j 1.0 Purpose

?

The purpose of this procedure is to establish the requirements for the Surveillance Testing Program to be used at CPSES, including responsibilities, scheduling, evaluation and reporting of results, i equipment operation, and review of surveillance tests.

2.0 Applicability This procedure is applicable to the CPSES surveillance requirements as i

specified in Section 4 of the Technical Specifications. This procedure becomes effective when issued.

3.0 Definitions 3.1 Surveillance Testing - Periodic testing to verify that safety-related structures, systems, and components continue to function or are in a state of readiness to perform their functions. For the purpose of this procedure, surveillance tests are those tests used to demonstrate compliance with the surveillance requirements of Section 4 of the Technical Specifications.

3.2 Inspection - Examination, observation, or measurement to deter-mine the conformance of materials, supplies, components, parts, appurtenances, systems, personnel, performance, procedures, processes or structures to predetermined requirements.

3.3 System - An integral part of a nuclear power plant comprising components which may be operated or used as a separate entity to perform a specific function.

3.4 Testing - Performance of those steps necessary to determine that systems or components function in accordance with predetermined specifications.

i 3.5 Data Sheet - Any paper used to document the performance or i results of a surveillance test. Data sheets may contain automat-ically recorded data, manually collected data or any computations or calculations made to arrive at the test results.

3.6 Problem Report - The form used by plant personnel for the f' internal reporting of variations from normally expected station 1

operations in accordance with STA-504, " Problem Report".

3.7 Supervisor Summary - The computer generated output of the Surveillance Tracking Program which lists surveillance tests that are due to be performed during a specified time interval.

3.9 Master Surveillance Test List (MSTL) - A controlled document which lists the procedure (s) which satisfies each surveillance requirement. ,It also contains the required frequency and applicable mode for each surveillance requirement. l 3.10 Surveillance Tracking Program - A computer based program which is used to track surveillance test requirements.

8 3.11 Test Verification Sheet - Normally a page of a surveillance procedure data package which documents supervisory approval of

the test results. The test verification sheet may be any sheet of the data package that contains, as a minimum, the required information prescribed in paragraph 4.6.4 of this procedure.

s 3.12 Surveillance Activity Report Form - A computer generated output g

of the Surveillance Tracking Program which lists the specific surveillance activities that are satisfied by a given surveillance procedure (s).

4.0 Instructions 4.1 Responsibilities 4.1.1 The Manager, Plant Operations has overall responsibility i for the station surveillance testing program.

4.1.2 The Station Operations Review Committee (SORC) shall be e

responsible for individual surveillance test procedure l review and concurrence as specified in STA-401, " Station 4

Operations Review Committee."

4.1.3 The Department Superintendents shall be responsible for I i developing and implementing procedures, instructions and

& schedules to describe and control a surveillance test

' program for those areas for which they are responsible as specified in the CPSES Operations Administrative Control and Quality Assurance Plan and in accordance with this procedure.

4.1.4 The Engineering Superintendent shall'be responsible for the overall coordination of the program to ensure that all F

surveillance requ aements are assigned to the department

4.1.9.2 Obtaining the Shif t Supervisor's permission prior to performing a surveillance test affecting permanent plant equipment.

4.1.9.3 Using written approved procedures for the performance of any surveillance testing.

4.1.9.4 Informing the Shif t Supervisor of any off-normal operating condition encountered during testing.

4.1.9.5 Informing the Shift Supervisor when testing is 1 complete and the system is restored to its pre-test condition.

4.2 Surveillance Scheduling 4.2.1 A Master Surveillance Test List (MSTL) is a listing that l shall include all station surveillance requirements

This listing shall contain, as a minimum, the following items for each surveillance test requirement:

4.2.2 The MSTL shall be maintained as a controlled document.

The Results Engineer shall be responsible for approving

,1 revisions to the MSTL as required, and distribution of the j MSTL shall be maintained by the CPSES Records Supervisor.

The MSTL shall be reviewed at least annually by the 9

Results Engineer.

, 4.2.3 The tracking of surveillance requirements which have a scheduled performance intervals greater than seven days lj shall be accomplished by the Surveillance Tracking Program. This program shall be established and maintained in accordance with EDA-109, " Surveillance Tracking

$ Program."

