Areas Inspected: Routine, unannounced inspection of operations, safety committees, internal review and audit, facility modifications, maintenance and followup on items of noncompliance.

Results: Of the six areas inspected, no items of noncompliance or deviations were identified in five areas; one apparent item of noncompliance (infraction -

(78-12-01) - failure to comply with established and posted SNM unit spacing requirements to control interaction paragraph 5.) was identified in one area.

The licensee is providing drainage slits for plastic wrapped fuel assemblies in conformance with procedural requirements (paragraph 5.c.).

3. Regulatory Compliance Connittee (RCC)
a. The inspector reviewed meeting minutes and support documentation for RCC meetings held on June 28, July 18 and August 10, 1978.

Meeting minutes verify Committee evaluation of the facility safety program, control of airborne activity, federal regulatory changes and facility modifications. Committee membership and meeting frequency meet or exceed requirements of License Condi-tion 19.

b. Membership for the ALARA Committee is the same as for the RCC.

An ALARA review was held on August 25, 1978, to review facil2ty performance for the first six months of 1978. A draft report was-prepared for the plant manager reviewing employee exposures, effluent release data and review of audit and inspection findings as required by License Condition 21. The inspector was informed that the report is scheduled for completion in final form by November 1, 1978.

c. No items of noncompliance or deviations were identified.
4. Internal Review and Audit
a. Audit records verify performance of radiation protection and nuclear criticality safety inspection of plant operations every two weeks for the period June 1 to September 1, 1978. This frequency exceeds the monthly frequency specified in Section 2.3.2.3 of the license application. Inspections are made in accordance with a written plan as required by License Condition 43.

Inspection and review records include safety evaluations of pro-posed modifications and records of unusual operating incidents including followup actions.

b. License Condition 44 requires safety review and surveillance of equipment or mode of operation changes significant to safety prior to and following initiation of the changes. Nuclear crits-cality and radiation safety review records document reviews prior to operation. Based on interviews with safety personnel and
project engineers, the inspector obtained assurance that safety surveillance is provided following initial operations although no formal records are maintained. Management representatives stated that a system for recording such inspections would be developed.

5. Operations
a. During tours of the operating and storage areas of the plant the inspector verified that operating procedures were available to the operators and that the procedures were the current revisions.

Procedural and safety criteria requirements were discussed with several operators,

b. The inspector identified a failure to maintain 12" spacing between suberits as required by criticality requirements and posted criticality control signs. .Three polypaks in a scrap recovery hood had less than 1" separation. Licensee representatives were informed that this constituted an item of noncompliance.
c. Verification was made that corrective actions had been completed to assure that drainage slits are provided in plastic wrapped fuel assemblies. A revised procedure requires verification by the area supervisor. This item is closed.
d. The UFs vaporization system and incinerator installation were inspected for conformance with equipment, centrols and safety interlock requirements specified in Sections 2.2.11 and 2.2.12 of the license application.
6. Facility Changes and Modifications
a. Management control of facility changes is maintained through use of division and facility procedures that require multiple review and approvals prior to installation. Facilities Planning prepares a conceptual design and appropriation request for management review. Detailed design and construction drawings require a series of reviews and approvals as defined in hTD Plant Procedure FP-102 " Drawing Approvals". The design package is approved by all affected groups prior to installation. Facilities Engineering prepares the operating procedures for the installed equipment.
b. The following facility change records were audited for required approvals and safety evaluations: ,

7. Maintenance ,
a. Maintenance activities are controlled by use of a maintenance work order system as defined in plant Procedure SY-509 "Mainte- ,

nance Work Order and Permit" The work order includes checkoff of safety items required and special air sample or bioassay requirements. The work order may specify requirements for a safety observer, tank entry permit or cutting / welding permit.

Maintenance Procedure SY-503 " Equipment Safety Lock-out and Tagging Procedure" is followed when applicable.

b. Several completed work order records were checked for conformance to procedural requirements and instructions and no items of nonconformance were identified.
c. Preventive maintenance and instrument calibration activities were discussed with Maintenance Department representativ . The inspector was informed that instrument PM and calib,ation schedules have recently been entered as a computer program. Instrument calibrations are performed as specified in Procedure FP-200

" Industrial Instrument Calibration" and by qualified instrumeni technicians. ,

d. Records and calibration stickers for ten safety-related instruments were audited for conformance to procedural and frequency require-ments. No items of noncompliance of deviations were identified.

8. Exit Interview The inspector met with licensee representatives (denoted in paragraph 1) on September 22, 1978. The inspector summarized the scope and findings of the inspection. Findings related to the one apparent item of non-compliance and corrective actions relating to previously identified items of noncompliance were discussed in detail.
