June 6, 1978 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE -- PNO-78-119 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance.
The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff as of this date.

DEFEAT OF DIESEL GENERATOR SEQUENCER CIRCUIT The Toledo Edison Company reported that during an integrated Safety Features Actuation System (SFAS) test, it was determined that if.a SFAS occurred in conjunction with loss of offsite power, the sequencer for the diesel generator would not function properly.
The automatic loading of critical equipment onto the bus would be negated.
This condition has apparently existed since a modification was made to add a memory interlock to the SFAS system during February 1977.

Davis-Besse is currently in a refueling outage to remove burnable poison I

f assemblies and fuel assembly orifices.
An inspector will be at the site on June 6,1978.
Region III (Chicago) received notification of this event by telephone from the licensee at 4:10 p.m., on June 5,1978.
Region III also received an inquiry on this matter on June 5,1978, from a Toledo, Ohio television station.
Region III will respond to the inquiry, but does not plan a press release at this time.
