301 6 ACRS Floodian of Turbine Buildian A Compliance report dated June 12, 1972, describes an incident which occurred on June 9,1972. While the reactor was shut down, maintenance was being performed on two circulating water system reverse flow valves i

located in the discharge lines from the main condenser.

The valves closed rapidly and a rub'>er expansion joint in the 8 ft. discharge was blown o0;. All three cir-culating water pumps were operating at the time and about 600,000 gallons of river water entered the turbine building in the next five minutes-(when the system was shut down) flooding the building to a height of 14% ft.

There were no injuries and no apparent structural damage, but the RHRS Service water pumps and the cooling water pumps for two of the three station diesels were flooded out. Unit 2 was shut down at the time, but its operation would have been precluded by loss of the diesels' cooling pumps.

No activity above background was detected when the water was pumped back to the river.

4 No ACRS action is recommended at this time, although the licensee's investigation is continuing.

4, De Fire in Cable Tray.

-l ACRS Directorate of Regulatory Operations Notification of an Incident A'

'or Occurrence No. 62, dated Jely 17. 1972 reports a cable tray fire at Unit 2 on July 16, 1972. The fire was indicated by erratic indications on control room process instrumentation and by tripping of a recirculation pump. Investigation revealed a smoldering fire in a cable tray in the reactor building. The cable tray contpined 24 cables, all of which were seriously damaged. The only equipments of safety significance affected were the Recirculation Loop "B" pump suction and discharge valve control circuits.

i The cause of this fire is, so far, undetermined but preliminary information indicates that the cable tray was not over loaded.

The cause may be a high resistance mechanical connection in a power cable. The investigation continues and more reports will be forthcoming.

No ACRS action is recommended.

(JCM) 1901 H-C Set Malfunction Conunonwealth Edison Company letter dated June 26. 19.72 clarifies an incident re prted by TWK on May 24, 1972. On May 25, 1972 an essential service motor generator set failed and caused a scram. The failure has been traced to dirty relay contacts in the M-G set control circuit.

No ACRS action is recommended.

HMH Relief V alve Malfunction Commonwealth Edison Company letter dated June 29. 1972 reports that an Electromatic Relief Valve failed to open when given a manual initiation signal. The failure was traced to a stuck switch between the Holding coil and the Operating coil. The i

switch was fouled by some wires within the control box. The situat ton was corrected and other boxes inspected. Unit 1 will l

be inspected on the next opportunity to enter the drywell.
