Subject:

REACTOR TRIP DUE TO EQUIPMENT MALFUNCTION; AIT DISPATCHED TO PLANT (THIRD UPDATE) l l On September 11, 1987, the Augmented Inspection Team (AIT) completed its review of the l circumstances surrounding the September 6,1987, reactor trip and subsequent equipment

problems.

l As a followup to AIT inspection, the resident inspectors and an inspector from Region III (Chicago) reviewed the status of maintenance on several systems involved with the reactor trip and related problems on September 12-13. Six systems were selected at random to determine their readiness to support plant restart and power operation. The systems were main feedwater, l auxiliary feedwater, integrated control system, station instrument air, generator hydrogen, l and generator seal oil. The special review included examination of outstanding maintenance l requests and a complete walkdown of the six systems. Additional systems were subjected to a partial walkdown.

l The inspectors concluded that the maintenance backlog for the systems should not affect the l

continued ability of the systems to perform their functions and that the backlog records ,

accurately reflect the condition of the systems examined. l A meeting is scheduled between the NRC staff and licensee management on September 15, 1987, i in Bethesda, Maryland, to discuss issues relating to the startup of the unit, including the results of the licensee's review of the September 6, 1987, reactor trip.

The State of Ohio will be informed of this updated information.

This information is current as of 4 p.m., September 14,19 7.
