ObservationsandFindinsTheALARAprogramcontinuedtobewellstaffedandwasbeingimplementedinaccordancewithproceduralcontrols.Post-jobreviewswerebeingusedtogenerateexposurereductionplan(ERP)itemsforimprovingtheALARAeffort.Annualandoutageperson-remgoalsforthesitewereestablished,andperson-remgoalsforeachworkgroupweredeveloped.Onaregularperiodicbasis,actualperson-remaccumulatedforthesiteandforeachworkgroupwascomparedtotheprojectionsbasedontheestablishedgoals.Lessons-learnedfrom1999werebeingusedinthedevelopmentoftheannualandoutagegoalsfortheyear2000.Tothisend,theSiteALARACommittee(SAC)establishedactionstoimprovedosecontrolfortheremainderof1999andwasinvolvedinthedevelopmentoflongerrangeexposurereductionactivitiesirtcludingimprovedhighradiationareacontrolsandevaluationofremotemonitoringmethods.Additionally,theSACinitiatedcorrectiveactionstoaddresslong-standingemployeeconcernsand,exposurereductionplanitems.ConclusionsTheALARAprogramremainsacceptable.QualityAssuranceinRP&CActivitiesTheinspectorevaluatedtheeffectivenessofPP&L'sself-identificationandcorrectiveaction'processesintheRP&Carea.Theevaluationincludedaselectivereviewofthevarioussurveillance,audit,andconditionreports.Theinspectoralsodiscussedtheprocesswithseveralcognizantpersonnel.ObservationsandFindinsTheHealthPhysicsProgramSRC/NASAuditNo.99-007wasbroadinscopeanddetailed.ThesevenQualitySurveillanceReportsreviewedweredetailed.Fourassessmentswerereviewedwhichcoveredradiationprotectionmanagement,externaldoseassessment,thevendor-providedTLDirradiationservice,andavendor-providedinstrumentationservice.Thebreadthvaried,buteachwasdetailedandresultedinrecommendationsforimprovement.Approximately160RadiologicalProtectionconditionreportswereissuedintheperiodfromDecember31,1998toNovember22,1999.AnumberofconditionreportsrelatedtoHPwerereviewedandfoundtoincludelowthresholditems.TheinspectorfoundthatforthoseHPconditionreportsreviewed,theissueswereelevatedtoanadequatemanagementlevel,evaluated,andadequatelycorrected.AdditionalinspectionofPP&L'scorrectiveactionprogramisdiscussedinInspectionReports05000387/1999013and05000388/1999013.
 
10c.ConclusionsPP8L'sself-identificationprocessintheareaofradiationprotectionwasgenerally,effective.S1ConductofSecurityandSafeguardsActivitiesDeterminewhethertheconductofsecurityandsafeguardsactivitiesmetPP8L'scommitmentsintheNRC-approvedsecurityplan(thePlan)andNRCregulatoryrequirements.ThesecurityprogramwasinspectedduringtheperiodofNovember29-December3,1999.Areasinspectedincluded:alarmstations;communications;protectedarea(PA)accesscontrolofpersonnel,packagesandvehicles.b.ObservationsandFindinsAlarmStationsTheinspectorverifiedthatthealarmstationswereequippedwithappropriatealarms,surveillanceandcommunicationscapabilities.Theinspectordeterminedininterviewswiththealarmstationoperators,thattheywereknowledgeableoftheirdutiesandresponsibilities.Theinspectoralsoverified,throughobservationsandinterviews,thatthealarmstationswerecontinuouslymanned,independent,anddiverse.Thealarmstationsdidnotcontainanyoperationalactivitiesthatcouldinterferewiththeexecutionofthedetection,assessment,andresponsefunctions.CommunicationsTheinspectordeterminedbydocumentreviewsanddiscussionswithalarmstationoperators,thatthealarmstationswerecapableofmaintainingcontinuousintercommunicationsandcommunicationswitheachsecurityforcemember(SFM)ondutyandwereexercisingcommunicationmethodswiththelocallawenforcement=agenciesascommittedtointhePlan.PAAccessControlofPersonnelVehiclesandHand-CarriedPackaesandMaterialOnNovember30,andDecember1,1999,theinspectorobservedpersonnelandpackagesearchactivitiesatthepersonnelaccessportal.TheinspectordeterminedthatpositivecontrolswereinplacetoensureonlyauthorizedindividualsweregrantedaccesstothePA,thatallpersonnelandhand-carrieditemsenteringthePAwereproperlysearchedandthatvehiclesenteringthePAwereproperlycontrolled,andsearched.C.ConclusionsSecurityandsafeguardsactivitieswithrespecttoalarmstationcontrols,communications,andprotectedareaaccesscontrolofpersonnel,packagesandvehicleswereeffectivelyimplementedandmetPP&LcommitmentsandNRCrequirements.

