21.21 UNSPECIFIED PARAGRAPH PART 21 INVOLVING A PUMP SHAFT COUPLING FAILURE A service water pump failed during operation on 9/29/2009. Upon disassembly it was detected that a shaft coupling installed on a repaired service water pump failed while in service. The failed coupling was replaced with a new coupling of a different heat code. According to the manufacturer, the threaded coupling design was only supplied for Palisades.

HydroAire made the assumption that Palisades would invoke part 21 under their QA program following USNRC guideline Section 21.2, Para (c) .This assumption was further supported by the corrective action request LO-CAR-2009-00103 initiated by Entergy dated 10/05/09, which was addressing the failure of P-7C service water pumps at Palisades.

1. A) DESCRIPTION OF NONCONFORMANCE; Palisades informed HydroAire via telephone conversation that a service water pump serial No P-7C failed during operation causing" loss of service water". Upon disassembly it was detected a coupling (2nd from the top) fractured and caused this failure.

B) RECOMMENDATION; Perform needed evaluations to find the cause for this failure of the coupling and determine the root cause. Notify appropriate individuals/groups of the findings and recommendations coming out of the root cause study.

Form a review team to determine and analyze the root cause. Update this Corrective action report upon completion.

None required at this time. Internal record search indicates, threaded coupling design was only manufactured for Palisades only. In other words, HydroAire never supplied threaded coupling to any other nuclear utility other than Palisades.

Received alarm EK- 1149, SERVICE WATER PUMPS STANDBY PUMP RUNNING, EK- 1163, CRITICAL SERV WATER HEADER 'B' LO PRESSURE, EK-] 164, CRITICAL SERV WATER HEADER 'A' LO PRESSURE, and EK-l 165, NONCRITICAL SERVICE WATER LOW PRESSURE, unexpectedly. Service Water P-7B started in Standby.

Service Water Pump P-7C was operating xith 3 lamps (normally greater than 80) and local indication of duress (shaft visibly vibrating with no discharge pressure). Secured P-7C. At the beginning of shift. Service Water Pumps P-7A and P-7C were in service with basket strainer DP's at 6 PSID and 5 PSID. Cycled through all the Service Water Pumps leaving P-7A & P-7C I/S. Final Basket Strainer DP's are: P-7A @ 4 PSID. P-7B @ 3.5 PSID. P-7C @ 3.5 PSID. Service Water Header Pressure rose 2.3 PSI. Entered ONP-6. l, "Loss of Service Water" and Technical Specification LCO 3.7.9 (72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />). There is no Emergency Plan impact and the event is not reportable.
