1.0 Purpose The purposa of this procedure is to describe the program for control and calibration of measuring and test equipment at CPSES.
2.0 Applicability l
This procedure applies to all measuring and test equipment as defined in 3.1 below, except chemistry, environmental and radiation protection equipment. Control and calibration of radiation protection equipment shall be in accordance with HPT-800, " Control and Calibration of
]
Radiation Protection Equipment." Control and calibration of chemistry j
and environmental equipment shall be in accordance with CHM-104, j
" Chemistry / Radiochemistry Quality Control Program." This procedure becomes effective when issued.

3.0 Definitions

3.1 Measuring and Test Equipment (M&TE) - Devices or systems used to a
calibrate, measure, gauge, test, inspect, or control in order to y
acquire research, development, test or operational data, or to determine compliance with design, specifications, or other f
technical requirements.
M&TE does not include permanently 1
installed operating equipment.

3.2 Calibration - Comparison of an item of M&TE with a reference j.
standard or with an item of M&TE of equal or closer tolerance to detect and quantify inaccuracies and to report or eliminate those i
inaccuracies.
3.3 Reference Standard - An item of M&TE used to calibrate other M&TE.
ii 3.4 Accuracy - Conformity of a measured value to an accepted standard value.
3.5 Quality Related Activity - Any activity which must be performed under a quality assurance program, or in other words, activities which are important to safety. For M&TE, this only includes final acceptance of an activity where accuracy is required.
3.6 Tolerance - The allowable deviation from a specified or true value.
3.7 Managed Maintenace Computer. System (MMCS) - A Maintentace Scheduling Computer similar to MODS.

4.0 Instructions 4.1 General
!i 4.1.1 The basic requirements of the M&TE Control Program are to assure that M&TE used in activities affecting quality are of the proper range, typ3, and accuracy to verify confor-mance to established requirements, that they be controlled, calibrated, adjusted, and/or maintained at prescribed intervals, and that M&TE be traceable to its l
use in an inspection, test, or calibration.
.I 4.1.2 Department or Section program procedures are not required unless more detailed instructions concerning control, use.
't document processing, etc. are desired.
4.1.3 Contractors using their M&TE in quality related activities shall have an approved QA program.
4.1.4 The Maintenance and Operations Data System data base shall be used to record, list, status, and schedule calibration of M&TE.
4.1.5 Use of the word procedure in this procedure also implies instructions.
4.2 Procurement 4.2.1 M&TE may normally be procured as a Non-Q item since the calibration of the M&TE is usually the quality related

activity and not the manufacturing of the M&TE.

.g be addressed on a case by case basis. An example of this would be hydrometers. Hydrometers do not need to be
.J recalibrated after initial receipt inspection, which includes a calibration verification. They do not require a unique tag number, but they must be identified to i
provide traceability to the receiving and verification documents.

4.3 Identification 4.3.1 Each piece of M&TE shall have a unique tag number assigned to it by the responsible organization. The tag number
-j shall be permanently affixed, engraved or marked on it, j
where practical. The carrying case may be marked where appropriate. Where carrying cases are marked, care shall j
be exercised to prevent mixing of the same type of instruments.
.l 4.3.2 The tag number shall consist of the Maintenance and Opera-tions Data System (MODS) computer code for responsible 4
organization plus a four digit identifier. The four digit identifier may be assigned at the discretion of the responsible organization.
Example: MM-0112 (Mechanical Maintenance No. 0112) 4.3.3 A MODS M&TE Data Description form shall be prepared by the responsible organization per APP-331, " MODS M&TE Data Input," and routed to Maintenance Engineering for computer input. This form need only be completed for new equipment added to the calibration program.
4.3.4 Items such as test boxes, power supplies, etc. that are not used for quantitative measurement may have tag numbers assigned and may be entered on the MODS program for the purposes of inventory, if desired.

4.4 Calibration t
l M&TE used in quality related activities shall be calibrated as described below. M&TE used only in non-quality related activities may be calibrated by any appropriate means as determined by the responsible organization. The calibration of all M&TE shall be documented in MODS by the submission of a MODS I
Worksheet or Form APP-361-1 as applicable.
4.4.1 TUCCO 4.4.1.1 M&TE calibrations performed by TUGC0 shall be accomplished using approved procedures.
These procedures shall include:
4.4.1.1.1 Identity of the item to be calibrated.