04OperatorKnowledgeandPerformance04.1Unit2ReactorShutdownandPlantRestartTheinspectorsreviewedPP8L'sresponsetoanincreaseinprimarycontainmentunidentifiedwaterleakage;themanagementevaluationanddecisiontoperformacontrolledreactorshutdown;andtheassessmentoftheoperatoractionsduringtheplantshutdown,equipmentrepairs,andsubsequentreactorstartup.Cb.ObservationsandFindinsInresponsetoanincreasingprimarycontainmentunidentifiedwaterleakage,operatorsstartedreducingreactorpoweronUnit2onDecember16,toallo'wforinspectioninsidetheprimarycontainment.Primarycontainmentunidentifiedwaterleakagehadincreasedto0.87gallonsperminute(gpm.)TheTechnicalSpecification(TS)limitwas5gpm.ThePP8Linspectiondiscoveredasmallwaterleakona3/4inchinstrumentpipeassociatedwiththepressureindicationforthe"A"reactorrecirculationpumpseal.Theleakwaslocatedatawelded45degreeelbowconnection.PP8Ldeterminedthattheweldcrackedduetohighcyclefatiguecausedbyvibrationalstresses.Thelinewasreplacedwithamodifiedpipethateliminatedthe45degreeelbowweld.Inaddition,apipehangerwasaddedtoprovideaddedsupport.PP8Lins'pectedthe"B"pumppipeandothersimilarreactorrecirculationpipesandfoundnoadditionalproblems.The"A"reactorrecirculationpumppiperepairswerecompletedDecember20,1999.PP8Lmanagementdecidedtokeeptheunitshutdowntorepair.severalotherequipmentreliabilityproblemspriortoplantrestart.ThedecisiontocorrectadditionalknownequipmentproblemspriortorestartwasinlinewithPP&L'snewpolicyconcerningimprovementinequipmentreliability.Unit2wasrestartedsafelyonDecember23,andreachedfullpoweronDecember25.C.ConclusionsPP&LmadeconservativeandeffectivedecisionsinresponsetoincreasingprimarycontainmentleakageonUnit2.08MiscellaneousOperationsIssues'8.1LicenseeEventReortLERReview(71707,92700)ClosedLER50-387/99-004-00ReactorCoreIsolationCooling(RCIC)ManuallyIsolatedduetoFailureofSteamLeakDetectorTemperatureSwitch 10c.ConclusionsPP8L'sself-identificationprocessintheareaofradiationprotectionwasgenerally,effective.S1ConductofSecurityandSafeguardsActivitiesDeterminewhethertheconductofsecurityandsafeguardsactivitiesmetPP8L'scommitmentsintheNRC-approvedsecurityplan(thePlan)andNRCregulatoryrequirements.ThesecurityprogramwasinspectedduringtheperiodofNovember29-December3,1999.Areasinspectedincluded:alarmstations;communications;protectedarea(PA)accesscontrolofpersonnel,packagesandvehicles.b.ObservationsandFindinsAlarmStationsTheinspectorverifiedthatthealarmstationswereequippedwithappropriatealarms,surveillanceandcommunicationscapabilities.Theinspectordeterminedininterviewswiththealarmstationoperators,thattheywereknowledgeableoftheirdutiesandresponsibilities.Theinspectoralsoverified,throughobservationsandinterviews,thatthealarmstationswerecontinuouslymanned,independent,anddiverse.Thealarmstationsdidnotcontainanyoperationalactivitiesthatcouldinterferewiththeexecutionofthedetection,assessment,andresponsefunctions.CommunicationsTheinspectordeterminedbydocumentreviewsanddiscussionswithalarmstationoperators,thatthealarmstationswerecapableofmaintainingcontinuousintercommunicationsandcommunicationswitheachsecurityforcemember(SFM)ondutyandwereexercisingcommunicationmethodswiththelocallawenforcement=agenciesascommittedtointhePlan.PAAccessControlofPersonnelVehiclesandHand-CarriedPackaesandMaterialOnNovember30,andDecember1,1999,theinspectorobservedpersonnelandpackagesearchactivitiesatthepersonnelaccessportal.TheinspectordeterminedthatpositivecontrolswereinplacetoensureonlyauthorizedindividualsweregrantedaccesstothePA,thatallpersonnelandhand-carrieditemsenteringthePAwereproperlysearchedandthatvehiclesenteringthePAwereproperlycontrolled,andsearched.C.ConclusionsSecurityandsafeguardsactivitieswithrespecttoalarmstationcontrols,communications,andprotectedareaaccesscontrolofpersonnel,packagesandvehicleswereeffectivelyimplementedandmetPP&LcommitmentsandNRCrequirements.
