Caussonwesith Edison company letter dated July 10. 1972 reports that, during a test, the final injection valve on the RCIC system failed to open. The valve had been operated just before the test and later in the day, after the failure, was opened manually. Torque switch settings were increased and proper operation was demonstrated.

Quad Cities Unit 2 10 01 Cable Tray Fire L
ACRS Commonwealth Edison Company letter dated July 24. 1972 reports a ftre in two electrical cable trays on July 16, 1972. Initial indications of a problem appeared at about 1:30 an and a search was initiated, at about 3:30 am a small fire was discovered in two electrical cable trays in the TIF room. No smoka was detected and the fire appeared to have satinguished itself. Some remaining sparks were put out with a portable CO2 extinguisher. The fire is attributed to a high resistance joint failure at a splice fron the containment penetration. Repairs were made and a plan of corrective action for similar cables initiated.
No ACRS action is recommended.

HMH Valve Malfunction Commonwealth Edison Company letter dated Aurust 3.1972 reports the failure of the inboard primary containment isolation valve in the RCIC system. The failure was found to be a faulty torque limit switch.
The switch was replaced and normal operation restored.
