ultrasonic, liquid penetrant, magnetic particle and visual examination techniques were employed to perform the required examinations.

2. EXAMINATION METHQQS L Nondestructive examinations were performed in accordance with the procedures contained in the l Yankee Atomic Electric Company Engineering Guidelines, Book 111, *lnservice inspection NDE Procedures", or vendor procedures reviewed and approved by Yankee Atomic Electric Company.

The examination procedures were reviewed and approved by personnel qualified to ASNT-TC-1 A, Level ill. These procedures conform to the requirements of ASME Section XI,1986 Edition. The inse.tvice examinations were performed and evaluated by personnel qualified as provided for in ,

l Section XI.

3. EVALUATION OF DATA The examinations were performed and results reviewed at the site by personnel qualified to at least Level ll in the examination discipline. Indications were evaluated to the acceptance standards defined in the Vermont Yankee Nuclear Power Station Technical Specifications, ASME Section XI, 1986 Edition, and the Yankee Atomic Electric Company Engineering Guidelines, Book 111,

ALSO, THERE WAS A LOOSE NUT AT THE TOP OF THE FW-12 SIDE OF THE HANGER. WORK ORDER 95-03147-00 WAS GENERATED TO RESET THE SPRING CAN SETTING AND TIGHTEN THE LOOSE NUT. FOLLOW UP VISUAL EXAMINATION WAS ACCEPTABLE. THE OUT OF TOLERANCE SPRING CAN SETTING WAS DETERMINED TO BE SERVICE RELATED AND ADDITIONAL HANGERS WERE EXAMINED. IN THE UPSTREAM DIRECTION, FW-14 WAS EXAMINED AND FOUND TO BE ACCEPTABLE. NO SUPPORTS EXIST DOWNSTREAM BETWEEN FW-12 & FW-13 AND THE REACTOR PRESSURE VESSEL SO NO EXPANSION WAS DONE IN THAT DIRECTION. THE EXPANSION SELECTION INCLUDED FW-18 & FW-19 (SAME TYPE AND DESIGN AND FUNCTION) . FW-18 & FW-19 WAS .

EXAMINED AND FOUND TO BE ACCEPTABLE. THE LOOSE NUT WAS EVALUATED TO BE INSIGNIFICANT TO THE PROPER FUNCTION OF THE COMPONENT. NO FURTHER EXPANSION WAS REQUIRED F-A RCW-H161 VISUAL EXAMINATION OF RCW-H161 REVEALED LOOSE BOLTS ON THE STRUT TO PIPE CLAMP AND STRUT TO BUILDING CONNECTIONS.
