ACCIDENTAL WEAPON DISCHARGE

'At about 5:20 a.m. (EDT), July 31, 1987, a security shift supervisor accidentally fired a

' .38 caliber bullet into a filing cabinet in the supervisor's office while performing a

! routine weapon operability test. No one was injured.

l The supervisor's office is located in the plant gatehouse and is adjacent to a bullet-resistent structure. It is not near vital area equipment, iThe supervisor, in accordance with procedures, was performing a weapon operability check on

a .38 caliber revolver used oy a second supervisor. The cylinder was opened to remove the
ammunition prior to the dry fire check. A bullet was prevented from leaving the cylinder by the oversized thumb support on the gun grip. The supervisor did not visually check the cylinder before pulling the trigger. The bullet was retrieved from the filing cabinet.

As a result of the weapon discharge, the licensee has changed procedures and now prohibits

supervisors from checking, storing or cleaning weapons in their office.
The licensee's preliminary investigation determined the incident to be accidental.

Nonetheless, the licensee plans to take disciplinary action against the supervisor.

The NRC Information Assessment Team and NRR Safeguards personnel have been notified.

Region III (Chicago) is following the licensee's course of action.

.The State of Ohio will be notified.
