The Inspection Report forms and checklists that will be used are delineated in the respective Quality Instructions and Procedures.

3.0 INSTRUCTION 3.1 NONCONFORMANCES AND DEFICIENCIES l Nonconformances and deficiencies will be documented and resolved per Reference 1-H.

3.2 UNSATISFACTORY CONDITIONS / ITEMS Unsatisfactory conditions are conditions which do not affect "as built" configuration, function or safety consideration and which may be corrected immediately. These shall be 62 4 860609 GARDEB6-36 ppg M%% ,

Unsatisfactory conditions or items will be denoted on the applicable Inspection Report by initialing or stamping the UNSAT block corresponding to the inspection point that was unsatisfactory. An entry, prefixed by the circled Laspec-tion point number, is required for all UNSAT items. Unsat-

When the unsatisfactory condition has been corrected, the QC Inspector will initial or stamp the corresponding SAT block on the original Inspection Report. When all inspec-tion points on an Inspection Report have been marked either SAT or NA, the Inspection Report will be closed by the Inspector's signature and date fallowing the statement "ALL items are complete and correct, r cleared by with NA entered in the blank. When any unsatisfactory coaditions or items have been transferred to a Nonconformance Report or Deficiency Report, and all other inspection points have been marked either SAT or NA, the Inspection. Report will be closed by the Inspector's signature and date and the clearing report (s) listed. ~After closure, all Inspection Reports will be transmitted to the QC Engineer for processing per paragraph 3.4.

3.3 SATISFACTORY CONDITIONS / ITEMS When all inspection points on an Inspection Report have been mark'ed either SAT or NA, the Inspection Report will be closed by the Inspector's signature and date on the closure statement with NA entered in the blank. All closed reports shall be forwarded to the QC Engineer for processing per paragraph 3.4.

l 3.4 INSPECTION REPORT PROCESSING l

! When an Inspection Report has been filled out by the Inspector, it shall be forwarded to the QC Engineer for review. After it l

has been reviewed, the QC Engineer or his designee shall number the report with the next consecutive number taken from the Inspection Report Log (Figure 1) and make an entry in the log describing the item inspected.

Reports which have not been closed per paragraphs 3.2 and 3.3

shall be copied and the original maintained in the OUTSTANDING UNSAT REPORT file in the QA Vault. Copies shall be distributed to the responsible superintendent for resolution, to 4
