NOTICE OF VIOLATION .

As a result of the inspection conducted on Marchid "18, 1988, and in' s accordance with the "General Statement of Policy and Procedure for NRC

, Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy 1986),

the.following violations were identified:

A. 10 CFR 19.12, Instructions to Workers, requires, in part, that all "

individuals workingein a restricted area be instructed in precautions or procedures to minimize exposure and in the purposes and functions of.

protecti.ve devices employed.

Contrary to the above, an individual,' working in the licensee's ,

restricted area during thre period February 26 - March 3, 1988, was -

provided inadequate instruction in precautions to minimize contamination ,

, of his person and in the fuoction and purpose of personnel contamination monitors which the individual used. As a result, the individual left the .

facility with an estimated 200,000 disintegrations per minute of radioactive material on his left shoe. p This is a Severity Level IV violation (Supplement IV). i; i

B. 10 CFR 20.201, Surveys, requires, in part, in Section (b), that each licensee shall make or cause to be made such surveys as (1) may be i necessary to comply with the regulations in this part and (2).are reasonable under the circumstances to evaluate the extent of radiation  :

l hazards that may be present. 10 CFR 20,201(a) defines a survey as an -

evaluation of the radiation hazards incident to, among other riatters, the presence of radioactive material and when a;,propriate includes measure- .

Contrary to the above, mea nrements of the ccncentrations of airborne radioactivity present were not made during replacement of a viewing window in Hot Cell No. 1 on February 27, 1988. The measurements were necessary to comply with the regulations in 10 CFR'20.103 and were ,

reasonable under the circumstances to evaluate the extent of radiation hazards that may be present.
