EDG 102 had just completed its 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> run to satisfy Tech. Spec.
requirement 4.8.1.1.2.8. The next step of the procedure required.a hot-restart of the engine within 5 minutes S Bus 102 was
.deenergized, starting EDG 102. The engine satisfactorily achieved its required frequency and voltage, but the output breaker did not close as' expected. Additional attempts to close the breaker were unsuccessful. The EDG was shut down approximately 2 minutes and 43 seconds after the engine was started. An immediate investigation ensued to determine the cause for the failure of the breaker to close. Operators inspected the cubicle where the breaker (102-8) was located. No unusual conditions were found. The breaker was removed from service and inspected and tested with acceptable results. The diesel test was performed three more times with a replacement breaker, without incident. EDG 102 was returned to service. A multi-disciplined review was conducted to determine the root cause of the failure of the breaker to close after receiving an EDG start signal.

-This is a Supplemental.to a Special Report submitted pursuant to l Technical Specification 6.9.2 to comply with Technical Specification 4.8.1.1.3.
.On March 10, 1989 at 2104, Emergency Diesel Generator (EDG) 102 was
- manually shutdown during an 18 month surveillance test due to the failure of the EDG' output breaker (1R43-ACB-102-B)-to close.
Operators had just completed a portion of SP 24.307.02 (TDI DG Emergency AC Power Load Sequencing Tests) to satisfy Tech. Spec.
requirement =4.8.1.1.2.8 (24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> EDG run) for EDG 102. The next, step of the procedure required a hot-restart of the engine within 5
- minutes. The RSST breaker was placed in " Pull to Lock". Then the NSST breaker was placed in " Pull to Lock" deenergizing Bus 102. The.
engine started and satisfactorily achieved its required generator
. frequency and voltage, but the. output breaker did not close as-expected.
,^ .At;this-time, it was observed that the green indicator light (for breaker position and control. power availability) had dimmed for a moment and then returned to its normal brightness. The red light 6 (closed indication) did not come on. The Watch Engineer instructed

) Control' Room. The EDG was shut down approximately 2 minutes-and 43
{ m onds after the engine had started. An immediate. investigation j onsued to determine the cause for the fallure of the breaker to close.

'(Control. Bldg. Elev. 25') to check the cubicle 102-8 relays (where the 4KV EDG. Output Breaker 102-8 is located) for trips and targets.
S None were found. The cubicle door was opened and no unusual conditions.wpre found. The'4KV Breaker was open, the aprings were charged and the' fuse holders were in place. The NSST and RSST
- supply cubicle relays were checked for trips and targets and none Jwere found. At this time, the operators placed the 4KV 102 bus back into service.
The following morning a visual inspection of the 102-8 breaker was Performed. No abnormal conditions were found. The breaker was cycled 6 times, with satisfactory results. In addition, a voltage was applied to the closing coil and the breaker closed at 50 V DC.
. Resistance readings were taken across the fuses in the circuit with acceptable results. The fuse holder tabs were inspected identifying no abnormal conditions. The tabs were cleaned and regreased with conductive greasc.

aignal. This involved reviewing the plant process computer logs, y plant-personnel statements, Control Room logs and associated electrical circuit-diagrams. However, a definitive cause could not be determined.
Further investigation to. determine the root-cause of the failure of the breaker to close has been deferred indefinitely due to the New '
York State - LILCO Shoreham Settlement Agreement.