, 4.2.4 Supervisor Summaries shall be distributed to Supervisors l at least weekly (for surveillance tests du? during the succeeding week) and at least monthly (for surveillance tests due during the succeeding 90 days).

4.2.5 Planner Summaries shall be distributed at least weekly.

l 1 4.2.6 Departmental procedures, instructions, and schedules shall

! specify methods for ensuring the completion of all surveillance tests. Each supervisor shall schedule the performance of all surveillance tests assigned to his section, interfacing with the station planning organization as necessary, to ensure his tests are y integrated with other planned activities.

4.3 Performance of Surveillance Tests 4.3.1 Each supervisor shall ensure that surveillance tests l assigned to his section are performed prior to the violation date of the surveillance as specified on the a Supervisor Summary or in departmental schedules.

jurisdiction of the Operations Department.

, 4.3.4 Surveillance tests shall be performed as required following plant modifications, significant maintenance and a repairs, or significant changes in operating procedures to i confirm that the modifications, maintenance, repairs, or 4

4.3.7 Testing of all permanent plant equipment shall be conducted within the applicable limits of the Technical i Specifications.

action shall be followed and the matter shall be referred to the Manager, Plant Operations or his designated representative for resolution.

4.4 Review and Approval of Test Results

]

4 4.4.1 Upon completion of a surveillance test the responsible m

Supervisor shall review and evaluate the test results and shall approve the results by signing the completed test verification sheet and any appropriate surveillance test i documentation. Additionally, if it is provided by Results Engineering, a surveillance activity report form shall be j

completed and attached to the test data package.

  • 4.4.2 The Supervisor shall transmit the original test verification sheet with any data sheets used to document

' the performance or results of the surveillance test and the surveillance activity report form (if completed) to the Results Engineer.

4.4.3 The Results Engineer shall ensure that the Surveillance Tracking Program is updated. He shall then ensure that i

the test verification sheet and all appropriate data sheets are reviewed and that information required for

.I analyzing performance trends or for preparing reports is extracted. If, for any reason, a discrepancy is noted

.' during this review, the responsible Section Supervisor

'! shall be notified. The Results Engineer shall then sign r the test verification sheet.

4.4.4 The Results Engineer shall then transmit the test verification sheet and all data sheets to the Records i

.j Center for retention in accordance with Procedure STA-302

l 4.5 Failure to Perform or Meet Surveillance Test Acceptance Criteria

,j 4.5.1 The Shift Supervisor shall be immediately notified and a Problem Report shall be initiated in accordance with I, STA-504 in any of the following circumstances:

4.5.1.1 An operating characteristic or condition is verified to be inconsistent with the acceptance criteria or other provisions of the test procedure.

4.5.1.2 A component fails or malfunctions during the test.

i j 4.5.1.3 Test prerequisites, conditions, or requirements j have not been met.

4.5.1.4 A surveillance test has not been performed within the required time frequency.

., 4.5.2 Discrepancies noted during the surveillance test, or which prevent the performance of the surveillance test, shall be noted on the test verification sheet. These discrepancies shall include unusual or unexpected results, unacceptable

[ results, problems with test equipment, discrepancies in the procedure under which the test was performed, or any condition which might invalidate the test results.

4.5.3 Corrective action taken based on the discrepancies noted shall be recorded on the test verification sheet and shall include actions taken by the person conducting the test, such as retest upon completion of corrective action, and actions taken by the Shift Supervisor, if any, as a result 4

of the discrepancies.

4.6 Surveillance Test Procedures 1

' accordance with written approved procedures.

} detailed step-by-step instructions or a refer-ence to applicable sections, paragraphs, or pages in another approved procedure, instruc-e tion, or reference document that provides details for performing the required manipula-

~{ tions of station equipment and verifying their proper functioning by comparison of either i

recorded data to the acceptance criteria. For each step in this section, where applicable, there should be a space provided for the person performing the test to initial indicating that the step has been completed and/or understood.

4.6.3.12 Restoration and Retesting. This section should contain the required steps to return the system or component to the original conditions that

contain a statement as to when retesting is required. After it has been determined that no further testing is required; i.e., the results are satisfactory, instructions should be provid-ed for the return of the station equipment under test to operational status.

(, 4.6.3.13 Computation. This section should provide instructions for the performance of any calcula-d tions or data reduction required to arrive at ll the end result.
