This supplemental report stated that inspections of the Train "A" Service Water Piping have been completed revealing no generic failures of the coatings. Several design modifications are being implemented on Train "A" and it will be held out of service until these are complete. Inspection on Train "B" will not start until Train "A" is returned to service. This report notifies the NRC that we are continuing our evaluation of corrective action.

Date Letter No. SDAR # Subject N/T9/86 TXX-5027 CP-86-18 Safety Chilled Water Chiller Units Followup-Interim Report - The NRC has previously been notified of a reportable item involving starting and operating the safety chilled water chiller units. Currently, Gibbs & Hill is performing a review of vendor specifications and updating the list of components serviced by the conponent cooling water system. The supplier, York, and Westinghouse are also investigating aspects of the issue. This report notifies _ the NRC that we are continuing our evaluation of corrective action.

This report states that the deficiency is reportable and that evaluation of the issue is continuing.

Evaluation revealed that although a lack of coordination exists for several safety related loads, their circuit breakers / fuses and respective cables are in areas

- where high or moderate energy lines are not located. Therefore, their location precludes a pipe whip or jet from compromising the safe shutdown capability of the plant.

The anomalies observed were a burned wire in one inverter and burned tywraps. The burned wire was attributed to an isolated case of bad crimping by the vende . The tywraps were used during the manufacturing process and the teWratures they were exposed to during the baking discolors and weakens

Date Letter No. SDAR # Subject TXX-4966 CP-86-34 Defective Fire Stop M772/86 Installations Final Report - Issue determined to be not reportable. Deficiency a concerned exceeding the design temperature of the unexposed surface for several fire seals. Evaluation showed that the seal was tested in accordance with an ASTM E-119 fire test criteria and found acceptable.

Furthermore, the fire protection design natrix has been upgraded by the supplier and a new fire test added for further verification of accepta-bility of the fire seal in question.

cabinets had a deficiency where a fault in one DC power supply could cause the common feeder to trip and result in the loss of both OC power supplies. However, control function would still be maintained through the redundant train BOP or NSSS 7300 analog cabinets. Therefore, this is not a significant deficiency in the final equipment design and there is no breakdown in the QA program.

The issue involves two blockout fire stops which were fiistalled in a larger-than-tested configuration and was identified during a comparison of the existing fire seals and test data submitted by BISCO. Evaluation has been completed on the current design testing program and procedures submitted by BISCO to qualify the block design.

I RESPONSES TO NRC INSPECTION REPORTS /

NOTICE OF VIOLATION / NOTICE OF DEVIATION The following correspondence was issued by TUGC0 to the NRC Region IV concerning inspection reports during the period July 26 - September 25, 1986:

Date Letter No. Subject UE765/86 TXX-4992 Response to NRC Notice of Violation for Inspection Report No. 50-445/85-16 The Notice of Violation contained four items (A-0). This letter trans-mitted our additional ~ response to two (2) concerns in Item A:

1) The NRC stated concern that our response failed to provide any action taken with respect to the failure of the design review process to identify the technical inadequacy of Design Change Authorization 13349. The reponse stated that the DCA was re-reviewed and addi-tional calculations were made to show that the design change was acceptable. It is believed that the instance noted in the Violation is an isolated occurance. However, the engineering personnel involved in the review and processing of DCA 13349 will be counseled regarding the applicable design control measures.
2) The NRC stated concern with whether the reliance solely on personnel discussions provides an adequate bases for determining that the identified use of a grouted Richmond Insert was a single case. It was stated that our previous response that this issue "is not generic" was based on extensive discussions with personnel of long tenure in the structural design of CPSES. To provide additional assurance, an engineering review will be conducted involving all design changes originated within the civil engineering discipline.

Date Letter No. Subject UB7T4/86 TIF4E -- Response to NRC Notice of Violation for Inspection Report l No. 50-445/85-14 This letter transmitted a revised corrective action date for Violation 445/8514-V-01 which concerned deficiencies in the water chemistry records with respect to the sampling frequency requirements, notification of out-of-specification results, corrective action for out-of-specification results and review of acceptability of water chemistry data.

50-446/85-09 This letter transmitted a revised corrective action date regarding Violation 446/8509-V-02 which concerned the lack of site prodedures to address the prohibition of use of mercury instruments, provide craf t guidance for contamination control or provisions to assure consumables are in compliance with specification requirements.

Dalte Leller No. SubjetL M775/86 TXX-4980 Response to Enforcement -Action 86-09 Our response to EA 86-09, dated August 4, 1986, inadvertantly omitted Appendix B, Item I.B.4.a. This letter transmitted our response to this item. The violation concerned an inboard cable support assembly which was not present for EPAs 1E76, 1E77, 1E78 and 1E79 even though Quality Control inspection had verified these installations. This deviation will be corrected with the replacement of all Bunker Ramo EPA modules. Further action to avoid recurrence will include preparation of travelers for the replacement of EPA modules to clearly convey installation and inspection requirements and review of present procedures regarding the preparation of construction travelers to determine if there is a need to revise them in light of this violation.

During the 80thesda meeting of August 28, 1986, the NRC staff informed us that the NRC had internally approved N-253-2 and intended to list that revision in the next revision of Regulatory Guide 1.84. For our purposes, N-253-2 is equally acceptable.
