During the performance of SP 44.655.03, Seismic Monitoring Instrumentation Calibration and Functional Test, the following Peak Acceleration Recorders were removed from service for calibration on 9/16/88: 294-ZR006A (Drywell - el. 67') and Z94-ZR006B (Main Steam Line, Reactor Bldg. - el. 131'). During calibration of Z94-ZR006A, new parts were required but were unavailable. New parts were obtained but the calibration could not be completed within the 30 day period. The ZR006B recorder was found to be inoperable. The scratch plates located in the recorder were found to have a white coarse powdery finish instead of a smooth metallic finish. This corrosion of the scratch plates was caused by moisture intrusion through a gasket that became deteriorated due to high temperatures.
This instrurHent is located on a main steam line to satisfy the requirement as committed to in Reg. Guide 1.12, Section C. This instrument is not safety related nor is it required to be Environmentally Qualified. CAUSE_QE_IHE EVENI The ZR006B recorder is physically mounted on a Main Steam Line.
This caused the cover gasket to be exposed to high temperatures -
during the periods that the plant operated at low power. Since the gasket is only rated for a maximum of 250 degrees F this caused the l gasket to deteriorate. When the gasket was removed it was hard and brittle and broke into pieces.
l Due to the deterioration of the cover gasket, moisture intrusion i I
occurred and caused the scratch plates to corrode. These plates, which are made of gold plated aluminum, should have had a smooth l 1
metallic finish but at the time of this event they had a coarse, gray, powdery finish and were beyond the point of being functional. !
Moisture intrusion is also indicated by the condition of the i desiccant which was pink and fused into larger pieces.

- 1. The cover gasket was replaced with a similar model. Since the plant is shutdown and defueled, the gathet will not be exposed to excessive temperatures. ,

- 2. Engineering Change Report, ECR H-1133, has been written to request the installation of a thermal barrier mount. This would protect the seismic recorder from the high temperature of the main steam line. Implementation of this ECR has been deferred indefinetely due to the New York State - LILCO Shoreham Settlement Agreement.
- 3. Station Procedure SP 44.655.03 has been replaced with five smaller surveillance procedures. This will allow the testing required by Technical Specification 4.3.7.2.1 to be performed ,
in stages and will minimize the chance of a seismic monitoring instrumuut, being inoperable for more than 30 days.
