SUMMARY

REPORT l

FEBRUARY 1, 1989 THROUGH APRIL 7, 1989

1. INTRODUCTION The attached Form NIS-1, " Owner's Data Report for In-Service Inspection,"

describes the in-service inspections performed during 1989 at the Vermont Yankee Nuclear Power Station, Vernon, Vermont. The examinations performed were those of the last outare of the second period of the second ten-year interval. The nondestructi /e examination procedures used for in-service inspection were in accorda7ce with the ASME Boiler and Pressure Vessel Code,Section XI, " Rules for In-Service Inspection of Nuclear Power Plant Components," as referenced by the plant Technical Specifications. The areas subject to examination and the method used were 'n accordance with the Vermont Yankee In-Service Inspection Program and the plant Technical Specifications. This report summarizes the components examined, the types of examinations, the non-conforming conditions noted, and the corrective actions taken. Ultrasonic, liquid penetrant, magnetic particle, and visual examination techniques were employed to perform the required examinations.

2. EXAMINATION METHODS Nondestruct!ve examinations were performed in accordance with the proce-dures contained in the Yankee Atomic Electric Company Engineering Guidelines, Book III, "In-Service Inspection NDE Procedures," or vendor procedures reviewed and approved by Yankee Atomic Electric Company. The examination procedures were reviewed and approved by personnel qualified to SNT-TC-1A, Level III. These procedures conform to the requirements of ASME Section XI, 1980 Edition, Winter 1980 Addenda, except where these editinnt are in conflict with Technical Specification requirements. The in-service examinations were performed and evaluated by personnel qualified as pro-vided for in Section XI. The procedures used for these examinations are listed in Section 10 of the NIS-1 Form.
3. EVALUATION OF DATA The examination results were reviewed at the site by personnel qualified to at least Level II in the examination discipline. Indications were eva-luated to the acceptance standards defined in the Vermont Yankee Nuclear Power Station Technical Specifications.
4. EXAMINATION RESULTS A list of examinations that were performed is contained in Section 10 of the NIS-1 Form. the detailed examination data along with the calibration records, procedures, equipment certifications, and personnel certifications are maintained at the plant site. Upon completion of the corrective actions detailed in Section 12 of the NIS-1 Form, there were no remaining unacceptable conditions. No continuing or follow-up items resulted from mandatory examinations performed under ASME Section XI.
5. CONCLUSIONS The scheduled inspections accomplished during the 1989 In-Service Inspections constituted the total number of inspections required to be completed during Period 2 of the second In-Service Inspection interval.

Test No. 2 and 19 This test was satisfactorily performed and included the RBCCW pump discharge lines from valves V70-94A and 948, through the RBCCW heat exchangers, fuel pool heat exchangers, the CRD and RHR pump coolers and up to but not including the RCU pump coolers, RCU non-regenerative heat exchanger, and the sample system coolers.

C. Test No. 5 and 9 This hydrostatic test was satisfactorily performed and encompassed the RHR-SW 1B pump motor cooling coil.

D. Test No. 6 and 10 This hydrostatic test was satisfactorily performed and encompassed the j RHR-SW 1D pump motor cooling coil. J E. Test No. 11 This hydrostatic pressure test was satisfactorily performed and included the RBCCW return piping from the CRD pump coolers, RHR pump coolers and the primary containment air compressor. The boundary also included piping from the RBCCW Surge Tank and the chemical feeder tank and extended to the RBCCW pumps.

2.- Core Spray System A. Test No. 1 This hydrostatic test was satisfactorily performed and encompassed the eight inch CS-38 line and a three quarter inch CS vent line. The test boundary extends from the MOV-12B valve to the MOV-118 valve and includes'the 248 valve.

B. Test-No. 3 This hydrostatic test was satisfactorily performed and encompassed the twelve inch CS-1B and ten inch CS-2B lines. The test boundary extended from the MOV-7B to MOV-8B valves and continues downstream through the-pump and ten inch CS-2B to MOV-118.

C. Test No. 4 This hydrostatic test was satisfactorily performed and encompassed the twelve inch CS-1A and ten inch CS-2A lines. The test boundary extended from the MOV-7A to CS-2A and MOV-11A.