4.4.1.2.1 The " CAL STATUS" field of all data sheets shall be used to record the current (as left) status of the M&TE (ie. CAL-Q, LIMITED USE, CAL NON Q etc.).
l 4.4.1.2.2 The PASS / FAIL field of all data sheets shall be used to record the PRE CAL (as found) condition of the M&TE.
4.4.1.3 All M&TE designated " Calibrate Prior to Use" O
shall have each calibration documented. The details for documenting these calibrations shall j
be contained in the calibration procedure or instruction for that piece of M&TE.
In addi-tion, a description of a "use" shall be entered in the remarks section of the M&TE Data Sheet.
Example: For the reactor head stud tensioners and torque handles "A use is one removal and reinstallation of the i
head".
4.4.1.4 If a piece of M&TE requires setting or calibra-tion to a specific value, it must be specified on the M&TE Data Sheet prior to calibration.
4.4.1.5 All out-of-tolerance data on an M&TE Data Sheet should be circled in red so as to not be overlooked.
4.4.2 Contractor
?j 4.4.2.1 Contractors calibrating M&TE shall have an j
approved QA program. These calibrations shall j
be handled in accordance with the terms of the governing contract.
j 4.4.2.2 This contract shall require that the calibration 4
records be forwarded to TUGCO. These records 1
l shall reflect pre-cal and post-cal readings, adjustments and/or repairs made, personnel performing calibration or other authorized signature of that company, and traceability to the standards used to perform the calibration.
4.4.2.3 M&TE calibrated by a contractor off-site shall i
be receipt inspected in accordance with proce-dure VHS-001, " Receiving and Inspection of Materials, Parts and Components", upon return.

4.4.2.4 Data sheets supplied by a contract calibration j
service shall be reviewed by the responsible

organization prior to placing MATE in service.
The M&TE Calibration Service Data Sheet (similar

to Attachment 5) shall be used to document this review. The responsible supervisor or designee shall indicate approval of the data sheet by signing in the appropriate space.
4.4.2.5 M&TE being shipped to an off site contractor for calibration shall be shipped in accordance with 2
WHS-004, "Pceking & Shipping of Materials, Parts, and Components."
4.4.3 Brown & Root, CPSES 4.4.3.1 While Brown & Root maintains a metrology lab for construction at CPSES, M&TE may be calibrated by 4
them.
4.4.3.2 This arrangement is subject to periodic surveil-lance and audit by the TUGC0 QA personnel.
4.4.3.3 The responsible organization should deliver the M&TE directly to the metrology lab and pick up the calibration data sheets (B&R) with the M&TE af ter calibration. These items do not require receipt inspection.
4.4.3.4 Data sheets supplied by a Brown & Root service shall be reviewed by the responsible j
organization prior to placing M&TE in service.

4.4.4.2 A MODS Procedure Application Data Description form, APP-351-1, or its MMCP equivalent shall be i
completed for all safety related M&TE requiring periodic calibration. This shall also apply to calibrations of TUCCO safety related M&TE performed by contractors. M&TE which is designated " Calibrate Prior to Use" need not j

The responsibility for obtaining the calibration summary belongs to the specific supervisor who is responsible for maintaining the M&TE for his section or department (for example, the I&C 3
Computer Group Supervisor is responsible for all lI I&C M&TE). The summaries are printed on request. The MODS summary is requested from the Computer Services Supervisor (the Maintenance Engineer for Maintenance Department M&TE)

procedure. These stickers shall represent the current status even if different from the status shown in the MODS M&TE Data Description file since it only indicates desired status. However, MODS should be maintained as current as practical.

4.6.1 When a piece of M&TE is found to be or is suspected of being out-of-calibration, not properly maintained, or subjected to possible damage, it shall be removed from f
service and recalibrated immediately if possible. If the item is not dispositioned immediately, a Hold Tag, shown 4

4.6.3 An evaluation shall be made of the validity of previous j

correct the problem shall be recorded in the comments block of Section B.
Nonconforming conditions identified during this evaluation shall be documented on a Nonconformance Report in accordance with reference 5.3.

4.6.8 The Operations Quality Assurance Section shall periodical-ly review Deficient M&TE Evaluation Reports for trends.

I M&TE found to be out of calibration on three successive intervals shall be replaced, repaired and/or its calibra-d tion interval re-evaluated. Results of trend analysis shall be documented.
f 4.7 Control and Handling of M&TE i
1 4.7.1 Access to M&TE shall be controlled by the owning organization by the use of a check-out log nr check-out

form. The log or form shall contain, as a minimum, the information listed below. The check-out log or check-out form is considered a working document and may be destroyed I
after the next calibration of the specific M&TE if an evaluation report is not required as defined in section I
4.6.
If an ER is required, the check-our log or check-out form may be destroyed after the ER is dispositioned.

organization. If such useage is allowed, the requirements of'4.7.1 above shall be followed.
5 4.7.3 Individuals using M&TE shall be responsible for determining that the M&TE is of the proper range, type, accuracy, calibration status, and is in the proper working order for the specific application and is handled in accordance with this procedure.
4.7.4 M&TE shall be handled in a manner which precludes any adverse effect on equipment accuracy.
4.7.5 M&TE subjected to possible damage, or suspected of being damaged, shall be returned to the calibration f acility for testing and evaluation.
i 4.7.6 M&TE, hoses, test fittings, tubing, etc., used on systems that contain oil, water, potentially contaminated water, l
etc., shall be controlled as necessary to prevent cross contamination of other systems.
4.7.7 The use of M&TE containing mercury is prohibited on nuclear safety related systems and shall be restricted to

uses as directed in writing by the Manager, Plant Operations.

