SUMMARY

REPORT AUGUST 7, 1987 THROUGH OCTOBER 5, 1987

1. INTRODUCTI_O_N.

The atteched Form NIS-1, " Owner's Data Report for In-Service Inspection " describes the in-service inspections performed during 1987 at the Vermont Yankee Nuclear Power Station, Vernon, Vermont. The examinations performed were those of the first outage of the second period of the second ten-year interval. The nondestructive examination procedures used for in-service inspection were in accordance with the ASME Boiler and j Pressure Vessel Code, Section XI, " Rules for In-Service Inspection of Nuclear Power Plant Components," as referenced by the piant Technical Specifications. The areas subject to examination and the methods used were in accordance with the Vermont Yankee In-Service Inspection Program and the plant Technical Specifications. This report summarizes the components examined, the types of examinations, the nonconforming conditions noted, and the corrective actions taken. Ultrasonic, liquid penetrant, magnetic particle, radiographic, and visual examination techniques were employed to perform the required examination.

2. EXAMINATION METHODS Nondestructive examinations were performed in accordance with the procedures contained in the Yankee. Atomic Electric Company Engineering Guidelines, Book III, "In-Service Inspection NDE Procedures " or vendor procedures reviewed and approved by Yankee Atomic Electric Company. The examination procedures were reviewed and approved by personnel qualified  ;

to SNT-TC-1A Level III. These procedures conform to the requirements of l ASME Section XI, 1980 Edition, Winter 1980 Addenda and the referenced

portions of ASME Section V, 1980 Edition, Winter 1980 Addenda, except i where these editions are in conflict with Technical Specification requirements. The in-service examinations were performed and evaluated by personnel qualified as provided for in Section XI. The procedures  ! used for these examinations are listed in Section 10 of the NIS-1 Form.

3. EVALUATION OF DATA The examination results were reviewed at the site by personnel qualified to at least Level II in the examination discipline. Indications were evaluated to the acceptance standards defined in the Vermont Yankee Nuclear Power Station Technical Specifications.
4. EXAMINATION RESULTS A list of examinations that were performed is contained in Section 10 of the NIS-1 Form. The detailed examination data along with the calibration records, procedures, equipment certifications, and personnel certifications are maintained at the plant site. Upon completion of the corrective actions detailed in Section 12 of the NIS-1 Form, there were no remaining unacceptable conditions other than Ite.ns E and F outlined in Section 11 of this report. No continuing or follow-up items resulted from mandatory examinations performed under ASME Section XI, other than those detailed in Items E and F of Section 11 of this report.

12. ABSTRACT OF CORRECTIVE MEASURES RECOMMENDED AND TAKEN A. Category B-F
1. The rejectable linear indications noted during liquid penetrant examination of nozzle to Safe-End Weld N6A-SE were removed by manual surface conditioning. The final liquid penetrant examination showed that all linear indications had been removed. Ultrasonic thickness measurement and measurements of cavity depth were evaluated by Yankee Atomic Elcetric Company

, engineering personnel and found acceptable. B. Category B-G-1

1. The rejectable linear indications found during magnetic particle examination of RPV Closure Head Nut Nos. 13 and 36 were subsequently solvent cleaned and a liquid penetrant examination was performed in the areas where the indications were found.

The liquid penetrant examinations performed revealed no indications thereby leading to the determination that the indications found by magnetic particle examination were extremely shallow surface scratches.

2. The rejectable linear indications found during magnetic particle examination, of RPV Closure Head Stud Nos. 14 and 18 were subsequently solvent cleaned and a liquid penetrant examination was performed in the areas where the indications were found.

The liquid penetrant examinations performed revealed no indications thereby leading to the determination that the indications found by the magnetic particle examination were extremely shallow surface scratches.

1. The lack of proper thread engagement exhibited during visual examination of bolting on Valves V10-25A (RHR), SV2-70B (MS),

and V10-46B (RHR) was evaluated by the Yankee Atomic Mechanical Services Group and determined to be so small in actual measurement as to be insignificant to the proper functioning of the component. Therefore, no corrective action was taken. See Yankee Atomic Memorandum VYM 205/87. 5999R/1.179

1. These indications were rejectable due to their length in accordance with the standards of Section XI, Table IWB-3514 .2. All indications noted occurred in the base material adjacent to the weld. In all cases, the base material is A-106 Grade B and had no surface examination performed on the welds prior to the 1987 ISI. All indications were investigated to establish the maximum through-wall penetration from the OD surface. In no case did the removal violate the nominal pipe thickness nor did the flaw prove to be deeper than that allowed by the volumetric criteria of Table IWB-3514-2, therefore, no sample expansion was required.

E. Category C-B

1. The welding-related indications found during surface examination of RHR Heat Exchanger Nozzle A-N4 reinforcement ring to nozzle and reinforcement ring to vessel welds are undergoing evaluation to determine if corrective action is to be taken. No surface examinations were required at construction and surface conditioning needed to properly examine these welds could reduce weld size to less than nominal design. Surface examinations, an completed, showed no service-related indications.

F. Category C-C

1. Welding-related indications found during surface examination of the RHR heat exchanger support bracket wold on Support Bracket A-RHR-CC-3 are undergoing evaluation to determine if corrective action is to be taken. These indications were rejectable due to their length in accordance with the standards of Section XI, Table IWB-3514-2. All indications were investigated to establish the maximum through-wall penetration from the OD surface. 2. The one rejectable linear indication found during magnetic particle examination of RHR Weld S-18 (also listed above) that did lie in the weld itself was subsequently removed by manual surface conditioning. The maximum through-wall dimension was 12% of the component thickness. The allowable through-wall dimension per ASME Section XI, IWB-3514 is 14% of the component thickness; therefore, the indication is not rejectable to Section XI and no samp.le expansion is required.

H. Category C-G

1. The rejectable indications found during magnetic particle examination of RHR Welds S-4 and S-26 and HPCI Welds F-8 and F-14 were subsequently removed or reduced to acceptable size by l manual surface conditioning. These indications were rejectable due to their length in accordance with the standards of Section XI, Table IWB-3514-2. All indications noted occurred in the base material adjacent to the weld. In all cases, the base material is A-106 Grade B and the weld had no surface examination performed prior to the 1987 ISI. 1. The loose bolt found on Hanger MS-32 was evaluated and it was determined that the nuts were tight and that ireedom of movement in that area of che pipe c1&.np is necessary for proper functioning. An expanded Jample was selected (MS-35) and inspected. No further problems were encountered.

2. The nut on the pipe clamp of HPCI Hanger HPCI-2 that exhibits lack of proper thread engagement was evaluated by Yankee Atomic's Mechanical Services Engineering Group and it was determined that since the bolt is in shear it should be accepted as is. See Yankee Atomic Memorandum VYM 205/87.
3. The cracked tack weld exhibited on the connect!on to the pipi on SPCI Hanger HD--201 was originally used for alignment pv.rposes during fabrication of the spcol piece. The tack weld served no purpose for the support to functica properly, therefore, the tack weld was ground off and the area of removal was subjected to a liquid penetrant examination. No indications were found.

There is an identical support on the same system that was examined as a result of the indication found on Hanger HD-201. HPCI Hanger HD-202 exhibited the same cracked tack weld as HD-201. The same corrective action was taken for Hanger HD-202 and no indications were found when the tack weld removal area was liquid penetrant examined. i