3. RHR System i

'h A. Test No. 2 This hydrostatic pressure test was satisfactorily performed and included the RHR shutdown cooling piping from the RHR-17 valve to the RHR-15 (A-D) valves.

4. HPCI System A. Test No. 2 This hydrostatic pressure test was satisfactorily performed and I included the HPCI turbine exhaust line from the HPCI-65 valve to the L HPCI-12 valve.

B. A system hydrostatic test was satisfactorily performed which encom- l passed the piping from FCV-43 to FCV-42. This test was performed to ,

1. The mid-wall ultrasonic reflector identified with the 60* search unit was evaluated in accordance with procedure YA-AS-XI, (acceptance criteria for ASME Sec. XI Examination) and found to be acceptable for continued service.

The indication was not observed during the 45' examination and was deter-mined to be a fabrication defect, not service related.

B. Category B-G-2 q

1. The mechanical damage of the nut on valve V2-539 was evaluated by the Yankee Atomic Mechanical Services Group. The damage was determined to be installa- i tion related and insignificant to the proper function of the component. l
2. .The incomplete thread engagement identified on valve V2-80B was evaluated by the Yankee Atomic Mechanical Services Group. The condition was determined to be installation related and insignificant to the proper function of the component.

-1. The rejectable magnetic particle indications identified on weld C7A'(MS-7A),

MF-4 (FDW-17), MF-4B (FDW-17) and F-1 (MS-4A) were subsequently removed or reduced to acceptable size by' manual surface conditioning. These indica-tions were determined to be seams caused by the pipe fabrication process.

Therefore, no sample expansion was required.

D. Cateaory B-P

1. The leakage identified on the 1" welded flow sens'ing line on the "A" Main Steam Line Flow Element was determined to be the result of inadequate-welding performed during initial construction. ~This connection was weld repaired-and inspected in accordance with IWB 4000. Three other similar connections were inspected and verified acceptable.

~E.- Cateaory F-C

1. 'The incomplete thread engagement identified on supports CS-1, CS-2, RR-83, HPCI-3, and HPCI-4 was evaluated by the Yankee Atomic Mechanical Services Group. The conditions were determined to be installation related and

-insignificant to the proper function of the component. A maintenance _

request was generated and the unbent cotter pin on support HPCI-4 was repaired.

2. A maintenance _ request was generated and a cotter pin was installed on sup-port FW-5. The cause of the missing cotter could not be determined. This condition was evaluated and it was concluded that it was insignificant con-sidering the physical configuration and that the support was operable at all times.
3. Maintenance requests were generated and spring can load settings on supports RR-59, CU-4, MS-1, MS-33, RHR-8 were readjusted. These hangers evidently drifted from their settings due to system dynamics effects.

S-28, (line RHR-1A), P-2 (line CRD-4) were subsequently removed by manual surface conditioning. The indications were determined to be. seams caused by the pipe fabrication process. Therefore, no sample expansion was required.

2. The rejectable linear indication on weld T-33 (line HPCI-3) was evaluated in accorCsnce with procedure YA-AS-XI (Acceptance Criteria for Section XI

-Examination) and found to be acceptable for continued service.

C. Category F-C

1. The incomplete thread engagement identified on supports H-191 (RHR-9), H-22 (HPCI-2), HD-202 (HPCI-BPMP), VI (SGT-14), H-56 (CS-28), CS-HD-54D (RHR-40) were evaluated by the Yankee Atomic Mechanical Services Group. The con-dition was determined to be installation related and insignificant to.the proper function of the component.
2. A Maintenance Request was generated and the spring can load settings on sup-ports HD-84 and HD-60C were readjusted.
3. Maintenance request were generated and the loose nuts identified on supports HD-85-A, HD-107C, H-87 and HD-24 were tightened.
4. The rod end bushing clearance on support CS-HD-86A was evaluated by engi-neering and determined to be acceptable for continued service.
5. A maintenance request was generated and the cracked weld on support HD-191 was repaired. This condition was evaluated by the Yankee Atomic Mechanical Services Group and determined to be fabrication related.
