On October 25, 1988, it was discovered that seismic monitoring instrumentation was returned to service prior to completing all cetions necessary for verifying operability as delineated in the curveillance procedure SP 44.655.03, Seismic Monitoring Instramentation Cal, and Funct. Test'. The plant was in Operational Condition 4 with the mode switch in Shutdown and all rods inserted in the core. These instruments are required to be operable at all times per Tech Spec 3.3.7.2. When the instruments were taken out of cervice for calibration, they were required to be returned to cervice within 30 days or the Action Statement would apply. This requires the submittal of a Special Report within 10 days. These instruments were not properly returned to service due to an inadequate review of the test procedure by the surveillance engineer cnd the Action Statement was not met. This event was discovered l during an I&C review of the surveillance tests. Upon discovery, the instruments and their associated monitors were declared inoperable, cwaiting data review and completion of the surveys. Plant Management was notified of the event and the NRC was notified at 0950 on 10/26/88e per 10CFR50.72. This Supplemental Report is being submitted to inform the Commission of the status of corrective l cotions.

.On October 25, 1988, it was discovered that seismic monitoring instrumentation was returned to service prior to completing all cetions necessary for verifying operability as delineated in the curveillance procedure SP 44.655.03, Seismic Monitoring l Instrumentation Calibration and Functional Test. These instruments cre required to be operable at all times per Tech Spec. 3.3.7.2.
When-the instruments were taken out of service for calibration, they were required to be returned to service within 30 days or the Action Statement would apply. The Action Statement required the submittal of a Special Report within 10 days. These instruments were not J properly returned to service by procedure and the Action Statement was not met. This event was discovered during an I&C review of the l I

CAUSE QLIBLEYEH The root cause of the event was personnel error. The individual responsible for the review of the surveillance test did not-odequately ensure that the acceptance criteria was met prior to placing the instruments back in service. In addition, there are inadequacies in SP 44.655.03 that contributed to this event.
As part of the procedure, when the seismic instruments are calibrated and placed back in the recorder, the recorder is reinstalled from where it was removed and then surveyed to ensure it is properly positioned. The need to perform this survey before returning the recorder to service'is identified in the Discussion cection (3.2) of the procedure. Although there are procedural steps for performing the survey, the body of the procedure does not cpecifically state that this must be done prior to returning the rscorders to service. In addition, there are sign-off steps in the instrument Time Keeping Log (Appendix 12.30) to verify removal from cnd return to service of the recorders, however, the sequencing of
.the steps does not ensure that the survey is performed before being i

i ANALYSIS _QE_IBE_ EYE R This event resulted in the violation of the reporting requirements specified in Technical Specification Section 3.3.7.2, Action Statement "a" and is reportable per 10CFR50.73(a)(2)(1). Technical Specifications require a Special Report to be submicted within 10 days following 30 days of seismic monitoring instrumentation inoperability. As discussed above, this reporting requirement was not met.

CORRECTIVE ACTIONS H
- 1. . The surveillance engineer responsible for the event has been reassigned. .
- 2. Station Procedure SP 44.655.03 has been replaced with five smaller surveillance procedures. This will allow the testing required by Technical Specification 4.3.7.2.1 to be performed in stages and will minimize the chance of a seismic monitoring instrument being inoperable for more than 30 days. These new Procedures clearly identify that the survey requirements must j ,~
be met before returning the instruments to service. Additional reviews have also been added to the procedure.
3- An Engineering Evaluation and Assistance Request.(EEAR 89-002) has been initiated to request the installation of permanent alignment pins or mounting plates. These will eliminate the need for surveying the recorders and reduce the 3 mount of time the recorders are inoperable due to surveillance testing. Work i
on the EEAR has been deferred indefinitely due to the New York
! State - LILCO Shoreham Settlement Agreement.
