WORKER EXTREMITY OVEREXPOSURE An NRC inspection of the subject facility on December 15-19, 1986 identified an apparent hand exposure to a worker in excess of NRC limits (18.75 rems / quarter). Licensee records indicated the worker received a cumulative exposure to the left hand of 21.4 rems during tha July-September quarter,1985. The exposure was received while performing repair work on mechanical manipulator hands which had been removed from licensee hot cells. Exposure to the worker was monitored by TLD rings worn on each hand during the repair work.

Radioactivity on the manipulator hands consisted of a mixture of uranium and fission products. The licensee did not report the overexposure since they erroneously considered th3 exposure to be from byproduct materials which are licensed by the State of New York.

New York regulations allow extremity exnosures up to 25 rem in any consecutive 13 week period.

A follow-up inspection of licensee radiological controls associated with manipulator hand r^ pair was conducted by the NRC Region I on January 14-15, 1987. Several additional violations, concerning licensee survey, personnel monitoring, training, and high radia-tion area control practices, were identified. Preliminary review of licensee dosimetry placement during the hand repair indicated that the recorded dose of 21.4 rems may be inaccurate. During a conference call with Region I officials on January 16, 1987, the licensee committed to perform an accurate dose assessment, and to specific improvements in the radiological controls associated with the manipulator hand repair operation and p; sting and control of high radiation areas. A Confirmatory Action Letter, detailing the actions committed to by the licensee, was issued to the licensee on January 20, 1987.

The State of New York has been notified.