4.7.8 The use of M&TE containing aluminum in contact with a j
process liquid is prohibited where boron is present.
1l 4.8 STORAGE OF M&TE 4.8.1 M&TE storage areas shall provide adequate physical or edministrative separation (ie. by tagging, removal of

tags, etc.) between ready-to-use M&TE and other M&TE to prevent inadvertent use.
4.8.2 M&TE Storage areas should have adequate environmental controls to preclude any adverse effect on M&TE accuracy.
Environmental factors that should be considered are temperature, humidity, vibration, radio frequency interference, electromagnetic interference and fumes. If q
the accuracy of M&TE or Reference Standards is effected by
=
environmental conditions, compensating corrections shall be determined and applied.

- i 4.9 Reference Standards tl, 4.9.1 Standards shall be traceable to the National Bureau of Standards or other nationally recognized standards and have an accuracy of at least four times that of the item being calibrated.

4.9.2 If either of the above requirements cannot be met, then the basis for the exception shall be documented by an engineering review as follows:
-l 4.9.2.1 The department er section responsible for the reference standard M&TE shall originate a "CPSES
- I M&TE Reference Standard Calibration Evaluation" Form STA-608-6 similar to Attachment 6.
A section supervisor should sign as originator.
4.9.2.2 The originator shall complete all information in Section I of Form STA-608-6, and shall attach 3
documentation (i.e. letter from calibration service, calibration specifications of M&TE, etc.) that describes the reason for the evaluation request. The Form STA-608-6 and attachments shall be forwarded to the Results Engineer.
4.9.2.3 The Results Engineer shall assign the evaluation to an engineer in the Results section (the reviewer).
.i 4.9.2.4 The reviewer shall verify that at least one of the conditions of section 4.9.1 cannot be met, 1 3

with procedure EDA-208, " Calculation and Analysis". All EDA-208-1 forms and any supporting documenf :fon shall be attached to the STA-608-6 form by the reviewer.
4.9.2.7 The reviewer shall sign and then forward the STA-608-6 form and all attachments to the

4.9.2.8 The Results Engineer shall ensure the adequacy of the review and accuracy of all calculations and shall signify his concurrence by signing all forms. The STA-608-6 form and all attachments shall then be submitted to the Engineering j
Superintendent for management approval.
l 4.9.2.9 If the Engineering Superintendent approves the review, he shall sign all forms and return the approved STA-608-6 form and all attachments to i
che reviewer.
I 4.9.2.10 The reviewer shall determine if any special instructions (section 4.4.1.1.5) or labeling i
(section 4.5) was required for the M&TE and shall take appropriate action to ensure that the originator of the STA-608-6 review is informed of the special instructions or labeling requirements.
l 4.9.2.11 The reviewer shall then forward the approved i
STA-608-6 form and all attachments to the Records Center to be handled in accordance with STA-302, " Station Records".
4.10 Records 4.10.1 Calibration records and the Deficient M&TE Evaluation Report shall be handled in accordance with STA-302,
" Station Records."
4.10.2 Records shall be reviewed and approved by the responsible supervisor or designee prior to placing calibration i
stickers on M&TE.
I 4.10.3 Contractor records, including Brown & Root, shall be 1
attached to a TUGC0 M&TE Calibration Service Data Sheet (similar to Attachment 5) and retained in accordance with STA-302, " Station Records". The comments portion of this form should refer to the attached contractor records.

4.10.4 M&TE used for activities affecting quality shall be recorded on the controlling work document (i.e., Work N
Order, Procedure Data Sheet, etc.) in order to provide traceability required to perform deficiency evaluations.
This excludes Calibrated (Non-Q) and commercial quality items, or M&TE being used for preliminary checking only.

i 4.10.5 M&TE calibration dates and M&TE usage shall be recorded in MODS. This will be accomplished through normal processing i
of MODS Worksheets and Work Orders. When unscheduled j
calibrations are performed without a MODS worksheet, an Equipment History Data Description form shall be completed per APP-361, " MODS Equipment History Data Input," and routed to Maintenance Services for computer input.
4.10.6 Check out records or check out logs of M&TE used by other 3
organizations shall be maintained by the responsible organization until the next satisfactory calibration.
