1. INTRODUCTION i The attached Form NIS-1, " Owner's Data Report for Inservice inspection," describesL [~

the inservice inspections performed during 1993 at the Vermont Yankee Nuclear Power Station, Vernon, Vermont. The examinations performed were those 'of the first outage .,

of the first period of the third ten-year interval. The nondestructive examination i procedures used for inservice inspection were in accordance with the ASME Boiler and  !

Pressure Vessel Code,Section XI,1986 Edition, as referenced in the plant Technical  :

Specifications. The areas subject to examination and the method used were in I accordance with the Vermont Yankee Inservice inspection Program and the plant  !

Technical Specifications. This report summarizes the components examined, the types of examinations, the status of the current interval, the non-conforming conditions  ;

noted, and the corrective actions taken. Ultrasonic, liquid penetrant, magnetic particle, a and visual examination techniques were employed to perform the required examinations.

2. EXAMINATION METHODS ,

Nondestructive examinations were performed in accordance with the procedures ,

contained in the Yankee Atomic Electric Company Engineering Guidelines, Book Ill,

" Inservice inspection NDE Procedures," or vendor procedures reviewed and approved by Yankee Atomic Electric Company. The examination procedures were reviewed and ,

approved by personnel qualified to SNT-TC-1 A, Level 111. These procedures conform' ,

to the requirements of ASME Section XI,1986 Edition. The inservice examinations l were performed and evaluated by personnel qualified as provided for in Section.XI. l The procedures used for these examinations are listed in Section 10 of the NIS-1 Furm.

3. EVALUATION OF DATA The examination results were performed and reviewed at the site by personnel j qualified to at least Level II in the examination discipline. Indications were evaluated j to the acceptance standards defined in the Vermont Yankee Nuclear Power Station j' Technical Specifications, ASME Section XI,1986 Edition, and the . Yankee Atomic Electric Company Engineering Guidolines, Book lit, " Inservice Inspection _ NDE 1 Proce A4rto ." j l

Abstract of Conditions Noted / Corrective Measures Taken Section 12/13 ASME Cat item ID Conditions Noted and Corrective Measures Taken F-A H103B Visual examination of H103B revealed a loose reservoir clamp. Work order 93-7932 was issued to tighten the clamp. Follow-up VT-3 was acceptable. This condition was evaluated to

  • be insignificant to proper function of the component.'

F-A HD-1078 Visual examination of HD-1078 revealed spring cans to be out of tolerance. Work order 93-7944 was issued to reset the spring can. Follow-up VT-3 was acceptable. This condition was determined to be service related and additional supports, HPCI-HD-84, HD-107A and HD-107C were inspected. HPCI-HD7 84 and HD-107A were found acceptable. HD-107C was found out of tolerance (see additional information below on HD-107C).

F-A HD-107C Visual examination of HD-107C revealed spring cans to be out of tolerance. Work order 93-8206 was issued to reset the spring can. Follow-up VT-3 was acceptable. This condition was determined to be service related and additional supports, HPCI-HD-35C and HD-35D were inspected. HPCI-HD-35C was found acceptable. HD-35D was found out of tolerance (see additional information below on HD-35D).

F-A HD-35D Visual examination of HD-35D revealed spring cans to be out of tolerance. Work order 93-8518 was issued to reset the spring can. Follow-up VT-3 was acceptable. This condition was determined to be service related and additional support, HPCI-H-32 was inspected. HPCI-H-32 was found acceptable. At this point, all double spring can in the HPCI system were examined.

F-A HD103C' Visual examination of HD103C revealed a loose bolt on the pipe clamp. Work order 93-7943 was issued to tighten the bolt. Follow-up VT-3 was acceptable. This condition was evaluated to be insignificant to proper function of the component.
